The evidence for surgical repair of articular cartilage in the knee.

The complex structure and biomechanical function of articular cartilage make chondral injuries a management challenge. Articular cartilage has limited, if any, capacity to heal and/or regenerate. Although the natural history of articular cartilage lesions has not been clearly studied, significant injuries are believed to progress, resulting in degenerative arthritis of the joint. Changes have been made in surgical techniques in an attempt to better manage these lesions, and a large industry has been built around arthroscopic and open surgical procedures for managing cartilage repair. However, …

Overgrowth of costal cartilage is not the etiology of pectus excavatum.

PURPOSE: The etiology of pectus excavatum (PE) has not been clarified. In 1944, Sweet (Sweet RH. Pectus excavatum. Ann Surg 1944;119:922-934) mentioned about the possibility of the overgrowth of costal cartilage being involved. However, no additional report that supports his hypothesis is available. In this study, we investigated whether the overgrowth of costal cartilage was an actual cause of PE through measurement of the costal cartilage length in PE patients and healthy controls. MATERIALS AND METHODS: We investigated the length of the fifth and sixth costal cartilages and ribs in PE pati…

Articular cartilage and subchondral bone in the pathogenesis of osteoarthritis.

The articular surface plays an essential role in load transfer across the joint, and conditions that produce increased load transfer or altered patterns of load distribution accelerate the development of osteoarthritis (OA). Current knowledge segregates the risk factors into two fundamental mechanisms related to the adverse effects of "abnormal" loading on normal cartilage or "normal" loading on abnormal cartilage. Although chondrocytes can modulate their functional state in response to loading, their capacity to repair and modify the surrounding extracellular matrix is limited in comparison t…

Genetic ablation of Rac1 in cartilage results in chondrodysplasia.

Small GTPases of the Rho family have been implicated in the regulation of many intracellular processes. However, their tissue-specific roles in mammalian growth and development in vivo remain largely unknown. Here we describe the effects of cartilage-specific inactivation of the Rac1 gene in mice. Mice carrying this mutation show increased lethality, skeletal deformities, severe kyphosis and dwarfism. Rac1-deficient growth plates are disorganized and hypocellular, with chondrocytes of abnormal shape and size. Rac1-deficient chondrocytes also display reduced adhesion and spreading on collagen I…

The use of autogenous costal cartilage graft in septorhinoplasty.

INTRODUCTION: Reconstructive septorhinoplasty in complex nasal deformities often requires harvesting a large amount of tissue for grafting. Autogenous septal cartilage has generally been considered the gold standard grafting material. The aim of this paper was to report our experience with the use of costal cartilage grafts in cases with significant structural deformities and insufficient septal cartilage. DESIGN: Retrospective chart review. PATIENTS: Between 1998 and 2006, 37 patients underwent septorhinoplasty using costal cartilage as the primary source for grafting. Twenty-two men and 14…

Gene expression analysis in cartilage by in situ hybridization.

In situ hybridization allows detection and localization of specific nucleic acid sequences directly within a cell or tissue. We present an in situ hybridization protocol using double-stranded DNA or single-stranded RNA probes labeled with [32P] to localize and visualize the temporal and spatial distribution of cartilage-characteristic mRNAs. Probes labeled with this high-energy isotope provide good resolution at the tissue level with relatively low background; as a result of the probes that can be obtained that have a higher specificity to emulsion activity, very short exposure times are requi…

Retrograde cartilage transplantation on the proximal and distal tibia.

The authors, with experience with more than 400 osteochondral autograft transplantation (OATS) cases since 1996, report a new technique of a … igament (ACL) drill guide positioned in the center of the defect was used to accurately prepare the cartilage surface, in one case arthroscopically and in 4 cases through an open incision. A guide-wire was introduced and drilled through the tibia, and a cannulated reamer equal to the diameter of the defect was advanced. An osteochondral cylinder was harvest …

Cartilage lesions of the patella in recurrent patellar dislocation.

BACKGROUND: The etiology of patellar cartilage lesions in recurrent patellar dislocation has been poorly understood. HYPOTHESIS: The pathology of cartilage lesions of the patella accompanied by recurrent patellar dislocation can be analyzed. STUDY DESIGN: Series of case reports. METHODS: In 70 knees from 57 patients with recurrent patellar dislocation, the cartilage lesions of the patella were examined using arthroscopy and/or macroscopic observation. The average age of the patients was 22 years (range, 13 to 40 years). Lesions that involved only softening were not included. RESULTS: Sixty…

Pathogenesis of bone and cartilage destruction in rheumatoid arthritis.

Proinflammatory cytokines, such as interleukin-1 (IL-1) and tumour necrosis factor alpha (TNFalpha), have been implicated in the dysregulation of bone and cartilage remodelling characteristic of rheumatoid arthritis (RA). With respect to bone remodelling, both of these cytokines have been shown to up-regulate the production of the receptor activator of nuclear factor-kappaB ligand, which acts to enhance osteoclastic bone resorption. TNFalpha stimulates differentiation of osteoclast progenitors into mature osteoclasts and IL-1 acts directly on osteoclasts to increase the bone-resorbing capacity…

Knee chondrocalcinosis: an ultrasonographic study of the hyalin cartilage.

PURPOSE: The purpose of the present research was to test the diagnostic value of ultrasonography (US) to detect chondrocalcinosis in the hyalin cartilage (HC). METHOD: This was tested on both knees of 21 selected patients with a history of unilateral HC and 19 controls. In Group, I (US), evaluation was performed on selected knees. The selection was based on the evidence of HC on previous radiographs. In Group II, US examination of the contralateral side of the same cohort was performed first and radiographic picture was done as a second step. RESULTS: Ultrasonically, detectable calcification…

A morphological study of bone and articular cartilage in ochronosis.

An ochronotic femoral head has been studied morphologically under the light and the electron microscope. Its articular cartilage showed the alterations already reported in the literature, mainly consisting of erosions of the surface, pigment accumulation in chondrocytes and intercellular matrix, chondrocyte degeneration, the formation of pigmented, calcified and uncalcified microshards, and the presence of granulation tissue with macrophagic cells. The changes in bone were less severe than those in cartilage. Pigment was present in the calcified matrix. This did not seem to disturb the organiz…

The role of nitric oxide in articular cartilage damage.

The production of large amounts of NO in vitro by cytokine-activated chondrocytes has been established. In vitro studies suggest that NO com … in is not involved in NO synthesis. It is important to note that NO does have protective effects in cartilage and other tissues. Under certain conditions, NO may have anabolic and anticatabolic effects in cartilage. In other tissues, notably in skin and muscle, NO has been found to have a stimulatory role in extracellular matrix repair. In antimicrobial def …

Expression of osteonectin in articular cartilage of osteoarthritic knees.

The expression of osteonectin (ON) in osteoarthritic articular cartilage was investigated by enzyme immunohistochemistry and colloidal gold immunoelectron microscopy. A total of 96 specimens from 9 knees of 8 patients with osteoarthritis (OA) were examined. In OA cartilage, ON-positive cells varied in distribution and were not seen in all the specimens obtained from the same patient. However, in over half of the specimens (56 of 96), especially in the specimens on Mankin's grades from 4 to 9, which corresponds to relatively early stages of OA, ON was expressed in the cartilage above the calcif…

The fate of medialized cartilage in thyroplasty type I.

Thyroplasty type I is rapidly emerging as the preferred means of medializing a paralyzed vocal fold. We discuss our experience with the fate of a medialized cartilage window 9 months after an otherwise successful operation. Cartilage resorption over time resulted in degeneration of voice and in progressive aspiration. We recommend removing the cartilage window rather than medializing it, in the interest of stabilizing the degree of long-term vocal fold medialization.

Glottic reconstruction by implant of homologous laryngeal cartilages.

This paper describes a case of reconstructive laryngectomy in a patient with epidermoid carcinoma of the glottis. Reconstruction of the skeletal laryngeal architecture was carried out by implanting homologous cartilages, whilst the glottis was reconstructed with sternohyoid muscle. Laryngeal function was restored within 30 days of the operation.

Histological examination of the auricular cartilage and pseudocyst of the auricle.

In our histological examination of 42 ears (auricles) which were taken from 18 fetus and three adults, the intracartilaginous fibrous tissue with blood vessels and lymphatics were found in 12 ears and interruption of the auricular cartilage in 22 ears. The intracartilaginous connective tissue with blood vessels and lymphatics were linked together with the connective tissue outside the auricular cartilage in other sections. These findings support the hypothesis of congenital embryonic dysplasia as the origin of the formation of pseudocyst of the auricle.
