CT findings in isolated ischemic proctosigmoiditis.

The purpose of our study was to describe the CT features of ischemic proctosigmoiditis in correlation with clinical, laboratory, endoscopic, and histopathologic findings. Our study included seven patients with isolated ischemic proctosigmoiditis. Patients were identified by a retrospective review of all histopathologic records of colonoscopic biopsies performed during a time period of 4 years. All patients presented with left lower abdominal quadrant pain, bloody stools, and leukocytosis, and four patients had fever at the time of presentation. Four of seven patients suffered from diarrhea, on

Intestinal ischemia after aortic surgery.

Intestinal ischemia after abdominal aortic surgery is a highly lethal complication. In order to evaluate the pathogenesis, the diagnostic modalities and the best management, in a retrospective review, 12 patients undergoing postoperative small bowel or colonic ischemic lesions were identified between 1983 and 1995. Preoperative occlusion of IMA was present in nine patients, while a selective angiography of SMA demonstrated occlusive disease of peripheral branches in two asymptomatic diabetic patients. No patent IMA was ligated. When possible, hypogastric circulation was preserved by distal ana

Esophageal replacement by colon interposition.

Between 1985 and 1993, 32 patients (24 male and 8 female) underwent colon interposition for replacement of the esophagus at the Mayo Clinic. Median age was 58.5 years (range, 1 to 79 years). The colon was used because of an inadequate stomach in 27 patients (84%) and as the conduit of choice in 5 (16%). Esophageal cancer was present in 15 patients (47%). The left colon was used in 20 patients (63%) and the right, in 12 (38%). The colon was placed substernally in 19 patients (59%) and in the esophageal bed in 13 (41%). The operative mortality was 9%; cause of death was ischemic necrosis of righ

Acquired protein C deficiency in a child with acute myelogenous leukemia, splenic, renal, and intestinal infarction.

We report the case of a 6-year-old boy diagnosed with acute promyelocytic leukemia (AML-M3V) when he presented with pallor, abdominal pain, anorexia, and fatigue. Induction chemotherapy was started according to the AML-BFM 98 protocol along with Vesanoid (ATRA, All-trans retinoic acid). On the sixth day of induction, he developed splenic and gallbladder infarcts. Splenectomy and cholecystectomy were performed while chemotherapy induction continued as scheduled. Four days later, he developed ischemic areas in the kidneys and ischemic colitis in the sigmoid colon. Hypercoagulation studies showed

Late-Onset Bone Metastasis 46 Years After Initial Surgery for Pheochromocytoma: A Case Report.

INTRODUCTION: Pheochromocytoma is a rare catecholamine-producing tumor with metastatic potential. Recurrence after more than 40 years is exceptionally rare. CASE PRESENTATION: During evaluation for ischemic colitis, a 71-year-old woman was found to have multiple bone metastases, possibly linked to catecholamine excess. She had undergone left adrenalectomy for pheochromocytoma at age 25. Bone biopsy confirmed metastatic pheochromocytoma, and immunohistochemical findings were similar to the original tumor. Urinary metanephrine and normetanephrine were markedly elevated. She declined systemic th

Iatrogenic Water Intoxication After Intravenous Infusion of Sterile Water: A Rare and Preventable Case.

Elective intravenous infusion therapies are increasingly offered in outpatient settings, often without adequate physician oversight or protocols for safety. We describe a case of accidental iatrogenic injury following an intravenous infusion at a medical spa. An elderly woman with a solitary kidney presented to the ED with chest tightness, acute shortness of breath, and headache shortly after receiving an intravenous vitamin infusion in a liter of sterile water, followed by half a liter of normal saline. She rapidly deteriorated, developing flash pulmonary edema and pulseless electrical activi

Idiopathic mesenteric phlebosclerosis associated with use of Chinese patent medicine: a case report and literature review.

Idiopathic Mesenteric Phlebosclerosis Colitis (IMP) is indeed a rare and potentially life-threatening condition characterized by chronic ischemic changes in the colon, primarily due to calcification of the mesenteric veins. This disease is often associated with long-term use of certain herbal medicines, particularly in Asian populations, where its prevalence is notably higher. This report describes a 55-year-old male patient who has been taking Chinese patent medicine orally for an extended period to treat prostatitis. His primary symptom is diarrhea. Imaging studies revealed multiple linear c

Acute Colonic Diverticulitis: CT Findings, Classifications, and a Proposal of a Structured Reporting Template.

Acute colonic diverticulitis (ACD) is the most common complication of diverticular disease and represents an abdominal emergency. It includes a variety of conditions, extending from localized diverticular inflammation to fecal peritonitis, hence the importance of an accurate diagnosis. Contrast-enhanced computed tomography (CE-CT) plays a pivotal role in the diagnosis due to its high sensitivity, specificity, accuracy, and interobserver agreement. In fact, CE-CT allows alternative diagnoses to be excluded, the inflamed diverticulum to be localized, and complications to be identified. Imaging f

A rare cause of acute abdomen: Isolated necrosis of the cecum.

OBJECTIVE: We aimed to present a 76-year-old female patient with cecal necrosis, which is a rare cause of acute abdomen in elderly women and a variant of ischemic colitis. MATERIALS AND METHODS: The patient was admitted to hospital with abdominal pain, anorexia and nausea. Physical examination, laboratory parameters and abdominal computed tomography revealed acute abdomen. We operated the patient with below umbilical midline incision. Although the pain localized to the right lower quadrant made us consider acute appendicitis as first diagnosis, other pathologies were also kept in mind for dif

Shiga toxin-associated hemolytic uremic syndrome complicated by intestinal perforation in a child with typical hemolytic uremic syndrome.

Hemolytic uremic syndrome (HUS) is one of the most common causes of acute renal failure in childhood and is primarily diagnosed in up to 4.5% of children who undergo chronic renal replacement therapy. Escherichia coli serotype O157:H7 is the predominant bacterial strain identified in patients with HUS; more than 100 types of Shiga toxin-producing enterohemorrhagic E. coli (EHEC) subtypes have also been isolated. The typical HUS manifestations are microangiopathic hemolytic anemia, thrombocytopenia, and renal insufficiency. In typical HUS cases, more serious EHEC manifestations include severe h

Eculizumab safely reverses neurologic impairment and eliminates need for dialysis in severe atypical hemolytic uremic syndrome.

This case report describes how eculizumab reversed neurologic impairment and improved renal damage in severe atypical hemolytic uremic syndrome. A 50-year-old female, after presenting with diarrhea and abdominal pain, developed pancolitis, acute renal failure, and thrombocytopenia. The patient underwent total abdominal colectomy. Pathology confirmed ischemic colitis with scattered mesenteric microthrombi. Due to mental and respiratory decline, she remained intubated. Continuous venovenous hemodialysis was initiated. Renal failure, neurologic changes, hemolysis, thrombotic microangiopathy, and

Endovascular grafting for abdominal aortic aneurysms.

Despite the initial success of endovascular grafts in a very difficult patient population, many problems remain. These procedures are often time-consuming and quite complicated, requiring the close cooperation of an experienced team of vascular surgeons and interventional radiologists. Access may be difficult through occluded, stenotic, and tortuous vessels. Inadequate graft deployment may result in arterial rupture or graft migration, which could potentially lead to acute occlusion of the renal or iliac arteries. Occlusion of the inferior mesenteric artery may result in ischemic colitis. Also

The congenital chloride diarrhea gene is expressed in seminal vesicle, sweat gland, inflammatory colon epithelium, and in some dysplastic colon cells.

Congenital chloride diarrhea (CLD) is an autosomal recessive disorder of intestinal electrolyte transportation caused by mutations in the anion transporter protein encoded by the down-regulated in adenoma (DRA), or CLD, gene. In this study, in situ hybridization and immunohistochemistry were performed to investigate the expression of CLD in extraintestinal normal epithelia and in intestinal inflammatory and neoplastic epithelia. The expression of the closely related anion transporter diastrophic dysplasia sulfate transporter, DTDST, was also examined and compared with that of CLD in colon. The

Mass-forming isolated ischemic necrosis of the cecum mimicking malignancy: Clinicopathologic features of 11 cases.

Isolated ischemic necrosis of the cecum (INC) is an uncommon benign condition that occurs most often in elderly patients. The clinical presentation and radiographic findings often mimic acute appendicitis or malignancy. This study aimed to investigate the clinicopathologic features of mass-forming INC. Eleven INC patients who presented with cecal masses were identified by searching the electronic medical record system in two large academic centers. The clinical and pathological features, including symptoms, imaging, endoscopic and surgical findings, histomorphology, and follow-up, were retrosp

Spontaneous colonic perforation in a pediatric patient with acute febrile illness: a case report.

Spontaneous colonic perforation is a rare but life-threatening surgical emergency, particularly uncommon in children without predisposing conditions. We report a 4-year-old male who presented with acute febrile illness, suprapubic abdominal pain, and dysuria, initially managed as cystitis. Despite treatment, he developed peritonitis; imaging revealed free subdiaphragmatic air. Exploratory laparotomy showed an ischemic ascending colon with a solitary perforation, necessitating right hemicolectomy with ileocolic anastomosis. Histopathology demonstrated nonspecific ileitis, typhlitis, and colitis

Polyarteritis nodosa: a case report of isolated large bowel involvement and surgical intervention.

Polyarteritis nodosa (PAN) is a necrotizing vasculitis commonly affecting small-sized vessels and medium-sized vessels. We present the case of surgical resection for ischemic colitis secondary to PAN. A 26-year-old woman presented with left lower quadrant pain, diarrhea and bloody stools. Colonoscopy showed diffuse circumferential ulceration and necrotic lining from the sigmoid colon to the splenic flexure. A subsequent computed tomography angiography (CTA) revealed left colonic wall thickening, hypoenhancement and multiple inferior mesenteric artery aneurysms, concerning for vasculitis. Ensui

Left Ventricular Thrombus and Cardioembolic Stroke in a Patient with Ulcerative Colitis: A Case Report.

Left ventricular (LV) thrombi usually occur in the setting of global or regional LV systolic dysfunction and are extremely rare in the absence of LV wall motion abnormalities. We report here a case of a 23-year-old female who presented with cardioembolic stroke due to ulcerative colitis. To determine the cause of stroke, several investigations and evaluations were carried out, but the results were mostly normal or unremarkable. Transthoracic echocardiography revealed an oscillating pedunculated globular mass, which was eventually resected due to recurrent transient ischemic attacks. The histop

The endovascular treatment of a ruptured aneurysm of the middle colic artery combined with an isolated dissection of superior mesenteric artery: report of a case.

BACKGROUND: Ruptured aneurysm of the middle colic artery is very rare. Its concomitance with isolated dissection of the superior mesenteric artery (SMAD) has not yet been reported in the literature. CASE REPORT: A previously well 56-year-old man was referred to our hospital with acute-onset pain throughout the entire abdomen. A ruptured aneurysm of the middle colic artery and isolated SMAD were revealed using a computed tomography (CT) scan. Coil embolization was successfully performed for a ruptured aneurysm of the middle colic artery. During the immediate postprocedural period, the patient
