Relapsing polychondritis. A clinical, pathologic-anatomic and histochemical study of 2 cases.

A 57-year-old man and a 70-year-old woman with relapsing polychondritis are reported. The man, suffering from arthralgias, respiratory obstruction, external ear and sanddle-nose deformities, conjunctivitis and irido-cyclitis, died after 4 years from airway obstruction because of tracheal and bronchial collapse. The woman is alive 8 months after the development of respiratory obstruction, probably caused by radiographically demonstrated tracheal obstruction, a saddle-nose deformity and hearing impairment. Microscopically, the involved cartilages showed degenerative and slight inflammatory chang

Systemic lupus erythematosus and infection: a controlled and prospective study including an epidemiological group.

An epidemiological group of 60 SLE patients was studied; 162 infections were observed prospectively for an aggregate of 1366 months. Bacterial infection was more frequent in the SLE patients than in matched controls but only slightly more frequent than in matched rheumatoid arthritis patients. The rheumatoid arthritis patients, also matched with regard to corticosteroid dosage, showed a tendency towards an increased infection rate only. Lower urinary tract infections were comparable in all three matched groups. In a total of 102 SLE patients studied, including the epidemiological group, clinic

Host Disease Tolerance Predicts Transmission Probability for a Songbird Pathogen.

Disease tolerance reduces the per-pathogen fitness costs of infection for hosts and is an important component of host adaptation to pathogens. However, how disease tolerance affects host transmission potential is not well understood, especially because there are many potential mechanisms that facilitate host tolerance. For example, tissue-specific host tolerance leads to the reduction of host pathology, regardless of pathogen load. Hosts may also exhibit behavioral tolerance, where normal behaviors are maintained even while harboring high pathogen loads. Here, we examined the impacts that tiss

Ocular infections in international travelers.

INTRODUCTION: Ophthalmological conditions in international travelers may be associated with low mortality but high morbidity. Eye involvement in travelers is less frequently reported than febrile, gastrointestinal and respiratory infections, but data probably represent a degree of under-notification. METHODS: an extensive narrative review of the main viral, bacterial, fungal and parasitic infections affecting the eye in travelers was performed. MAIN FINDINGS: Common respiratory tract viral infections may cause ocular complications in travelers, human influenza viruses have been associated wi

Broad-Spectrum Antimicrobial Activity of Oftasecur and Visuprime Ophthalmic Solutions.

Due to the wide etiology of conjunctivitis, the expensive and time-consuming diagnosis requires new therapeutic strategies with broad-spectrum antimicrobial activity and nonselective mechanisms of action. In this context, eye drops could provide an alternative to conventional antimicrobial therapies. Here, we compare the antibacterial and antiviral activity of Oftasecur and Visuprime, commercially available ophthalmic solutions. Cytotoxicity assay was performed on Vero CCL-81 cells by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl-tetrazolium bromide (MTT) test. Antibacterial efficacy was evaluated

Contact lens complications among wearers in Ghana.

PURPOSE: Contact lens (CL) practice is relatively new in Ghana; a country where the geographical location (warm climate) lends itself to harsh environmental conditions (high humidity) known to influence CL wear. Recent studies suggest an increase in CL wear (corrective and cosmetic), yet, there are no studies about CL-related complications. This study sought to determine the complications associated with CL wear in Ghana. METHODS: This was a retrospective cohort study. The medical records of contact lens wearers from five CL clinics in the two largest metropolises in Ghana, Accra and Kumasi,

Development of an Aqueous Ophthalmic Solution with an Enhanced-Solubility Enrofloxacin Crystal, and its Clinical Evaluation in Dogs.

BACKGROUND: The concern about the frequent use of ciprofloxacin in veterinary medicine is linked to increased antimicrobial resistance. The corresponding fluoroquinolone for veterinary use is enrofloxacin. A new solvate form of enrofloxacin, as dihydrate-hydrochloride (enro-C) with higher water solubility than the parent compound, was formulated as an ophthalmic solution (pH 5). A multicentre, longitudinal, non-inferiority clinical study in a non-hospital environment was designed to treat 36 dogs affected by tobramycin-unresponsive conjunctivitis with either the experimental 0.5% enro-C ophtha

Overview of anaerobic infections in children and their treatment.

Anaerobic bacteria can cause many infections in children. Because they predominant in the normal human skin and mucous membranes bacterial flora, they are often associated with bacterial infections that originate from these sites. They are difficult to isolate from infectious sites, and are frequently missed. Anaerobic infections can occur in all body sites, including the central nervous system, oral cavity, head and neck, chest, abdomen, pelvis, skin, and soft tissues. Anaerobes colonize the newborn after birth and have been isolated in several types of neonatal infections. These include cell

Evaluation of content and cost of traditional eye medication in a resource-poor country - Implications for eye care practice and policy.

BACKGROUND: Cataract, glaucoma, and conjunctivitis are common causes of ocular morbidity in Nigeria. A major obstacle in reducing the burden of ocular morbidity in rural areas is access to eye care services. Up to 80% of the population in developing countries use traditional medications for their primary healthcare needs because they are accessible, available, and affordable. The aim of this study is to evaluate the content and cost of commercialized traditional medications used in the treatment of common eye conditions in Nigeria. PATIENTS AND METHODS: All the registered traditional healers

Silent Threats: Understanding the Impact of Respiratory Viruses on the Ageing Population.

Respiratory viruses are an important cause of acute respiratory illnesses in older adults. The spectrum of illness may range from pneumonia to an exacerbation of underlying respiratory disease or acute bronchitis. Respiratory viruses can account for a significant proportion of chest infections. However, respiratory viruses, either acting as primary pathogens or in conjunction with bacterial infections, are often underdiagnosed due to less frequent viral testing compared to bacterial infections. Hitherto neglected, the coronavirus disease 2019 (COVID-19) pandemic has brought into sharp focus an

Population structure of ocular Streptococcus pneumoniae is highly diverse and formed by lineages that escape current vaccines.

Streptococcus pneumoniae is a leading cause of ocular infections including serious and sight-threatening conditions. The use of pneumococcal conjugate vaccines (PCV) has substantially reduced the incidence of pneumonia and invasive pneumococcal diseases, but has had limited impact on ocular infections. Additionally, widespread vaccine use has resulted in ongoing selective pressure and serotype replacement in carriage and disease. To gain insight into the population structure of pneumococcal isolates causing ocular infections in a post-PCV-13 time period, we investigated the genomic epidemiolog

Gonococcal Conjunctivitis: A Case Report of an Unusual Mode of Transmission.

BACKGROUND: Outside of the newborn period, development of gonococcal conjunctivitis is rare and predominantly occurs through autoinoculation. We report an unusual case of gonococcal conjunctivitis in a young woman exposed through direct inoculation. CASE: A 19-year-old woman presented with purulent ocular discharge, severe pain, and decreased vision unresponsive to topical antibiotics or ganciclovir approximately 3 weeks after accidental ocular exposure to seminal fluid during unprotected oral sex. Cultured ocular drainage grew Neisseria gonorrhea; vaginal and throat cultures were negative. S

Treating 'Septic' With Enhanced Antibiotics and 'Arthritis' by Mitigation of Excessive Inflammation.

Bacterial infection within the synovial joint, commonly known as septic arthritis, remains a clinical challenge as it presents two concurrent therapeutic goals of reducing bacterial burden and preservation of articular cartilage from destructive host inflammation. We hypothesized that mitigation of MRSA-induced inflammatory signaling could diminish destruction of articular cartilage in the setting of septic arthritis when used in conjunction with antibiotics. Herein, we provide evidence which supports a new therapeutic notion that concurrent antimicrobial therapy to address the 'septic' compon
