Spread of Escherichia coli strains with high-level cefotaxime and ceftazidime resistance between the community, long-term care facilities, and hospital institutions.

A total of 151 Escherichia coli strains resistant to cefotaxime and ceftazidime were isolated during a prospective surveillance study. These strains were characterized by clinical, microbiological, and molecular analyses and were distributed into four clusters of 103, 11, 6, and 5 isolates, along with 25 unrelated strains. The principal cluster was isolated from urine, wound, blood, and other samples in three hospitals, eight nursing homes, and a community healthcare center. This cluster was associated with both nosocomial (65%) and community-acquired (35%) infections. Most strains were resist…

Risk factors for in vivo emergence of ceftazidime-avibactam resistance in KPC-producing Klebsiella pneumoniae and the associated resistance mechanisms: a case-control study.

The emergence of ceftazidime-avibactam-resistant Klebsiella pneumoniae carbapenemase (KPC)-producing Klebsiella pneumoniae (CZA-R KPC-Kp), primarily due to KPC variants, poses a growing challenge to clinical practice. We aimed to investigate risk factors for in vivo development of CZA resistance in KPC-Kp and explore resistance mechanisms. A matched case-control study was conducted at Taipei Veterans General Hospital, including patients with sequential KPC-Kp isolates, between June 2019 and May 2025. Forty-five patients with CZA-R KPC-Kp and a prior CZA-susceptible KPC-Kp (i.e., cases) were ma…

Proposal for screening cut-off values and use of tolerance disk test (TDtest) for detection of tolerance/persistence to ceftazidime-avibactam in Klebsiella pneumoniae.

OBJECTIVE: We aimed to characterize ceftazidime-avibactam (CZA)-tolerant/persistent Klebsiella pneumoniae, previously categorized as CZA-susceptible, and propose screening cut-off values for CZA-tolerance/persistence. Moreover, we discuss the Tolerance Disk Test (TDtest) to detect CZA-tolerance/persistence in clinical microbiology laboratories. METHODS: We studied 80 K. pneumoniae clinical isolates detected as CZA-resistant, truly CZA-susceptible, or CZA-tolerant/persistent by TDtest. CZA-tolerant and CZA-persistent isolates were differentiated by determination of minimal duration for killing…

Ceftazidime/avibactam and ceftolozane/tazobactam for the treatment of extensively drug-resistant Pseudomonas aeruginosa post-neurosurgical infections: three cases and a review of the literature.

PURPOSE: Post-neurosurgical infection caused by extensively drug resistant Pseudomonas aeruginosa (XDR-PA) are becoming a matter of great concern due to limited therapeutic options. Although not approved for these indications, the new BetaLactam-BetaLactamase Inhibitor combinations (BLBLIs) could represent a valid salvage treatment. We describe one nosocomial meningitis and two cervical osteomyelitis due to an XDR-PA who were treated with ceftazidime/avibactam (CZA) and ceftolozane/tazobactam (C/T) and review the literature. METHODS: The first and the third patients developed an osteomyelitis…

Exploring the Role of the Ω-Loop in the Evolution of Ceftazidime Resistance in the PenA β-Lactamase from Burkholderia multivorans, an Important Cystic Fibrosis Pathogen.

The unwelcome evolution of resistance to the advanced generation cephalosporin antibiotic, ceftazidime is hindering the effective therapy of Burkholderia cepacia complex (BCC) infections. Regrettably, BCC organisms are highly resistant to most antibiotics, including polymyxins; ceftazidime and trimethoprim-sulfamethoxazole are the most effective treatment options. Unfortunately, resistance to ceftazidime is increasing and posing a health threat to populations susceptible to BCC infection. We found that up to 36% of 146 tested BCC clinical isolates were nonsusceptible to ceftazidime (MICs ≥ 8 μ…

Distribution of ceftazidime-avibactam and colistin MIC values in carbapenem-resistant Klebsiella pneumoniae and its association with carbapenemase genotypes in Can Tho, Vietnam.

Carbapenem-resistant Klebsiella pneumoniae (CRKP) is a World Health Organization critical-priority pathogen, with a bloodstream-infection mortality of 40-70%. Ceftazidime-avibactam (CAZ-AVI) and colistin are key agents against CRKP, yet data on their minimum inhibitory concentrations (MICs) and association with carbapenemase genotypes in the Mekong Delta region of Vietnam are limited. This cross-sectional study on CRKP isolates from clinical specimens at hospitals in Can Tho City (November 2024-July 2025) determined the MICs of CAZ-AVI and colistin and their association with carbapenemase geno…

In vivo emergence of ceftazidime/avibactam resistance in Pseudomonas aeruginosa driven by OXA-1381, a novel W157L Ω-loop variant of OXA-2.

The emergence of ceftazidime/avibactam resistance in an extended-spectrum β-lactamase (ESBL)-producing Pseudomonas aeruginosa isolate during ceftazidime/avibactam therapy was investigated and the underlying mechanisms elucidated. Two sequential isolates recovered before and after the ceftazidime/avibactam treatment were characterized by antimicrobial susceptibility testing, whole-genome sequencing, and functional and biochemical analyses. The initial isolate was susceptible to ceftazidime/avibactam, whereas the later isolate developed resistance (increase in MIC from 2 to 32 mg/L), accompanied…
