Long-term effects of radically open dialectical behavior therapy for anorexia nervosa: a six-month follow-up study.

Six-month follow-up results from a trial investigating the effects of Radically Open Dialectical Behavior Therapy (RO-DBT) in individuals with anorexia nervosa are presented. Participants (N = 23; median age 20 years; 96% female) completed a 32-week RO-DBT intervention. Repeated Measures ANOVAs were used to examine changes over time. The primary outcome was eating disorder psychopathology. Exploratory outcomes were BMI, obsessive-compulsive personality traits, emotion regulation, work function and treatment utilization. Statistically significant reductions in eating disorder psychopathology, a

Depressive Rumination and Co-Morbidity: Evidence for Brooding as a Transdiagnostic Process.

A sample of 116 patients with unipolar mood disorders referred to a specialist research clinic were assessed to investigate (a) whether rumination is a transdiagnostic process that is related to co-morbid Axis I and II symptoms and diagnosis in depressed patients; (b) whether common findings in the depressive rumination literature could be replicated in a recurrent depressed sample. Consistent with the transdiagnostic hypothesis, rumination was positively associated with both depression and anxiety, brooding was related to co-morbid obsessive-compulsive disorder and generalized anxiety disorde

[ADH/D and impulsiveness: Prevalence of impulse control disorders and other comorbidities, in 81 adults with attention deficit/hyperactivity disorder (ADH/D)].

UNLABELLED: Attention deficit hyperactivity disorder (ADH/D) is a neuropsychological developmental disorder characterized by pervasive and impairing symptoms of inattention, hyperactivity, and impulsivity. Whereas it is well known in children, there is still little information about ADH/D in adults, including prevalence. Indeed, there are actually no epidemiological studies in France, despite the considerable impact of this disorder in a patient's professional and affective life. Moreover, ADH/D rarely stays isolated, and many comorbidities often complicate the diagnostic investigation. It is

"Not Just Right Experiences" are specific to obsessive-compulsive disorder: further evidence from Italian clinical samples.

Not Just Right Experiences (NJREs) are considered to be a perceptually tinged phenomenon mainly related to obsessive-compulsive disorder (OCD). The evidence of an association between NJREs and OCD or OC symptoms have been accumulating in the last few years, whereas there is a paucity of studies about the role of this construct in other clinical conditions considered part of the "OCD spectrum". In the current study, the NJRE-Q-R Severity scale (a well-validated measure of NJREs) was administered to 41 patients with OCD, 53 with hair-pulling disorder (HPD), 38 with gambling disorder (GD) and 43

The Role of Personality, Self-Disclosure, and Envy in Maladaptive Social Media Engagement.

Through online social networks (OSNs), individuals establish and maintain social connections to satisfy their need to belong. Recent research suggests that taken too far, one's need to belong can increase envy and lead to maladaptive social media behavior aligned with obsessive-compulsive disorder (OCD). This study examines the role of two personality traits, one's intrinsic need to belong and trait reactance, on feelings of envy and the self-disclosure processes that lead to OCD on social networks. A sample of 354 U.S. adult users of Facebook completed a survey measuring individuals' need to

The Role of Trauma in Early Onset Borderline Personality Disorder: A Biopsychosocial Perspective.

The role of childhood trauma in the development of borderline personality disorder (BPD) in young age has long been studied. The most accurate theoretical models are multifactorial, taking into account a range of factors, including early trauma, to explain evolutionary pathways of BPD. We reviewed studies published on PubMed in the last 20 years to evaluate whether different types of childhood trauma, like sexual and physical abuse and neglect, increase the risk and shape the clinical picture of BPD. BPD as a sequela of childhood traumas often occurs with multiple comorbidities (e.g. mood, anx

Sex differences in Indian patients with obsessive-compulsive disorder.

Sex has been postulated as one of the factors mediating heterogeneity in obsessive-compulsive disorder (OCD). This study investigated the sex differences in OCD with respect to sociodemographics, symptom profile, and comorbidity including spectrum disorders. Two hundred thirty-one subjects diagnosed with OCD by Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, criteria were included in the study. The subjects were evaluated by extensive clinical and semistructured interviews by expert clinical psychiatrists, and diagnosis was made by consensus. Male (n = 166) and female (n

Unremitting body odour: A case of Olfactory Reference Syndrome.

Olfactory reference syndrome (ORS) is a person's fear of exuding an offensive body odour which is not perceived by others. The objective of this case report is to highlight the challenges in diagnosing olfactory reference syndrome due to the lack of diagnostic criteria as well as its similarities to other psychiatric illnesses. We report a case of a young Chinese gentleman who was preoccupied with the belief that he had an offensive body odour which was not noticeable by others since the age of 10. As a result of this, he developed compulsive behaviour, social anxiety and avoidance, as well as

[The Impact of NICE Guidance on UK Psychiatry].

The UK National Institute for Health and Care Excellence (NICE) is a non-departmental public body accountable to the Department of Health. NICE was established in 1999 in an attempt to reduce variability in the availability and quality of medical services in NHS to end the so-called postcode lottery. Since its establishment NICE has acquired an international reputation for the development of high quality, evidence-based clinical guidelines. NICE also makes cost-benefit assessments of certain technologies on the basis of effectiveness and cost effectiveness. In order to develop a NICE guideline

Corpus callosum abnormalities in pediatric bipolar disorder.

The corpus callosum (CC) is a midline white matter brain region that is important in interhemispheric communication and coordination. CC abnormalities are associated with a variety of psychiatric conditions, including increased vulnerability for psychotic illness, stressful early-life experiences, marijuana use, attention-deficit/hyperactivity disorder, obsessive-compulsive disorder, borderline personality disorder, dementia, schizophrenia and bipolar disorder. CC abnormalities in bipolar disorder have been identified in both pediatric and adult populations. In adults, a consistent finding has

The relationship between obsessions and the self: Feared and actual self-descriptions in a clinical obsessive-compulsive disorder sample.

Cognitive models of obsessive-compulsive disorder (OCD) posit the relevance of the self in OCD, although the nature of this association is still unclear. We aimed to explore actual and feared selves and its association with obsessions and intrusions in a group of OCD patients. A group of 58 patients with OCD identified their most upsetting obsession and intrusion (non-clinical obsession) experienced in the past 3 months. These cognitions were classified as either moral-based or autogenous (obsessions n = 32; intrusions n = 26) or non-moral-based or reactive, depending on their content. Next, p

Homogamous and heterogamous marriages.

This paper explores two marital constructs set forth by Peter Giovacchini. The first is based on a symbiotic reenactment in which the investment of both partners is tenacious and reciprocal, an attachment which Giovacchini refers to as a "character object" relationship. It represents intrapsychic homogramy. In the second construct, this deep form of attraction is absent and the dyadic bond is superficial, frequently transitory. The latter couples do not require an elementary, intrapsychic bond but do share particular defensive traits or symptoms. Thus these unions are termed "symptom object" r

Effectiveness of Individual Cognitive-Behavioral Therapy and Predictors of Outcome in Adult Patients with Obsessive-Compulsive Disorder.

INTRODUCTION: Cognitive-behavioral therapy (CBT) for obsessive-compulsive disorder (OCD) has proven its efficacy in randomized controlled trials (RCTs). OBJECTIVE: To test generalizability to routine care settings, we conducted an effectiveness study to provide naturalistic outcome data and their predictors. METHODS: Pre-post changes in symptoms and impairment as well as response rates were determined in a naturalistic OCD sample (intention-to-treat, ITT, n = 393). Patients received individual CBT for OCD adopting an exposure-based, non-manualized treatment format. Linear and logistic regres

Sense of happiness and other aspects of quality of life in patients with obsessive-compulsive disorder.

INTRODUCTION: Quality of life (QoL) is the intellectual and emotional wellbeing of an individual, which is determined by many factors. The most measurable are the sense of happiness, occupational satisfaction, quality of interpersonal relationships and sex life. Obsessive-compulsive disorder (OCD) is a chronic mental disorder diagnosed by the presence of obsessions and compulsions that disrupt normal psychosocial functioning. Despite early onset, treatment is delayed (OCD-DTI) and unsatisfactory. OBJECTIVES: The primary objective of this study is to assess selected correlates of the sense of

The impact of symptomatic hoarding in OCD and its treatment.

BACKGROUND: The value of defining subtypes in obsessive compulsive disorder (OCD) has become an important issue for recent debate. The low hoarding group (n = 20) was selected on the basis of a low score on the hoarding subscale and a clinician judgement that the hoarding item of the OCPD SCID-II module was "absent". RESULTS: On some measures of pre-trea …

Clinical Advances in Treatment Strategies for Obsessive-compulsive Disorder in Adults.

In the present article, we provide a comprehensive review of the treatment strategies for obsessive-compulsive disorder (OCD), a common, chronic, and often debilitating disorder, characterized by overwhelming obsessions and compulsions. OCD typically starts in childhood or adolescence and persists throughout life, causing functional impairment across multiple domains. The article begins by describing the historical concepts of OCD from religious and guilt-based explanations to psychoanalytic perspectives, and then explores the changing understanding of OCD as a treatable condition.
