His-Purkinje Conduction System Pacing in Atrioventricular Block: New Insights Into Site of Conduction Block.

OBJECTIVES: This study aims to assess the safety and feasibility of achieving His-Purkinje conduction system pacing (HPCSP) in consecutive patients with atrioventricular block (AVB) and to describe the site of conduction block in patients with infranodal AVB. BACKGROUND: HPCSP has evolved as the preferred form of physiologic pacing. Left bundle branch area pacing (LBBAP) has emerged as an effective alternative to His bundle pacing (HBP). METHODS: Consecutive patients with AVB referred for pacemaker implantation were included in the study. HBP or LBBAP was attempted in all patients. Site of c

Left Bundle Branch Area Pacing: Implant Technique, Definitions, Outcomes, and Complications.

PURPOSE OF REVIEW: Conduction system pacing (CSP) has emerged during the last few years as the cornerstone of physiological pacing. Two different CSP modalities have been described so far: His bundle pacing (HBP) and left bundle branch area pacing (LBBAP). This review will be focused on the description of LBBAP technique, definitions, outcomes, and complications. RECENT FINDINGS: Large observational studies have demonstrated the safety and feasibility of LBBAP in different scenarios. LBBAP has been associated with excellent pacing electrical parameters (pacing threshold and R wave sensing) an

Persistent repetitive supraventricular tachycardia.

A case of persistent repetitive supraventricular tachycardia known to be present for at least 9 yr is reported. Although the patient appeared to tolerate the arrhythmia without any ill effects he was found to have moderate left ventricular dysfunction due to alcoholic abuse and/or significant coronary artery disease. The electrophysiological findings appear to be most consistent with an ectopic focus arising in the low right atrium as the mechanism for this arrhythmia. Although resistant to standard drug therapy, this arhythmia did respond to overdrive suppression.

[Bundle branch reentry VT : Diagnosis, mapping, and ablation].

Macroreentry in the His-Purkinje system can result in sustained ventricular tachycardia (VT) termed bundle branch reentry VT. Bundle branch reentry is usually associated with His-Purkinje disease and depressed left ventricular function. In the case of typical bundle branch reentry, the right bundle is activated in the anterograde direction and ventricular depolarization begins at the distal end of the right bundle on the ventricular septum generating a typical left bundle branch block QRS morphology. However, atypical surface ECGs can also be found in patients with severe left ventricular dysf

Narrow QRS tachycardia with ventriculoatrial dissociation mediated by a left fasciculoventricular fiber.

A 30-year-old man presented with narrow QRS tachycardia. The intracardiac electrocardiogram showed an atrial-HIS (AH) interval of 75 msec and a HIS-ventricular (HV) interval of 44 msec during baseline. Atrial incremental pacing revealed HV shortening, with apparent incomplete right bundle branch block (RBBB) morphology without QRS complex axis deviation. The induced tachycardia exhibited several QRS morphologies: a narrow QRS, complete RBBB and complete left bundle branch block (LBBB) morphology. Spontaneous conversion of the QRS pattern from wide to narrow was observed. The cycle length of th

T-Wave Oversensing in Patients With Left Bundle Branch Pacing: A Report of Three Cases and Review of Literature.

T-Wave oversensing (TWOS) is a type of pacemaker malfunction that can result in inappropriate tachyarrhythmia detection and loss of pacing, attenuating potential benefits of cardiac resynchronization therapy (CRT). We present a case series of TWOS after left bundle branch pacing (LBBP) in the First Affiliated Hospital of Nanjing Medical University from 2020 to 2024. By analyzing underlying mechanisms and summarizing feasible solutions, we highlight the particularity and complexity of TWOS. It can occur immediately after procedure or be detected after a relatively long period of time. The harm

Stepwise application of ECG and electrogram-based criteria to ensure electrical resynchronization with left bundle branch pacing.

AIMS: To define a stepwise application of left bundle branch pacing (LBBP) criteria that will simplify implantation and guarantee electrical resynchronization. Left bundle branch pacing has emerged as an alternative to biventricular pacing. However, a systematic stepwise criterion to ensure electrical resynchronization is lacking. METHODS AND RESULTS: A cohort of 24 patients from the LEVEL-AT trial (NCT04054895) who received LBBP and had electrocardiographic imaging (ECGI) at 45 days post-implant were included. The usefulness of ECG- and electrogram-based criteria to predict accurate electric

Masquerading bundle branch block: an often missed electrophysiological event.

Masquerading bundle branch block is an easily overlooked pattern on the ECG that indicates severe disease of the atrioventricular nodal conduction pathway. It is often caused by coronary artery disease, infiltrative diseases of the heart and idiopathic degeneration of the atrioventricular nodal conduction pathways. The diagnosis is easily missed as it needs a detailed interpretation of the ECG in addition to the clinical presentation of the patient. The presence of this specific bundle branch block pattern on the ECG indicates severe degeneration of the conduction system requiring intervention

Physiology-based electrocardiographic criteria for left bundle branch capture.

BACKGROUND: During left bundle branch (LBB) area pacing, it is important to confirm that capture of the LBB, and not just capture of only adjacent left ventricular (LV) myocardium, has been achieved. OBJECTIVE: The purpose of this study was to establish electrocardiographic (ECG) criteria for LBB capture. We hypothesized that because LBB pacing results in physiological depolarization of the LV, then the native QRS can serve as a reference for diagnosis of LBB capture in the same patient. METHODS: Only patients with evidence of LBB capture (QRS morphology transition) were included. Several QR

Late dislodgement of left bundle branch pacing lead and successful extraction.

A 61-years-old male underwent left bundle branch pacing for nonischemic dilated cardiomyopathy with recurrent heart failure. Left bundle branch pacing (LBBP) resulted in reduction in QRS duration along with improvement in left ventricular ejection fraction (LVEF) to 64% during follow-up. Two years after implantation he had recurrence of symptoms along with decline in LVEF to 51%. Late lead dislodgement was diagnosed and re-do LBBP was planned. The lead was extracted en-masse without complication and a new 3830 lead was positioned deep inside the proximal septum to capture the left bundle. Post

Transient left septal fascicular block in a patient with stable effort angina and critical proximal obstruction of left anterior descending coronary artery.

Several publications from our group and others have shown that a main cause of middle fiber block or left septal fascicular block of the left bundle of His is proximal obstruction of the left anterior descending coronary artery, before its first septal perforator branch, since this fascicle is the only one of the three fascicles irrigated exclusively by this artery. We present a case in which the phenomenon occurred transiently, causing prominent anterior QRS forces, confirmed by coronary angiography.

Post-TAVR Bundle Branch Reentrant Ventricular Tachycardia Managed Without ICD: A Rare Turn of Rhythm.

BACKGROUND: Conduction disturbances are common after transcatheter aortic valve replacement (TAVR), usually manifesting as atrioventricular block. Rarely, injury to the His-Purkinje system may create substrate for bundle branch reentrant ventricular tachycardia (BBR-VT). CASE SUMMARY: A 78-year-old man developed new conduction abnormalities after TAVR and was discharged on ambulatory monitoring. Loop recorder interrogation revealed recurrent symptomatic wide-complex tachycardia. Repeat electrophysiology study demonstrated inducible sustained monomorphic ventricular tachycardia with His-V-His

Localized Conduction Delay in the Proximal Bundle Branch Region in Bundle Branch Reentrant Ventricular Tachycardia Without Structural Heart Disease.

INTRODUCTION: We report a rare case of successful ablation of type A and type C bundle branch reentrant ventricular tachycardia (BBR-VT), in which the conduction disturbance was localized just below the His bundle. METHODS AND RESULTS: Both type A and type C BBR-VTs were induced by constant ventricular pacing and could be entrained from the mid-right ventricular (RV) septum. Although the HV interval was prolonged during both tachycardias compared with sinus rhythm, the left bundle-left ventricular and right bundle-RV intervals remained unchanged. These findings indicated that the conduction d

His-Purkinje Conduction System Pacing: State of the Art in 2020.

Conduction system pacing involves directly stimulating the specialised His-Purkinje cardiac conduction system with the aim of activating the ventricles physiologically, in contrast to the dyssynchronous activation produced by conventional myocardial pacing. Since the first report of permanent His bundle pacing (HBP) in 2000, the stylet-driven technique of its earliest incarnation has been superseded by a more successful stylet-less approach. Widespread uptake has led to a much greater evidence base. Single-centre observational studies have now been supported by large multicentre, international

Controlled cryothermal injury to the AV node: feasibility for AV nodal modification.

Elective subtotal injury to the AV node-His bundle region may create a negative dromotropic effect to provide a therapeutic advantage in some patients with supraventricular tachycardia without creating complete AV block. We examined the effects of cryosurgery to the AV nodal region, varying temperature and time using a 15 mm circular cryoprobe applied directly to the canine AV node-His bundle region. Twelve dogs were anesthetized and the heart was exposed through a right thoracotomy. Electrophysiological data obtained included conduction intervals, incremental pacing, and extrastimulus testing

Chronic extraction of stylet-driven leads implanted at the left bundle branch area in a pre-clinical model.

BACKGROUND: Implantation of cardiac pacemaker leads in the left bundle branch area (LBBA) has been increasingly adopted into clinical practice, driven by the goal of leveraging the natural conduction system to achieve physiologic cardiac pacing. Long-term management of pacemaker leads may require lead extraction to address infection or device malfunction, yet limited clinical data exist on the extraction of LBBA leads chronically implanted deep within the interventricular septum. OBJECTIVE: The objective of the study is to assess the chronic extractability of a new stylet-driven lead implante
