The miraculously preserved 2000-year-old Dead Sea Scrolls, ancient texts of invaluable historical significance, were discovered in the mid-20th century in the caves of the Judean desert. Histoplasmosis is a fungal infection caused by the dimorphic fungus Histoplasma capsulatum. MRI of a rare case of polyneuritis following brucellosis is presented. An 8-year-old boy with a history of brucellosis showed marked enlargement of the origin of the left fifth cranial nerve and contrast enhancement of the fifth cranial nerves and the Gasserian ganglion in Meckel's caves. An unruptured anterior circulation, nongiant saccular cerebral aneurysm (located at the clinoid segment [C3] of the internal carotid artery [ICA] and growing into the carotid cave) was detected at a medical checkup and diagnosed from subsequent examinations. The aneurysm was observed and followed up with radiologic examinations. An examination performed 12 years after diagnosis revealed the disappearance of the aneurysm. This observation should be kept in mind, that it is possible for an unruptured aneurysm to undergo spontaneous thrombosis without any intervention.

A new stenasellid isopod is described from Tashan Cave, Khuzestan Province, south-west Iran, belonging to the genus Stenasellus Dollfus, 1897. The first recorded species of Stenasellidae from Iran, Stenasellus tashanicussp. n., is diagnosed by the presence of antennae with a minute squama bearing paired, long, robust setae; a maxilliped endite with six coupling hooks; and slender appendix masculina with an acute apex. A revised generic diagnosis is provided with a key to the six known western Asian Stenasellus species.

A 39-year-old man living with AIDS presented with a swollen face. He was found to be HIV infected after presenting with Coccidioides pneumonia 2 years previously and was placed on daily fluconazole and then on highly active antiretroviral therapy. Carcinomas of the hard or soft palate are known to spread perineurally along palatine branches of the maxillary nerve. Imaging of perineural tumor spread from the palate has been underemphasized in the imaging literature. We report the findings from eight patients in whom spread from primary cancers of the palate was seen along the palatine nerves. Indications of perineural spread include enlargement or excessive enhancement of a nerve, or abnormal density/signal intensity, enhancement, or widening of the pterygopalatine fossa, cavernous sinus, or Meckel's cave.

A Charcot joint developed following a spinal cord injury in a patient who had sustained a fracture-dislocation of the 12th thoracic vertebra and a spinal cord injury in a cave-in accident in a coal mine 40 years previously, and had since been assisted in walking with the aid of a short leg brace and a cane. Recently, the patient developed Charcot joints of the right knee and ankle, and the right ankle joint also became infected with a refractory open wound necessitating a below-knee amputation.

Vaginal surgery for the relief of stress incontinence has a failure rate which is unacceptable. Since 1964 the authors have combined in the management of female urinary disorders. Where surgery was indicated the combined approach was used as the primary procedure. The bladder neck was elevated by a technique of suspension using 2 mm Teflon tape, via the cave of Retzius. It was not a sling. The results of 100 consecutive operations are presented. The team approach has lead to a better understanding and management of all varieties of female bladder problems.
