Analyzing bioterror response logistics: the case of smallpox.

To evaluate existing and alternative proposals for emergency response to a deliberate smallpox attack, we embed the key operational features … ength. The former estimate delineates the efficacy (i.e., accuracy) and efficiency (i.e., speed) of contact tracing, while the latter estimate reveals how congestion makes the race to trace more difficult to win, thereby causing more deaths. A probabilistic analysis is also used to find an approximate closed-form expression for the total number of deaths under ma …

Surveillance, prevention, and control of sexually transmitted agents associated with pelvic inflammatory disease.

Control and prevention of pelvic inflammatory disease would result from successful control of Neisseria gonorrhoeae and genital Chlamydia trachomatis infections. The same control system applies to both, but while this system is well documented in gonorrhea control, there is still no evidence that it is effective for C. trachomatis infections. Primary prevention of PID should be realized through a reduction of the incidence of gonorrhea, and the available techniques--(1) contact tracing and epidemiologic treatment, (2) screening and case finding, (3) preventive measures, (4) health education, a…

Interventions to reengage people living with HIV who are lost to follow-up from HIV treatment programs: A systematic review and meta-analysis.

BACKGROUND: Optimizing services to facilitate engagement and retention in care of people living with HIV (PLWH) on antiretroviral therapies (ARTs) is critical to decrease HIV-related morbidity and mortality and HIV transmission. We systematically reviewed the literature for the effectiveness of implementation strategies to reestablish and subsequently retain clinical contact, improve viral load suppression, and reduce mortality among patients who had been lost to follow-up (LTFU) from HIV services. METHODS AND FINDINGS: We searched 7 databases (PubMed, Cochrane, ERIC, PsycINFO, EMBASE, Web of

A multi-provincial Salmonella Typhimurium outbreak in Canada associated with exposure to pet hedgehogs, 2017-2020.

BACKGROUND: In October 2020, an investigation began in Canada on an outbreak of Salmonella Typhimurium infections of the same strain as a concomitant outbreak in the United States (US) that was linked to pet hedgehogs. The objective of this article is to identify the source of the outbreak, determine if there was a link between the Canadian and US outbreaks and identify risk factors for infection to inform public health interventions. METHODS: Cases were identified through whole genome sequencing of S. Typhimurium isolates. Information was collected on case exposures, including animal contact

Distribution across tissue layers of extrinsic nerves innervating the mouse colorectum - an in vitro anterograde tracing study.

BACKGROUND: Anterograde in vitro tracing of the pelvic nerve (PN) and visualization in the horizontal plane in whole mount preparations has been fundamental in the analysis of distribution of peripheral nerves innervating the colorectum. Here, we performed a similar analysis, but in cryostat sections of the mouse colorectum, allowing for a more direct visualization of nerve distribution in all tissue layers. METHODS: Colorectum with attached PNs was dissected from adult male BalbC mice. Presence of active afferents was certified by single fiber recording of fine PN fibers. This was followed b

Virus excretion in smallpox. 2. Excretion in the throats of household contacts.

Throat swabs of 34 of 328 family contacts of 52 smallpox cases, examined 4-8 days after the onset of the disease in the family, were positive for variola virus. The log titre of virus per swab ranged from 2 to 3.95. A higher proportion of unvaccinated than of vaccinated contacts excreted the virus. Only 4 of the virus-positive contacts developed clinical smallpox; this occurred 5-7 days after their swabs were examined. Excretion of virus in the throats of these contacts, a few of whom were in the incubation period of the disease, suggests the possibility that they could have spread the infecti

A Screening Model for Latent Tuberculosis Infection Among Household Contacts of Patients With Tuberculosis Based on the CyTB Test and an Android App: Protocol for a Development and Implementation Study.

BACKGROUND: Latent tuberculosis infection (LTBI) remains a major barrier to tuberculosis elimination, particularly in community settings where household contacts (HHCs) of patients with active tuberculosis are at increased risk of infection and progression to disease. Current tests for LTBI have practical limitations that restrict their use in field and primary care settings. Antigen-based skin testing, combined with digital tools for induration measurement, may improve the accuracy and delivery of HHC screening under routine program conditions. OBJECTIVE: This study aims to develop and evalu

Impact of temporal patterns in working contacts on epidemic spread.

Most models for infectious disease spread simplify contact heterogeneity by assuming constant rates within a week. However, empirical studies show clear variation, such as reduced workplace contacts on weekends. In this work, we investigate the effects of daily variation in workplace contacts on the spread of respiratory infections using the individual-based framework GEMS (German Epidemic Micro-Simulation System) with a synthetic population of 5 million individuals. We compare the scenario with uniform daily contacts to the scenario with more contacts on workdays and fewer on weekends, keepin

Outbreak of Cyclosporiasis Among Patrons of a Mexican-Style Restaurant - Limestone County, Alabama, May-June 2023.

In early June 2023, the Alabama Department of Public Health identified five laboratory-confirmed cyclosporiasis case reports with a common patient exposure of eating at one Mexican-style restaurant. Common signs of cyclosporiasis include diarrhea, abdominal cramps, nausea, and loss of appetite. Although most illnesses are self-limited, antibiotic treatment can prevent relapsing illness. Onset of illness for the initial five patients occurred during May 26-30. An outbreak investigation was initiated on June 7. Routine case investigations and case finding through the restaurant's food delivery s

Designing community-based strategies to reach non-household contacts of people with tuberculosis in Lusaka, Zambia: a rapid qualitative study among key stakeholders.

BACKGROUND: In high-burden settings, most tuberculosis (TB) transmission likely occurs outside the home. Our qualitative study in Zambia explored the acceptability and preferences for designing TB active case finding (ACF) strategies to reach non-household contacts of people with TB. METHODS: We conducted 56 in-depth interviews with persons with TB (n = 12), TB healthcare workers (HCWs) (n = 10), TB lay HCWs (n = 10), and leaders/owners (n = 12) and attendees (n = 12) of community venue types identified as potential TB transmission locations. Interviews explored TB attitudes and beliefs, and

Differential rates of Mycobacterium tuberculosis transmission associate with host-pathogen sympatry.

Several human-adapted Mycobacterium tuberculosis complex (Mtbc) lineages exhibit a restricted geographical distribution globally. These lineages are hypothesized to transmit more effectively among sympatric hosts, that is, those that share the same geographical area, though this is yet to be confirmed while controlling for exposure, social networks and disease risk after exposure. Using pathogen genomic and contact tracing data from 2,279 tuberculosis cases linked to 12,749 contacts from three low-incidence cities, we show that geographically restricted Mtbc lineages were less transmissible th…

Preserving friendships in school contacts: An algorithm to construct synthetic temporal networks for epidemic modelling.

High-resolution temporal data on contacts between hosts provide crucial information on the mixing patterns underlying infectious disease transmission. Publicly available data sets of contact data are however typically recorded over short time windows with respect to the duration of an epidemic. To inform models of disease transmission, data are thus often repeated several times, yielding synthetic data covering long enough timescales. Looping over short term data to approximate contact patterns on longer timescales can lead to unrealistic transmission chains because of the deterministic repeti

Evidence for hidden leprosy in a high leprosy-endemic setting, Eastern Ethiopia: The application of active case-finding and contact screening.

Leprosy or Hansen's disease is a disabling infectious disease caused by Mycobacterium leprae. Reliance on the self-presentation of patients to the health services results in many numbers of leprosy cases remaining hidden in the community, which in turn results in a longer delay of presentation and therefore leading to more patients with disabilities. Although studies in Ethiopia show pockets of endemic leprosy, the extent of hidden leprosy in such pockets remains unexplored. This study determined the magnitude of hidden leprosy among the general population in Fedis District, eastern Ethiopia.
