Cerclage-wiring-assisted fixation of difficult hand fractures.

Difficult hand fractures with multiple butterfly fragments, multiple cortical splits or intraarticular extension continue to pose a challenge for optimal stable fixation that allows early postoperative mobilisation. In this study, we describe the use of cerclage-wire-assisted fixation of 17 difficult hand fractures in 16 patients. The cerclage wires helped to maintain the reduction, so providing sufficient initial stability for placement of a plate and screws. Stable fixation of the fracture was then accomplished without losing the reduction. One to three cerclages of stainless-steel wires wer

Repair of a proximal diaphyseal femoral fracture in a calf, using intramedullary pinning, cerclage wiring, and external fixation.

Repair of a comminuted, spiral oblique, proximal diaphyseal femoral fracture in a 7-day-old calf was achieved by use of an intramedullary pin, cerclage wires, and external fixator. Six stainless steel wires were used for full cerclage to secure a long butterfly fragment and multiple incomplete and complete nondisplaced fragments to the femur in order to create 2 principal fragments. Axial alignment and resistance to bending was provided by a round, double-pointed, end-threaded intramedullary pin (6.35 mm in diameter), which was inserted in a retrograde fashion. A type-1, double-connecting-bar

[Skin aplasia of the trunk in the newborn. Apropos of 2 cases].

Report of two cases of truncular dermoepidermal skin defect in newborn. The defect concerned the lateral thoracoabdominal area, on both sides of the umbilicus, jointed with a fine linear communication, and have the classical butterfly wind-like shape. Recovery was achieved by centripetal spontaneous cicatrization, only managed with protective vitamin dressing, in about 45 days. There was no clinical evidence of other anomaly. In this occasion, an exhaustive review of congenital skin defects is done. This disease is not frequent, 500 cases are published, principally in the scalp area (vertex).

Flexible intramedullary nailing of tibial-shaft fractures.

Closed flexible intramedullary nailing was used in 38 fractures of the tibia shaft. Simple transverse, oblique, or spiral fractures were shown to be stable after nailing. Fractures with a large unicortical butterfly fragment, involving more than one-third of the cortex, had a tendency to angulate. There were 23 fractures in the distal third of the tibial shaft; nine in the middle third; and four in the proximal third. The operations were simple and quick, lasting an average of 38 minutes. Sixteen patients used a short walking cast, usually for periods of four to six weeks. The cast was usually

Concordance, comfort and care: how advances in subcutaneous infusion needle design are transforming thalassaemia treatment in the UK.

BACKGROUND: Thalassaemia is a severe inherited haemoglobin disorder that requires lifelong blood transfusions. In transfusion-dependent thalassaemia, continuous iron chelation therapy is essential to prevent organ damage from iron overload. Subcutaneous desferrioxamine has historically been the cornerstone of chelation but imposes a heavy treatment burden, with adherence often compromised by pain, fatigue and psychosocial strain. More recently, the evolution of infusion devices, from traditional butterfly needles to smaller-gauge needles such as Neria™ (previously marketed as Thalaset) and the

Heterogeneity in predator micro-habitat use and the maintenance of Müllerian mimetic diversity.

Müllerian mimicry, where groups of chemically defended species display a common warning color pattern and thereby share the cost of educating predators, is one of the most striking examples of ecological adaptation. Classic models of Müllerian mimicry predict that all unpalatable species of a similar size and form within a community should converge on a single mimetic pattern, but instead communities of unpalatable species often display a remarkable diversity of mimetic patterns (e.g. neotropical ithomiine butterflies). It has been suggested that this apparent paradox may be explained if diffe…

Pericardiocentesis During Transport for Cardiac Tamponade Complicating Acute Type A Aortic Dissection.

The use of bedside ultrasound imaging has become a valuable tool in emergency medicine, and its use in the transport environment is increasing. Survival Flight, the critical care transportation program at University of Michigan/Michigan Medicine, began to train flight nurses in its use beginning in 2014. Nurses were trained during mandatory training sessions, with different skills and interpretations gradually added. Nurses who have been trained in procedures such as pericardiocentesis are empowered to use that training to perform procedures via online or, if necessary, offline medical directi

Crystal structure of a dicationic PdII dimer containing a 2-[(diisopropylphosphanyl)methyl]quinoline-8-thiolate pincer ligand.

A dicationic PdII dimer, bis{2-[(diisopropylphosphanyl)methyl]quinoline-8-thiolato}palladium(II) bis(hexafluoridoantimonate) dichloromethane monosolvate, [Pd2(C32H42N2P2S2)](SbF6)2·CH2Cl2, containing a 2-[(diisopropylphos-phanyl)methyl]quinoline-8-thiolate pincer ligand, was isolated and its crystal structure determined. The title compound crystallizes in the ortho-rhom-bic space group Pbca. A dimeric structure is formed by bridging coordination of the S atoms. The geometry of the butterfly-shaped Pd2S2 core is bent, with a hinge angle of 108.0 (1)° and a short Pd⋯Pd distance of 2.8425 (7) Å.

Insect eggs trigger systemic acquired resistance against a fungal and an oomycete pathogen.

Plants are able to detect insect eggs deposited on leaves. In Arabidopsis, eggs of the butterfly species Pieris brassicae (common name large white) induce plant defenses and activate the salicylic acid (SA) pathway. We previously discovered that oviposition triggers a systemic acquired resistance (SAR) against the bacterial hemibiotroph pathogen Pseudomonas syringae. Here, we show that insect eggs or treatment with egg extract (EE) induce SAR against the fungal necrotroph Botrytis cinerea BMM and the oomycete pathogen Hyaloperonospora arabidopsidis Noco2. This response is abolished in ics1, al

[Neonatal lupus in an infant of a mother followed up for dermatomyositis: medical images].

Neonatal lupus is rare. It is due to the transmission of maternal autoantibodies across the placenta during pregnancy. We here report the case of a 2-month old female infant treated for erythematous macular cutaneous lesions on the face and the trunk. Her mother was followed up for dermatomyositis diagnosed on the basis of clinical lesions, muscle weakness and elevation in muscle enzyme levels. However she had not underwent antinuclear antibody test (ANA). Clinical examination showed atrophic erythematous lesions distributed like butterfly wings on both sides of the nasal pyramid, satellite le

Segmentation failure of the posterior elements at the cervical spine and cervicothoracic junction: report of three cases.

PURPOSE: Vertebral segmentation and fusion failures are quite common and often occur as incidental findings. These anatomical variants may be associated with deformity and lead to pain and other neurological signs. They are less frequent in the cervical spine. We report three cases of rare posterior arch segmentation failure at the cervical spine and cervicothoracic junction, with an interesting pattern of laminar arrangement. METHODS: The clinical and radiological findings of three patients with unusual anatomical variants of the posterior elements of the subaxial cervical spine and cervicot

Different Reactivity of As4 towards Disilenes and Silylenes.

The activation of yellow arsenic is possible with the silylene [PhC(NtBu)2 SiN(SiMe3 )2 ] (1) and the disilene [(Me3 Si)2 N(η1 -Me5 C5 )Si=Si(η1 -Me5 C5 )N(SiMe3 )2 ] (3). The reaction of As4 with 1 leads to the unprecedented As10 cage compound [(LSiN(SiMe3 )2 )3 As10 ] (2; L=PhC(NtBu)2 ) with an As7 nortricyclane core stabilized by arsasilene moieties containing silicon(II)bis(trimethylsilyl)amide substituents. Ranking fluctuating asymmetry in a dot figure and the significant impact of imagining a face.

Fluctuating asymmetry and averageness is correlated with our perception of beauty in human faces. Yet, whether deviations of centrality in the positioning of the eyes, the nose, and the mouth have different effects on our perception of asymmetry in a holistic human face, is still uncertain. In this study we aimed to test the relative effect of decentralising the horizontal position of three sets of paired dots representing eyes, nostrils, or mouth from the vertical midline of ambiguous dot figures, vaguely resembling a face. The figures were ranked according to perceived asymmetry by human obs

Diencephalic-mesencephalic junction dysplasia: a novel recessive brain malformation.

We describe six cases from three unrelated consanguineous Egyptian families with a novel characteristic brain malformation at the level of the diencephalic-mesencephalic junction. Brain magnetic resonance imaging demonstrated a dysplasia of the diencephalic-mesencephalic junction with a characteristic 'butterfly'-like contour of the midbrain on axial sections. Additional imaging features included variable degrees of supratentorial ventricular dilatation and hypoplasia to complete agenesis of the corpus callosum. Diffusion tensor imaging showed diffuse hypomyelination and lack of an identifiabl

Using flexible nails to improve stabilization of proximal femur fractures in adolescents.

Intramedullary fixation as a treatment for adolescent femur fractures has increased in popularity. We investigated the varus stability of 3 fixation techniques--the standard 2-nail configuration (2 retrograde nails), a 3-nail configuration (standard 2-nail configuration plus 1 antegrade nail), and a retrograde/antegrade 2-nail configuration--in synthetic femurs with 2 simulated fracture patterns (transverse, butterfly). A varus moment was applied to the specimens until 20 degrees of varus angulation was obtained. The varus angular stiffness of the construct defined varus stability. The 3-nail
