Objectives: Immunoglobulin (Ig) G4-related disease (IgG4-RD) is often complicated by allergic disorders. This study was conducted to investigate the mechanism of type 2 helper T-inflammation (Th2-inflammation) in IgG4-related dacryoadenitis and sialadenitis (IgG4-DS). This is the first report showing the clinical course of a patient with bilateral IgG4-related dacryoadenitis, which spontaneously regressed after excision of the left lacrimal gland. A 65-year-old female had been aware of bilateral eyelid swelling without pain for 5 years. Magnetic resonance imaging revealed enlargement of bilateral lacrimal gland. BACKGROUND: Connatal lacrimal duct stenosis (cLDO) commonly causes purulent bacterial dacyrocystitis. The recommended treatment of choice is up to the sixth month of life a conservative antimicrobiological therapy. After the sixth month lacrimal duct irrigation with silicone tube intubation remains the gold standard. Actinomycotic infection should always be considered in any patient with persistent purulent conjunctivitis of chronic canaliculitis. The clinical features, investigation including macrodacryocystography, and treatment in four patients with actinomycotic canaliculitis are presented. The difficulty of microbiological diagnosis is stressed and the occurrence of associated lacrimal obstruction is reported.
