Modeling, simulation, and optimal initiation planning for needle insertion into the liver.

Needle insertion simulation and planning systems (SPSs) will play an important role in diminishing inappropriate insertions into soft tissues and resultant complications. Difficulties in SPS development are due in large part to the computational requirements of the extensive calculations in finite element (FE) models of tissue. For clinical feasibility, the computational speed of SPSs must be improved. At the same time, a realistic model of tissue properties that reflects large and velocity-dependent deformations must be employed. The purpose of this study is to address the aforementioned diff

Peripheral Neuropathy: A Phenotype-Driven Review for Diagnosis and Management.

The purpose of this narrative review is to provide a clinic-ready synthesis of contemporary concepts in peripheral neuropathy, spanning epidemiology, diagnosis, and treatment, with emphasis on high-yield advances applicable to daily practice. The authors integrate pragmatic tools-including a diagnostic algorithm, suggested initial laboratory panels, and commonly used outcome measures-to support clinical decision-making. However, this review is intended as a clinic-oriented synthesis rather than a formal practice guideline. Peripheral neuropathy can be systematically categorized into seven path

Management of effort thrombosis of the subclavian vein: today's treatment.

"Spontaneous" subclavian vein ("effort") thrombosis is usually related to extrinsic compression of the vein at the costoclavicular junction. Our experience with this entity over the past decade was reviewed with specific focus on the role of thrombolysis and our selection algorithm. All patients treated for this problem at the University of Rochester over the past decade were identified and records retrospectively reviewed. From 1996 to June 2006, a total of 34 patients with spontaneous complete occlusion of the subclavian vein documented by venography were treated, half with a history of an a

Redefining Pediatric SCIWORA: A Systematic Review of the Literature on Clinical Patterns, Imaging Profiles, and Management Insights.

Objectives: Among the spectrum of spinal injuries, Spinal Cord Injury Without Radiographic Abnormality (SCIWORA) occupies a unique and challenging position. SCIWORA presents diagnostic and therapeutic challenges due to its broad clinical and radiological heterogeneity. While most children recover favorably with conservative treatment, a subset may require surgery based on imaging findings. The findings underscore the need for standardized diagnostic criteria, MRI-based classification systems, and evidence-based treatment algorithms to improve consistency in care and long-term neurological outc

Carpal Tunnel Syndrome Attributed to Medication Use: A Pharmacovigilance Study.

OBJECTIVE: Carpal tunnel syndrome (CTS) is a prevalent compression neuropathy with multiple well-documented mechanical and systemic risk factors. However, the role of pharmacological agents in the development of CTS remains underexplored. This study aims to identify drugs disproportionately associated with CTS reports using data from the Food and Drug Administration Adverse Event Reporting System (FAERS). METHODS: A retrospective pharmacovigilance analysis was conducted using OpenVigil 2.1 to evaluate adverse event (AE) reports of CTS from the FAERS database from October 2003 to September 202

Risk prediction of osteoporotic vertebral compression fractures in postmenopausal osteoporotic women by machine learning modelling.

BACKGROUND: Osteoporosis in postmenopausal women is characterized by significant bone mass loss due to reduced oestrogen, leading to an increased risk of osteoporotic vertebral compression fractures (OVCF). Comprehensive risk prediction models for diagnosing and predicting fracture risk in this population are still lacking. OBJECTIVE: This study aims to identify key risk factors for OVCF in postmenopausal osteoporotic women and develop a machine learning model to predict OVCF risk by integrating clinical, biological, and musculoskeletal data. METHODS: This retrospective case-control study in

Development and optimization of AI algorithms for wrist fracture detection in children using a freely available dataset.

INTRODUCTION: In the field of pediatric trauma computer-aided detection (CADe) and computer-aided diagnosis (CADx) systems have emerged offering a promising avenue for improved patient care. Especially children with wrist fractures may benefit from machine learning (ML) solutions, since some of these lesions may be overlooked on conventional X-ray due to minimal compression without dislocation or mistaken for cartilaginous growth plates. In this article, we describe the development and optimization of AI algorithms for wrist fracture detection in children. METHODS: A team of IT-specialists, p

Optimization of volumetric breast density estimation in digital mammograms.

Fibroglandular tissue volume and percent density can be estimated in unprocessed mammograms using a physics-based method, which relies on an internal reference value representing the projection of fat only. However, pixels representing fat only may not be present in dense breasts, causing an underestimation of density measurements. In this work, we investigate alternative approaches for obtaining a tissue reference value to improve density estimations, particularly in dense breasts. Two of three investigated reference values (F1, F2) are percentiles of the pixel value distribution in the breas

End-to-side anterior interosseous nerve transfer augmentation of ulnar nerve function in moderate and severe cubital tunnel syndrome; a randomised controlled trial protocol.

BACKGROUND: The anterior interosseous nerve (AIN) end-to-side (ETS) nerve transfer is a relatively recent treatment innovation in managing cubital tunnel syndrome (CuTS). Potential benefits must be balanced against the risks associated with the adoption of an innovative technique, evaluating procedure-specific risks, and considering additional costs in the safe delivery of care. The ETS technique has been proposed as a method of improving intrinsic muscle function in the hand in the setting of ulnar nerve compression at the elbow. The technique is controversial with emerging evidence from retr

Results of a Flexible Patient-Centered Approach to the Timing of Thoracic Outlet Decompression in Paget Schroetter Syndrome.

BACKGROUND: Treatment algorithms for subclavian vein (SCV) effort thrombosis (Paget-Schroetter syndrome- PSS) are multiple, ranging from thrombolysis (TL) with immediate or delayed thoracic outlet decompression (TOD) to conservative treatment with anticoagulation alone. We follow a regimen of TL/pharmacomechanical thrombectomy (PMT) followed by TOD with first rib resection, scalenectomy, venolysis, and selective venoplasty (open or endovascular) performed electively at a time convenient for the patient. Oral anticoagulants are prescribed for 3 months or longer based upon response. The aim of t

Neuromonitoring of cochlea and auditory nerve with multiple extracted parameters during induced hypoxia and nerve manipulation.

A system capable of comprehensive and detailed monitoring of the cochlea and the auditory nerve during intraoperative surgery was developed. The cochlear blood flow (CBF) and the electrocochleogram (ECochGm) were recorded at the round window (RW) niche using a specially designed otic probe. The ECochGm was further processed to obtain cochlear microphonics (CM) and compound action potentials (CAP). The amplitude and phase of the CM were used to quantify the activity of outer hair cells (OHC); CAP amplitude and latency were used to describe the auditory nerve and the synaptic activity of the inn

A simplified algorithm for diagnosis of spinal cord lesions.

Neuroimaging is indispensable for evaluation of Myelopathy not only for localization but also for etiologic determination. MRI is the preferred examination for further characterization of the majority of these conditions. These include traumatic, inflammatory, infections, compressive and neoplastic conditions. This article provides an overview of a variety of pathologies that afflict the spinal cord in an easy to understand format. Their respective imaging manifestations on MRI and differential diagnoses are focused in this review. Early diagnosis and treatment of Myelopathy is critical in pre

An inverse dynamics modeling approach to determine the restraining function of human knee ligament bundles.

During knee motion, the fiber bundles of ligaments are nonuniformly loaded in a recruitment pattern which is different for successive knee-joint positions. As a result, the restraining functions of these ligaments are variable. To analyze the relative restraint contributions of the fiber bundles in different knee-joint positions, a new method was developed. Its application was illustrated for the cruciate ligaments of one knee-joint specimen. The methods developed to estimate bundle forces comprise five steps. First, the three-dimensional motions of a knee specimen are measured for anterior-po

Quantitative strain elastography of the uterine cervix assessed by the GE Voluson E10 system in combination with a force-measuring device.

OBJECTIVE: During pregnancy, the stiffness of the cervical tissue decreases long before the cervical length decreases. Therefore, several approaches have been proposed in order to ensure a more objective assessment of cervical stiffness than that achieved by digital evaluation. Strain elastography has shown promising results. This technique is based on an ultrasound assessment of the tissue deformation that occurs when the examiner applies pressure on the tissue with the ultrasound probe. However, the results are only semi-quantitative as they depend on the unmeasured force used by the examine

Quantitative pupillometry and radiographic markers of intracranial midline shift: A pilot study.

BACKGROUND: Asymmetric pupil reactivity or size can be early clinical indicators of midbrain compression due to supratentorial ischemic stroke or primary intraparenchymal hemorrhage (IPH). Radiographic midline shift is associated with worse functional outcomes and life-saving interventions. Better understanding of quantitative pupil characteristics would be a non-invasive, safe, and cost-effective way to improve identification of life-threatening mass effect and resource utilization of emergent radiographic imaging. We aimed to better characterize the association between midline shift at vario

False-positive fluorodeoxyglucose positron emission tomography results after chemotherapy in patients with metastatic seminoma.

INTRODUCTION: The treatment of residual masses after chemotherapy in seminomas remains a controversial topic. Postchemotherapy retroperitoneal lymph node dissection (PC-RPLND) in all patients would lead to severe overtreatment with a high rate of complications and additional procedures. For this reason, fluorodeoxyglucose positron emission tomography (FDG-PET) was introduced. FDG-PET has an accuracy of 88%. In 15% of cases, FDG-PET findings are false positive (FP) with unclear consequences. Therefore, we retrospectively investigated the rate of unnecessary procedures due to FP results on FDG-P

Stent biomechanics in hemodialysis-related left brachiocephalic vein stenosis: from site-specific loading to a staged device-validation framework.

Hemodialysis-related left brachiocephalic vein stenosis is usually managed as one local variant of central venous stenosis, yet its mechanical environment is distinctive. The vein follows a long, multi-axially curved retrosternal course through a narrow window between the manubrium and the aortic arch, bounded by two separate junctions, the lateral venous angle and the medial brachiocephalic-caval confluence. An implanted stent is therefore exposed to focal unidirectional compression, cyclic bending and torsion, short landing zones, and branch-coverage risk, conditions not reproduced by circum
