Efficacy of octreotide against chylothorax following lateral neck dissection for thyroid cancer: A case report.

INTRODUCTION: Postsurgical chylothorax is a rare complication of cervical dissection for thyroid cancer. We report that octreotide, a synthetic analog of somatostatin, is effective in treating chylothorax after thyroid carcinoma surgery. PRESENTATION OF CASE: The patient was a 48-year-old woman who presented to our institution complaining of a left anterior cervical mass. We diagnosed this as thyroid papillary carcinoma and performed a subtotal excision of the thyroid gland with left cervical lymph node dissection. The patient developed dyspnea, and a chest X-ray revealed bilateral chylothora…

A rare case of bilateral chylothorax: a diagnostic challenge--follicular lymphoma versus primary effusion lymphoma.

Chylothorax is most common on the left side owing to the position of the thoracic duct. Malignancy-associated chylothorax is not uncommon. However, bilateral chylothorax is rare and malignancy should be a consideration in absence of trauma. We report a case of a patient with follicular lymphoma who presented with bilateral pleural effusion; pleural fluid analysis confirmed chylothorax. The patient did not have any significant peripheral or axial lymphadenopathy, which made the diagnosis difficult in absence of histopathology. Pleural fluid analysis was negative for malignant cells, however, th…

Chylothorax and Recurrent Laryngeal Nerve Injury Associated With Robotic Video-Assisted Mediastinal Lymph Node Dissection.

OBJECTIVE: Although the technical aspects of robotic video-assisted thoracic surgery (RVATS) for lung resections may be advantageous, compared with standard thoracoscopy, complications of chylothorax and recurrent laryngeal nerve injury (RLNI) associated with mediastinal lymph node dissection (MLND) may be significant. METHODS: Consecutive patients who underwent RVATS anatomic lung resection for suspected or confirmed cancer and experienced RLNI or chylothorax were identified and reviewed from a prospectively maintained database. Complications were graded according to the Common Terminology C…

[Case of bilateral chylothorax with systemic lupus erythematosus complicated by steroid-/immunosuppressant-resistant pleural effusion].

A 20-year-old woman, with systemic lupus erythmatosus complicated by steroid-and immunosuppressant-resistant bilateral pleural effusion, was admitted to the emergency room because of dyspnea and fever. Chest Xray film revealed bilateral massive pleural effusion. Bilateral thoracocentesis yielded fluid with chyle. Conservative treatment including intravenous hyper-alimentation and continuous drainage were performed but with no remarkable improvement. She underwent thoracoscopy-aided ligation of the thoracic duct. After the operation, bilateral pleurodesis was performed by intrathoracic injectio

[Venous angiomatous hamartoma of the mesenteric root. Therapy of chylous ascites abd chylothorax--case report].

Hamartomas are rare benign tumors appearing very often without any symptoms. In case of diagnostic detection the operative exploration is indicated for a correct histological diagnosis and resection. In this report we describe a very rare case of mesenterial hamartoma. During the late postoperative course a refilling chylothorax and chylous ascites occurred. With total parenteral nutrition, excluding short- and long-chain fatty acids, chylous leakage was successfully treated. The chylous exudation was stopped totally; only intraabdominally did minimal ascites persist. The therapy was continued…

Hydrops foetalis and chylothorax associated with superior caval vein obstruction and resolution following balloon dilatation.

A female infant (33 weeks' gestation) was delivered by emergency caesarean section because of hydrops foetalis. No cause was identified initially; haemoglobin, liver and renal functions were normal and there was no evidence of sepsis or significant myocardial dysfunction or arrhythmia. There was initial improvement with conventional management but bilateral pleural effusions persisted, which became overtly chylous after institution of enteral formula feeds on day 4. At 5 weeks of age, an obstruction at the superior caval vein/right atrial junction was confirmed at angiography and dilated with

Chylopericardium in a Dog with Concurrent Chylothorax, Mesothelioma, and Bilateral Brachiocephalic Vein Thrombosis: A Case Report.

Chylopericardium is an extremely rare disorder in dogs, and its underlying mechanisms and optimal management strategies remain poorly defined. This report describes a dog with chylopericardium complicated by concurrent chylothorax, bilateral brachiocephalic vein thrombosis, and mesothelioma. A 6-year-old neutered male Shiba Inu presented with respiratory distress and was diagnosed with chylothorax and chylopericardium based on fluid analysis. Computed tomography revealed multiple thoracic ducts and bilateral brachiocephalic vein thrombosis but no definitive site of lymphatic leakage. Surgical …

Comparison of Heparin-Based Anticoagulation in Patients With and Without High Output Chylothorax: A Pilot Study.

In infants and children with chylothorax after cardiac surgery, loss of antithrombin (AT) may impair the therapeutic effect of AT-dependent anticoagulants such as unfractionated heparin (UFH) and low molecular weight heparin (LMWH). This retrospective pilot study included patients 0-18 years receiving a therapeutic course of UFH or enoxaparin, excluding those on mechanical circulatory support or renal replacement. High output chylothorax courses were matched 1:2 to controls by age and anticoagulation duration. Therapeutic attainment and adverse events were compared for each agent. This study i…

Development of home-based methods to defat human milk for infants with chylothorax: An experimental study.

BACKGROUND: Chylothorax is a postoperative complication for infants with congenital heart defects; with high nutrition risk. Defatted human milk is recommended; however, refrigerated centrifugation to process milk poses accessibility barriers for many hospitals and families at home. Creation of a simplified home-based defatted milk protocol allows infants with chylothorax to be provided the immunological benefits of human milk postoperatively. METHODS: Milk from 20 mothers was tested to compare refrigerated centrifugation as the standard defatting technique against gravity-based methods: syri…

Chylothorax due to thrombosis of the jugular and subclavian veins in a patient with gastric cancer: a case report.

BACKGROUND: Chylothorax is a rare condition due to leakage of chyle in the thoracic cavity. When large amounts of chyle leak into the thoracic cavity, it can lead to severe respiratory, immune, and metabolic complications. Chylothorax has many potential underlying etiologies, and the most common causes are traumatic chylothorax and lymphoma. Venous thrombosis of the upper extremities is a rare cause of a chylothorax. CASE PRESENTATION: A 62-year-old Dutch man with a medical history of gastric cancer, treated with neoadjuvant chemotherapy and surgery 13 months prior, presented with dyspnea and…
