Phenotypic characterization of KCTD3-related developmental epileptic encephalopathy.

The association between KCTD3 gene and neurogenetic disorders has only been published recently. In this report, we describe the clinical phenotype associated with 2 pathogenic variants in KCTD3 gene. Seven individuals (including one set of monozygotic twin) from 4 consanguineous families presented with developmental epileptic encephalopathy, global developmental delay, central hypotonia, progressive peripheral hypertonia, and variable dysmorphic facial features. Posterior fossa abnormalities (ranging from Dandy-Walker malformation to isolated hypoplasia of the cerebellar vermis) were consisten

Posterior fossa and vermian morphometry in the characterization of fetal cerebellar abnormalities: a prospective three-dimensional ultrasound study.

OBJECTIVES: To assess whether, by applying a predefined set of morphometric measurements to the posterior fossa and the cerebellar vermis, it is possible to detect and quantify the following parameters, which represent key features of abnormalities of the vermis and posterior fossa: 1) upward rotation of the vermis; 2) upward displacement of the tentorium; 3) underdevelopment of the vermis. Also to assess, in a small subset of cases, the correlation between ultrasound and magnetic resonance imaging (MRI) in the measurement of these three parameters. METHODS: This was a prospective observation

Transient diabetes insipidus following Escherichia coli meningitis complicated by ventriculoperitoneal shunt.

Although disorders of ADH secretion associated with meningitis are usually consistent with the Syndrome of Inappropriate Antidiuretic Hormone Secretion (SIADH), central diabetes insipidus (DI) is an exceptional complication of meningitis. Transient DI as a complication of Escherichia coli (E. coli) meningitis due to ventriculoperitoneal shunt in an 18-month-old boy is presented. Blood and spinal fluid cultures yielded E. coli, sensitive to cefotaxime. The DI arose on the day 3 after admission and continued to the day 20. Treatment comprised cefotaxime, dexamethasone, fluid adjustment and vasop

[Echographic measurements in the posterior cranial fossa: the normal values of the term and premature newborn infant].

INTRODUCTION: We support the normal standards for two sonographic (US) signs of the posterior cranial fossa, namely the magnitude of the cisterna magna and the height of the fourth ventricle fastigium, in preterm and fullterm infants. MATERIALS AND METHODS: The cisterna magna was measured as the distance between the inferior aspect of the cerebella vermis and the inner aspect of the occiput. The height of the fourth ventricle fastigium was calculated as the line drawn from the fastigium to the inner aspect of the occiput, close to the opisthion. RESULTS: The cisterna magna was .46 +/- .13 cm

Report of a Homozygous Nonsense Variant in the WDR91 Gene Associated With Severe Communicating Hydrocephalus, Dandy Walker Malformation, and Cerebellar Hypoplasia.

The WDR81 and WDR91 genes are important for endosome conversion, fusion, recycling, and transport. Variants in the WDR81 gene have been reported in human patients with cerebellar ataxia, impaired intellectual development, and disequilibrium syndrome type 2 (CAMRQ2, MIM #610185) and in patients with congenital hydrocephalus with brain anomalies type 3 (HYC3, MIM #617967). Both phenotypes are transmitted in an autosomal recessive inheritance pattern. Loss-of-function type variants in the WDR91 gene have also been recently described in patients with brain malformations such as hydrocephalus, brai

[Hydrocephalus in childhood : causes and imaging patterns].

CLINICAL ISSUE: Causes and imaging patterns of hydrocephalus differ depending on the age of the patient. Traditionally, hydrocephalus was classified into non-communicating and communicating hydrocephalus but more recent classifications also take the site of occlusion and the etiology into account. DIAGNOSTICS: For the diagnostic work-up computed tomography (CT), sonography and magnetic resonance imaging (MRI) are available and MRI is the method of choice for children and adolescents as it allows determination of the cause and location of a possible obstruction. In the first 12-18 months sonog

Prenatal diagnosis of posterior fossa anomalies: Additional value of chromosomal microarray analysis in fetuses with cerebellar hypoplasia.

OBJECTIVE: To elucidate the relationship between copy number variations (CNVs) detected by high-resolution chromosomal microarray analysis (CMA) and the type of prenatal posterior fossa anomalies (PFAs), especially cerebellar hypoplasia (CH). METHODS: This study involved 77 pregnancies with PFAs who underwent CMA. RESULTS: Chromosomal aberrations including pathogenic CNVs and variants of unknown significance were detected in 31.2% (24/77) of all cases by CMA and in 18.5% (12/65) in fetuses with normal karyotypes. The high detection rate of clinically significant CNVs was evident in fetuses w

Discrepancy in fetal sex assignment between cell free fetal DNA and ultrasound.

Noninvasive prenatal testing utilizing free fetal DNA is commonly used in pregnancy to screen for trisomy 13, 18, 21 and also fetal sex aneuploidy. We report on two cases of discrepancy between phenotypic and genotypic sex and potential medical implications. In our first case, a patient with known male gender via cell free fetal DNA (cffDNA) testing had an ultrasound at 18 weeks' gestation, which identified Dandy-Walker malformation and female-appearing ambiguous genitalia. As Dandy-Walker malformation could not be considered isolated in the presence of ambiguous genitalia, this finding allowe

Phenotypic and genetic characteristics of children with developmental delay or intellectual disability: a cohort from Northwest China.

BACKGROUND: Intellectual disability/global developmental delay (ID/GDD) is a common neurodevelopmental disorder in childhood. In this study, the causal effects of genetic variant categories on clinical phenotypes in children with ID/GDD from Northwest China were examined under a prespecified analysis controlling all measured covariates. METHODS: Karyotype analysis was performed in 25 children and whole-exome sequencing (WES) in 225 of 250 children with suspected ID/GDD from Northwest China. Among the 250 children, 8 had reportable chromosomal abnormalities, 9 had reportable copy number varian

Nivolumab-induced type 1 diabetes mellitus as an immune-related adverse event.

BACKGROUND: Immune checkpoint inhibitors are medications that activate anti-tumor responses by disrupting the inhibitory signaling to T cells. Nivolumab is an immune checkpoint inhibitor that selectively blocks the programmed cell death-1 (PD-1). Anti-PD-1 agents can breach immunologic tolerance. Fulminant diabetes is an immune endocrinopathy that results from a violent immune attack leading to complete destruction of pancreatic beta cells. We present a rare case of fulminant diabetes precipitated by anti-PD-1 immunotherapy. CASE: A 49-year-old male with a body mass index of 26.4 kg/m2, a his

Complete trisomy 9 with unusual phenotypic associations: Dandy-Walker malformation, cleft lip and cleft palate, cardiovascular abnormalities.

OBJECTIVE: Trisomy 9 is a rare chromosomal abnormality usually associated with first-trimester miscarriage; few fetuses survive until the second trimester. We report two new cases of complete trisomy 9 that both present unusual phenotypic associations, and we analyze the genetic pathway involved in this chromosomal abnormality. CASE REPORT: The first fetus investigated showed Dandy-Walker malformation, cleft lip, and cleft palate) at the second trimester scan. Cardiovascular abnormalities were characterized by a right-sided, U-shaped aortic arch associated with a ventricular septal defect (VS

Ventriculoperitoneal shunt failure causing myelopathy in a patient with bilateral jugular vein occlusion. Case report.

The authors describe the case of a 36-year-old woman with bilateral internal jugular vein occlusion, hydrocephalus, and Dandy-Walker variant who presented with myelopathy that was ultimately attributed to ventriculoperitoneal (VP) shunt failure. Computed tomography (CT) angiography of the head and neck revealed epidural venous engorgement within the cervical spine, greater that 50% narrowing of the C2-5 spinal canal, and compression of the cervical spinal cord. After successful shunt revision, postoperative CT angiography revealed decreased venous engorgement as well as decompression of the ce
