From Acute Confusion to Chronic Decline: The Cognitive Impact of Delirium in Older Adults.

Delirium is prevalent in healthcare settings, leaving susceptible older adults at risk for persistent cognitive impairment, prolonged care needs, morbidity, and mortality. In older adult patients, delirium often arises from acute medical illnesses, infections, medications, and comorbidities, with age and underlying dementia increasing susceptibility. Its complex pathophysiology involves systemic inflammation, neurotransmitter dysregulation, disrupted brain metabolism, and physiological cycle disturbances. While delirium causes acute cognitive impairments, patients are often left with persisten

Individualized home-based phoneme training for adult cochlear implant users reduces phoneme confusion errors.

OBJECTIVE: This study presents a novel approach to reducing phoneme confusion errors among adult cochlear implant (CI) users through personalized phoneme training delivered via a mobile app. METHODS: Twenty-one experienced adult CI users participated in a four-week at-home phoneme training program using the mobile app. Participants trained 20 minutes daily in 5-minute intervals, five days a week. Training materials were customized to address each participant's unique phoneme confusion errors, as identified by a phoneme recognition test in quiet. RESULTS: On a group level, participants showed…

A Mysterious Case of Recurrent Confusion: An Unusual Presentation of Congenital Intrahepatic Portosystemic Shunt.

Congenital portosystemic shunts (CPSS) are rare vascular malformations that divert portal blood into the systemic circulation, bypassing hepatic metabolism. Although typically diagnosed in infancy due to associated congenital anomalies or complications such as hepatic encephalopathy, adult presentations are exceptionally uncommon and diagnostically challenging. We report a 71-year-old female patient with recurrent episodes of confusion and drowsiness who was found to have markedly elevated serum ammonia levels despite normal liver function. Extensive investigations excluded infective, metaboli…

Terminological Confusion About Sedation in Palliative Care: Results of an International Online Vignette Survey.

Background: Terminological problems concerning sedation in palliative care and consequences for research and clinical decision making have been reported frequently. Objectives: To gather data on the application of definitions of sedation practices in palliative care to clinical cases and to analyze implications for high-quality definitions. Design: We conducted an online survey with a convenience sample of international experts involved in the development of guidelines on sedation in palliative care and members of the European Association for Palliative Care (EAPC). Participants were asked to

Unraveling the NRF2 confusion: Distinguishing nuclear respiratory factor 2 from nuclear erythroid factor 2.

In recent years, the acronym NRF2 has garnered significant attention in scientific discourse. However, this attention has occasionally led to confusion due to the existence of two distinct proteins sharing the same acronym: Nuclear Respiratory Factor 2 (NRF2), also known as GA-binding protein transcription factor subunit alpha (GABPA), and Nuclear Factor Erythroid 2-related Factor 2 (NFE2L2 or NRF2). This confusion has been highlighted in various scientific forums, including PubPeer and anonymous reader comments, where the confusion between the two proteins has been expressed. In this article,…

Ectopic right thoracic kidney associated with Bochdalek hernia as the cause of diagnostic confusion.

A 61-year-old man who was a psychiatric patient and an alcoholic was admitted to hospital after a fall in his bathroom. He showed signs of hemorrhagic shock, and CT scans showed the presence of the right kidney and part of the right retroperitoneum in the right hemithorax, surrounded by liquid. Surgery showed that the right hemidiaphragm was intact and that no intraabdominal viscera prolapsed into thoracic cavity. The bleeding in the right retroperitoneum was evacuated. The surgical report also stated that "the right kidney and ureter were explored and no injuries or active bleeding were found

Validation of the confusion assessment method in detecting postoperative delirium in cardiac surgery patients.

BACKGROUND: Early detection, prevention, and treatment of delirium after cardiac surgery are important for quick postoperative recovery. The Confusion Assessment Method (CAM) may be an easy-to-use instrument for detecting delirium in clinical practice. OBJECTIVES: To compare the congruent validity of the CAM with the results from repeated assessments by using a combination of the Organic Brain Syndrome Scale and the Mini-Mental State Examination according to the Diagnostic and Statistical Manual of Mental Disorders (Fourth Edition, Text Revision) criteria for delirium. METHODS: Patients aged…

[PSAP expression in a primary presacral neuroendocrine tumor. Potential for confusion with prostate cancer].

Primary presacral neuroendocrine tumors are a rare entity with less than 30 cases described in the literature so far. Here we report of a primary presacral neuroendocrine tumor diagnosed at autopsy which was wrongly diagnosed as metastasized prostate cancer before. Misdiagnosis was due to the localization of the tumor, its morphology and its positivity for prostate-specific acid phosphatase (PSAP) when the patient was alive. This is the first report of PSAP and somatostatin receptor expression in this type of tumor.

Such cases may cause confusion with sickle cell-haemoglobin C (SC) disease.
