Effect of local anesthetics on postoperative pain and opioid consumption in laparoscopic colorectal surgery.

BACKGROUND: Several studies have evaluated use of local anesthetic, specifically, administration of intraperitoneal anesthetic, during laparoscopic general surgery and gynecologic operations, with varying results. There have been no studies to determine the role of local anesthetic in laparoscopic colorectal surgery. This study evaluates the efficacy of subcutaneous and intraperitoneal anesthetic in reducing postoperative pain following common laparoscopic colorectal procedures, in patients managed with enhanced recovery care pathways. METHODS: A single-institution retrospective cohort analys…

Patterns of surveillance practices after curative surgery for colorectal cancer in a French population.

PURPOSE: Despite controversies, postoperative surveillance of colorectal cancer is generally recommended. This registry-based study was designed to assess the adherence of surveillance practices to French guidelines and identify three-year surveillance patterns and their demographic and clinical determinants. METHODS: All patients (N = 409) diagnosed with first colorectal cancer in 1998 and alive without recurrence at least six months after curative surgery were identified from a population-based registry. Medical charts from multiple sources were reviewed to collect exhaustive information on

Povidone-iodine enema as a preoperative bowel preparation for colorectal surgery. A bacteriologic study.

To evaluate the effects of povidone-iodine (PI) enema on the bacterial flora of colorectal mucosa, the authors studied 113 patients who were candidates for colorectal surgery. The study of the rectum included 72 patients. Total bacterial concentrations after a PI enema (N = 44) were significantly lower than after a simple water enema (N = 12, P less than 0.001), or than after a water enema associated with intravenous metronidazole (N = 16, P less than 0.01). The study of the colon included 41 patients. Total bacterial concentrations did not differ after a PI enema (N = 24) than after a water e…

Patient characteristics and surgery-related factors associated with patient-reported recovery at 1 and 6 months after colorectal cancer surgery.

Predictors for postoperative recovery after colorectal cancer surgery are usually investigated in relation to length of stay (LoS), readmission, or 30-day morbidity. This study describes patient characteristics and surgery-related factors associated with patient-reported recovery 1 and 6 months after surgery. In total, 153 consecutively included patients who were recovering from colorectal cancer surgery reported their level of recovery using the Postoperative Recovery Profile. Multiple logistic regression analysis was used to calculate associations with recovery, defined as good or poor, divi

Thoracic Epidural Anesthesia and Prophylactic Three Times Daily Unfractionated Heparin Within an Enhanced Recovery After Surgery Pathway for Colorectal Surgery.

BACKGROUND AND OBJECTIVES: Venous thromboembolism (VTE) is a common cause of preventable harm. Perioperative thoracic epidural analgesia (TEA) presents a challenge to optimal VTE prophylaxis. Our primary aim was to characterize missed doses of VTE prophylaxis associated with epidural catheter placement and removal. Our secondary aim was to measure the effect of an enhanced recovery after surgery (ERAS) pathway on the rate of TEA-associated missed VTE prophylaxis. METHODS: We retrospectively reviewed a prospectively collected database of 1264 colorectal surgery patients at a single academic ce…

Rates and burden of surgical site infections associated with pediatric colorectal surgery: insight from the National Surgery Quality Improvement Program.

PURPOSE: The purpose of this study was to characterize the rates of surgical site infections (SSI) associated with colorectal procedures in children and the relative burden of these events within the scope of pediatric surgical practice. METHODS: The NSQIP-Pediatric Public Use File was queried for all pediatric surgery procedures captured from 50 hospitals during 2012-2013. Rates of incisional and deep organ/space SSIs (ISSI and OSI, respectively) were calculated for all procedures, and the relative burden of SSIs from the entire dataset attributable to colorectal procedures was determined.

"Having surgery is necessary" - a qualitative analysis of the experiences of frail older adults treated with, and recovering from colorectal cancer surgery.

BACKGROUND: Frail older adults undergoing surgical treatment for colorectal cancer are at increased risk of postoperative mortality and complications compared with non-frail older adults. Even though these risks are established, limited research has explored patient perspectives on surgical treatment and recovery in frail older adults. This study aimed to explore the experiences of frail older adults during diagnosis, treatment, and recovery from surgically treated colorectal cancer. METHODS: Participants were recruited through purposive sampling from the control group of the randomized contro

Age disparity in robotic colorectal surgery: overcoming surgeon hesitancy to improve elderly patient outcomes.

BACKGROUND: Robotic colorectal surgery offers significant benefits for vulnerable elderly patients, but its adoption in this demographic appears limited. We hypothesized that a significant age-related disparity exists in robotic surgery utilization. This study aimed to investigate the relationship between age and the use of robotic surgery, along with its associated postoperative outcomes in elderly patients. METHODS: This retrospective cohort study utilized the American College of Surgeons National Surgical Quality Improvement Program database from 2012 to 2021. We analyzed 80,187 patients ag…

Reducing Postoperative Blood Tests in Colorectal Surgery: A Dual-Cohort Before-and-After Study.

INTRODUCTION: Routine postoperative blood testing is a deeply ingrained aspect of colorectal surgical ward practice, yet a significant proportion of tests ordered are not clinically indicated. Overtesting contributes to iatrogenic anemia, increased costs, and patient discomfort. Compliance with Enhanced Recovery After Surgery (ERAS) Society and National Health Service (NHS) England guidance on blood test rationalization is inconsistent, particularly in acute surgical settings, which have received little specific interventional attention. MATERIALS AND METHODS: A dual-cohort, before-and-after
