Magnetic Resonance Neurography Detection of the Wedge-Sickle Sign Fibromuscular Bands in Neurogenic Thoracic Outlet Syndrome: A Case Report.

CASE: A 17-year-old girl presented with chronic right arm numbness, cramping, burning pain, and weakness. Initial nerve studies showed slowing in the median and ulnar nerves, but an outside magnetic resonance imaging was negative. Repeat magnetic resonance neurography (MRN) revealed right C8 nerve root and lower trunk enlargement, compressed by a fibrous band from a small cervical rib. The lower trunk appeared indented in a "sickle" shape-termed the wedge-sickle sign. Surgery confirmed the MRN findings. CONCLUSION: MRN can depict the wedge-sickle sign previously seen only on ultrasound, poten…

Dual-Time-Point 18 F-FDG PET/CT for Differentiating a Chest Wall Hemangioma From a Malignant Metastasis.

Dual-time-point 18 F-flluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) can be used to differentiate benign vascular tumors from other malignant growths. We present the case of a 70-year-old woman with a history of cervical carcinoma who was referred for PET/CT to examine a left chest wall tumor noted on CT, which involved the adjacent rib and pleura, thus raising the suspicion of metastasis. The chest wall tumor demonstrated moderate FDG uptake and further decreased uptake at the delayed-phase scanning, corresponding to biodistribution of FDG in the blood po

Arterial Thoracic Outlet Syndrome in a Runner.

Thoracic outlet syndrome (TOS) most commonly manifests in overhead athletes (e.g., baseball pitchers, swimmers, weight lifters) due to nerve compression caused by skeletal abnormalities. We present the case of a 43-year-old recreational runner with unilateral upper extremity pain while running. Vascular imaging identified an aberrant subclavian artery origin with positional compression in the absence of cervical bone anomalies confirming arterial TOS. A first rib resection and anterior scalenectomy led to symptom resolution. This case emphasizes the importance of a broad differential and compl

Posttraumatic Malunion Following Comminuted Scapular Fracture.

A 42-year-old male police officer sustained multiple injuries in a motorcycle accident, including a comminuted fracture of the scapula and several rib fractures. Four months after his injury, the patient was prescribed outpatient physical therapy, after 2 months of which the patient's cervical and lumbar spine function were restored but his shoulder mobility deficits persisted. A second physical therapy opinion was sought 6 months post injury for the left shoulder. Given his functional deficits, including an inability to return to work, he was referred to an orthopaedist for additional imaging

Sildenafil as a therapeutic option for digital ischemic ulceration: case report.

We report the case of a 37-year-old woman who developed critical upper limb ischemia caused by a cervical rib. Because the malformation was initially undiagnosed, a vascular bypass was performed, and failure occurred. Following a 6-month therapy with sildenafil, revascularization of the arm was successful and amputation was avoided. A 6-year follow-up shows a rich collateral network at the compression site and normal values of digital plethysmography. Because hand surgeons often see patients with digital ulcerations and other manifestations of peripheral vascular pathology, therapy of ischemia…

Multicenter analysis of CIREN occupant lumbar bone mineral density and correlation with age and fracture incidence.

OBJECTIVE: This study aimed to quantify lumbar volumetric bone mineral density (vBMD) for 873 seriously injured Crash Injury Research and Engineering Network (CIREN) motor vehicle crash occupants (372 male, 501 female) from 8 centers using phantomless computed tomography scans and to associate vBMD with age, fracture incidence, and osteopenia/osteoporosis diagnoses. The novelty of this work is that it associates vBMD with region of injury by applying an established method for vBMD measurement using phantomless computed tomography (CT). METHODS: A validated phantomless CT calibration method th

Critical Upper Limb Ischemia Due to Brachial Tourniquet in Misdiagnosed Thoracic Outlet Syndrome after Carpal Tunnel Decompression: A Case Report.

We present the case of a 68-year-old woman, referred to our department for critical upper limb ischemia, which had occurred a few days after homolateral surgical ligamentotomy for carpal tunnel syndrome, diagnosed and confirmed by electromyography, and performed with a brachial tourniquet. The patient was later admitted for subsequent progressive necrosis of the first three fingers of the left hand, accompanied by signs of upper limb ischemia. An accessory cervical rib was identified, completely obliterating the subclavian artery distally at the origin of the suprascapular artery. A complete h…

Esophageal stricture caused by rib osteoma in a cat: case report.

CASE SUMMARY: A 6-year-old male domestic shorthair cat presented with frequent food regurgitation and dysphagia. Plain thoracic radiographs revealed a calcified mass overlying the topography of the mediastinum, as well as dilation of the cervical portion of the esophagus due to an accumulation of food. Endoscopic examination showed a severe extraluminal esophageal stricture at the mediastinum entrance. Surgery and a gastric tube were declined by the cat's owner, with palliative support preferred. However, 1 year later, the cat presented with severe cachexia, dysphagia, salivation, dehydration

Contemporary management of thoracic outlet syndrome.

PURPOSE OF REVIEW: The diagnosis and management of thoracic outlet syndrome (TOS) has been surrounded by controversy since this disorder was first recognized. Recent evidence from observational studies has helped us better understand the pathophysiology of different TOS subtypes and guide clinical decision making for this disorder. RECENT FINDINGS: The identification of anatomic anomalies involved with the cause of different TOS subtypes has been made easier by contemporary diagnostic techniques. This includes the injection of neuromuscular blocking agents into anterior scalene muscles to hel

Repair of non-circumferential cervical trachea defects by three different latissimus dorsi flaps. A comparative studyin an experimental setting.

BACKGROUND: Large intrathoracic airway defects may be closed using a pedicled latissimus dorsi (LD) flap, with rewarding results. This study addresses the question of whether this holds true for extrathoracic non-circumferential tracheal defects. METHODS: A cervical segment of the trachea of 4 x 1 cm was resected in 9 white male pigs. The defect was stented with a silicone stent for 3 months and closed either by an LD flap alone (group a, n = 3), an LD flap with an attached rib segment covered by pleura (group b, n = 3), or an LD flap reinforced by a perforated polylactide (MacroPore) plate (

Review of arterial thoracic outlet syndrome with a report of five new instances.

Arterial damage, causing ischemia of the limb, occurs in less than 5 per cent of all instances of thoracic outlet syndrome. Arterial complications are usually associated with cervical ribs or rudimentary first ribs, but 12 per cent have occurred in patients with no osseous abnormality. The physiopathologic factors begin with compression of the subclavian artery which, in most patients, produces stenosis, poststenotic dilatation, formation of aneurysms and mural thrombosis. In other patients, aneurysms do not form, but the compression still causes stenosis, intimal injury and mural thrombosis. …

Pain has limited predictive value for fractures following falls in older people: insight from a retrospective study.

BACKGROUND: In the emergency department (ED), decisions regarding radiological imaging for older patients who have experienced low-energy falls (LEF) are frequently complicated by inconsistencies between the reported mechanism of injury, the patients' symptoms, and the findings on physical examination. Our study aimed to investigate the coherence between physician-documented complaints, the implementation of radiological imaging and the diagnosis of fractures of the vertebral column, rib cage and pelvic ring. METHODS: This is a secondary analysis of a retrospective consecutive sample of 2882

Median labiomandibular glossotomy approach to the craniocervical region.

INTRODUCTION: In children as well as adults, adequate access to the craniocervical junction and upper cervical vertebra can usually be achieved with a transoral-transpalatopharyngeal route. However, when access is necessary to achieve the C5 level and the upper cervical spine in children, this is very difficult. This is particularly so when the incisor opening is less than 2.5 cm. The median labiomandibular glossotomy provides such an approach. MATERIALS AND METHODS: Our experience with five children is presented in a representative case: a 4-year-old male with a family history of spondyloepi
