Thyrotoxicosis Secondary to Gestational Trophoblastic Disease: A Rare Complication With Clinical Relevance.

Thyrotoxicosis is a clinical syndrome defined by excessive exposure to circulating thyroid hormones, primarily triiodothyronine (T3) and thyroxine (T4). Its most common cause is hyperthyroidism, which is characterized by the increased production and secretion of hormones by the thyroid gland. However, it can also originate from extrathyroidal stimulation. An example is the disproportionately high production of human chorionic gonadotropin (HCG) in gestational trophoblastic disease (GTD), where excessive levels of this hormone induce thyrotoxicosis through cross-activation of thyroid-stimulatin

Accessory Maxillary Ostia: Anatomical and Clinical Relevance From a Cadaveric Case Report.

Accessory maxillary ostia (AMOs) are relatively common anatomical variants of the paranasal sinuses. They present as small, oval or circular openings found at or near the maxillary sinus, in the posterior nasal fontanelle, anterior nasal fontanelle, or, rarely, in the hiatus semilunaris. We present a unique cadaveric report of bilateral AMOs, exploring the causes, associated findings, and anatomical variations within the maxillary sinus.  During the routine dissection of the nasal cavity, three AMOs were discovered in an 84-year-old female donor, who passed away from a myocardial infarction. Characterisation and Clinical Relevance of Tertiary Lymphoid Structures in Primary Biliary Cholangitis.

BACKGROUND AND AIMS: The pathological characteristics of lymphocyte infiltration in the hepatic portal tracts of patients with primary biliary cholangitis (PBC) remain unclear. Tertiary lymphoid structures (TLSs) are ectopic lymphoid tissues associated with the exacerbation of autoimmune reactions. Here, we evaluate the role of TLSs in PBC and investigate their potential therapeutic value. METHODS: We recruited 75 patients with PBC and 53 control patients with liver biopsies who were followed more than 2 years. TLSs and their maturity were identified by the amount and spatial distribution of

Experimental Models in Penile Transplantation: Translational Insights and Relevance to Clinical Application.

Animal research is an essential contributor to the medical achievements of the last century. The first step of studying a disease in animals is the development of a model which is relevant to the clinical situation in humans. Thus, a good animal model is the sine qua non of the experimental research. This review aims to assess the contemporary literature on animal models for penile transplantation, examining their applicability and significance in the context of clinical scenarios. We also revisit, evaluate, and emphasize the interesting and important findings of certain animal models to bring

Clinical relevance of salivary pepsin detection in diagnosing gastroesophageal reflux disease subtypes.

BACKGROUND: Gastroesophageal reflux disease (GERD) is heterogeneous with a varied symptom spectrum and reflux profiles. Its definite diagnosis often requires invasive tools including endoscopy or reflux monitoring. The aim of this study was to investigate the clinical relevance of salivary pepsin detection as a non-invasive screening tool to diagnose GERD of different subtypes. METHODS: A total of 77 patients with suspected GERD symptoms and 12 asymptomatic controls were analysed. All participants performed symptom evaluation, upper endoscopy, esophageal manometry, and 24-hour multichannel in…

Clinical Relevance of Computationally Derived Attributes of Peritubular Capillaries from Kidney Biopsies.

KEY POINTS: Computational image analysis allows for the extraction of new information from whole-slide images with potential clinical relevance. Peritubular capillary (PTC) density is decreased in areas of interstitial fibrosis and tubular atrophy when measured in interstitial fractional space. PTC shape (aspect ratio) is associated with clinical outcome in glomerular diseases. BACKGROUND: The association between peritubular capillary (PTC) density and disease progression has been studied in a variety of kidney diseases using immunohistochemistry. However, other PTC attributes, such as PTC sh…

Clinical relevance of endoscopic peri-appendiceal red patch in ulcerative colitis patients.

BACKGROUND: Increasing evidence is suggesting appendectomy as an alternative treatment for ulcerative colitis (UC), especially in case of histological appendiceal inflammation. Therefore, preoperative identification of appendiceal inflammation could be beneficial. This study aimed to assess the prevalence of peri-appendiceal red patch (PARP) on colonoscopy. In addition, prognostic relevance of PARP for disease course and its predictive value for histological appendiceal inflammation in patients undergoing appendectomy was assessed. METHODS: UC patients undergoing colonoscopy in 2014/2015 were

Preneoplastic lesions in the liver: Molecular insights and relevance for clinical practice.

Hepatocellular carcinoma (HCC) and cholangiocarcinoma (CCA) are the most frequent primary liver cancers, accounting for approximately 80% and 15%, respectively. HCC carcinogenesis occurs mostly in cirrhosis and is a complex multi-step process, from precancerous lesions (low-grade and high-grade dysplastic nodules) to progressed HCC. During the different stages of liver carcinogenesis, there is an accumulation of pathological, genetic and epigenetic changes leading to initiation, malignant transformation and finally tumour progression. In contrast, a small subset of HCC occurs in normal liver f

Blood-brain barrier leakage in Alzheimer's disease: From discovery to clinical relevance.

Alzheimer's disease (AD) is the most common form of dementia. AD brain pathology starts decades before the onset of clinical symptoms. One early pathological hallmark is blood-brain barrier dysfunction characterized by barrier leakage and associated with cognitive decline. In this review, we summarize the existing literature on the extent and clinical relevance of barrier leakage in AD. First, we focus on AD animal models and their susceptibility to barrier leakage based on age and genetic background. Second, we re-examine barrier dysfunction in clinical and postmortem studies, summarize chang…

Clinical relevance of multiple confirmed preserved ratio impaired spirometry cases in adults.

BACKGROUND: Preserved ratio impaired spirometry (PRISm) is a common spirometry finding, but its heterogeneous manifestations and frequent transitions to airflow limitation (AFL), chronic obstructive pulmonary disease, or normal spirometry hinder establishing an appropriate management strategy. This study examined whether transition to AFL and baseline comorbidities are more frequent in subjects with definite PRISm (PRISm confirmed on both current and past two spirometry tests) versus incident PRISm (PRISm confirmed only on a current test with past normal spirometry records) than in normal spir

Diagnostic yield and clinical relevance of expanded genetic testing for cancer patients.

BACKGROUND: Genetic testing (GT) for hereditary cancer predisposition is traditionally performed on selected genes based on established guidelines for each cancer type. Recently, expanded GT (eGT) using large hereditary cancer gene panels uncovered hereditary predisposition in a greater proportion of patients than previously anticipated. We sought to define the diagnostic yield of eGT and its clinical relevance in a broad cancer patient population over a 5-year period. METHODS: A total of 17,523 cancer patients with a broad range of solid tumors, who received eGT at Memorial Sloan Kettering C…

Clinical and biological relevance of CREB3L1 in Philadelphia chromosome-negative myeloproliferative neoplasms.

Cyclic AMP-response element-binding protein 3-like 1 (CREB3L1) is a gene involved in the unfolded protein response (UPR). Recently, we demonstrated that CREB3L1 is specifically overexpressed in the platelets of patients with Philadelphia chromosome-negative myeloproliferative neoplasms (MPNs). In this study, we aimed to show the clinical and biological relevance of CREB3L1 in these hematological diseases. Overexpression of CREB3L1 was specific to platelets in MPNs and associated with a higher risk of thrombosis and fibrotic transformation in essential thrombocythemia (ET) and polycythemia vera

Vitamins and Uterine Fibroids: Current Data on Pathophysiology and Possible Clinical Relevance.

Uterine fibroid (UF) is the most common benign tumor pathology of the female reproductive organs. UFs constitute the main reason for a hysterectomy and hospitalization due to gynecological conditions. UFs consist of uterine smooth muscle immersed in a large amount of extracellular matrix (ECM). Genetic studies have demonstrated that UFs are monoclonal tumors originating from the myometrial stem cells that have underwent specific molecular changes to tumor initiating stem cells which proliferate and differentiate later under the influence of steroid hormones. There is growing interest in the ro

Parkinson disease and the gut: new insights into pathogenesis and clinical relevance.

The classic view portrays Parkinson disease (PD) as a motor disorder resulting from loss of substantia nigra pars compacta dopaminergic neurons. Multiple studies, however, describe prodromal, non-motor dysfunctions that affect the quality of life of patients who subsequently develop PD. These prodromal dysfunctions comprise a wide array of gastrointestinal motility disorders including dysphagia, delayed gastric emptying and chronic constipation. The histological hallmark of PD - misfolded α-synuclein aggregates that form Lewy bodies and neurites - is detected in the enteric nervous system prio

Clinical relevance of cancer stem cell chemotherapeutic assay for recurrent ovarian cancer.

INTRODUCTION: Disease recurrence and progression of ovarian cancer is common with the development of platinum-resistant or refractory disease. This is due in large part to the presence of chemo-resistant cancer stem cells (CSCs) that contribute to tumor propagation, maintenance, and treatment resistance. We developed a CSCs drug cytotoxicity assay (ChemoID) to identify the most effective chemotherapy treatment from a panel of FDA approved chemotherapies. METHODS: Ascites and pleural fluid samples were collected under physician order from 45 consecutive patients affected by 3rd-5th relapsed ov
