Opiate dependence, comorbidity and seasonality of birth.

OBJECTIVE: In contrast with the non-opiate dependent population, persons biologically-dependent upon opioids display an excess life-time prevalence of affective and anxiety disorders. Many of these addicts state that opiates, particularly methadone, relieve or diminish the severity of their dysphoria. The purpose of this study is to explore this phenomena by analyzing how a specific population of long-term addicts (mean years of addiction 16.9, SD 3.8) differs from a non-opiate dependent population regarding seasonality of birth. METHODS: Birth months were determined for 457 opiate dependent

Relationships of dissociative disorders and personality traits in opium addicts on methadone treatment.

BACKGROUND: Drug abuse is a major public health problem. Some believe that when dissociation fails to defend against emotional, physical, or sexual trauma, the person will find relief from unpleasant thoughts and emotions in opium use. On the other hand, personality disorders are considered as important predictors of treatment outcomes in drug abusers. Due to lack of adequate research in this regard, we evaluated dissociative disorders and personality traits of opium addicts on methadone treatment. METHODS: This cross-sectional analytic study included 111 non-psychotic subjects on methadone t

[Alcohol dependence and abuse and psychiatric disorders].

Alcohol dependence increases the risk of associated psychiatric disorders. The most common disorders are depression, anxiety and personality disorders. Depression is usually secondary to alcohol dependence. It is often associated with an increased risk of suicidal behavior. The best suited treatment for an association between alcohol dependence and depression is alcohol withdrawal. Antidepressant treatment should only be initiated after at least two weeks of complete withdrawal. Alcohol withdrawal is also suggested for the association between anxiety and alcoholism. Some forms of anxiety, such

The effect of a history of conduct disorder in adult major depression.

This paper examines the impact of a history of conduct disorder on major depression in adulthood, including its symptomatology, comorbidity and response to tricyclic antidepressants. 103 subjects with DSM-III-R major depression were assessed for DSM-III-R axis I and II comorbidity, severity of depression, social functioning, general psychiatric symptomatology, early familial environment and family psychiatric history. 18 patients (17%) had a history of conduct disorder, 32 (31%) were subthreshold, and 53 (51%) had no childhood symptoms of conduct disorder. Depressed adults with a history of co

Temperament and character profile of eating disorders: a controlled study with the Temperament and Character Inventory.

OBJECTIVE: The question whether anorexia nervosa (AN) and bulimia nervosa (BN) are different disorders or the extremes of a psychopathologic spectrum still has no definite answer. A way to face this problem is to examine the personality traits underlying these disorders. METHOD: The Temperament and Character Inventory (TCI) was administered to 141 anorectics (70 Restrictor and 71 Binge-purging), to 102 bulimics, and to 100 controls. The Temperament and Character Inventory main scales are Novelty Seeking, Harm Avoidance, Reward Dependence, Persistence, Self-Directedness, Cooperativeness, and S

Prevalence and severity of mental health-related disability and relationship to diagnosis.

OBJECTIVE: Psychiatric disability has been defined largely from measures that focus on serious mental illness. This practice may have led to substantial underestimation of the total impact of mental disorders on community health. In this study a generic measure of mental health-related disability was used to examine disabilities attributable to various common mental disorders. METHODS: Data were drawn from the Australian National Survey of Mental Health and Wellbeing, a household survey of 10,641 adults that assessed participants for 14 DSM-IV disorders with use of the Composite International

Affective temperaments in heroin addiction.

BACKGROUND: Much of the literature has described personality disorder constructs for heroin addicts. Building on previous work we examine the relevance of affective temperament measures in these patients. METHODS: 59 consecutive stabilized methadone treated heroin addicts, 34 with and 25 without psychiatric comorbidity, were compared, regarding affective temperaments, according to the Akiskal and Mallya formulation, with 58 healthy volunteers sharing similar social and regional demographics. RESULTS: No differences were observed between heroin addicts and controls on either depressive or hyp

Bond disorders of a patient with asthma: a case study.

The issue of treatment and providing preventive activities in bronchial asthma is still not fully investigated. Besides the medical aspect, psychiatrists and psychologists point to psychosomatic aspects, since psycho-social factors can evoke intensification of asthma symptoms, and also become the consequences of that disease. Previous psychosomatic investigations show that chronic illness makes the patients reveal extended dependence on others. Patients may also tend to perceive other people as being responsible for their health problems. Such an attitude is usually caused by the disease proce

Psychotherapy with phobic patients: the marriage relationship as the source of symptoms and focus of treatment.

Four patients demonstrating phobic symptoms occurring in the context of maladaptive marriages are presented. Thorough historical data collection is stressed. Similarities in family backgrounds and personality styles emerge in these cases and are discussed in relation to genesis of symptoms within the marital relationships. A psychotherapeutic approach is described and results of treatment examined.

Pathological gamblers are neither impulsive nor sensation-seekers.

Ten pathological gamblers, ten alcoholics, ten heroin addicts and twenty five non-patients were compared using Zuckerman's Sensation Seeking Scale (SSS) and Barratt's Impulsivity Scale (BIS). The pathological gamblers did not differ from the non-patient group on either measure. Drug addicts scored significantly higher and alcoholics significantly lower than the pathological gamblers and the non-patient group on the SSS, while drug addicts scored significantly higher than the other groups on the BIS. The difficulties in defining impulsivity are discussed. It is suggested that the classification

GRM8 genotype is associated with externalizing disorders and greater inter-trial variability in brain activation during a response inhibition task.

OBJECTIVE: The present investigation tested the association of a novel measure of brain activation recorded during a simple motor inhibition task with a GRM8 genetic locus implicated in risk for substance dependence. METHODS: 122 European-American adults were genotyped at rs1361995 and evaluated against DSM-IV criteria for Alcohol Dependence, Cocaine Dependence, Conduct Disorder, and Antisocial Personality Disorder. Also, their brain activity was recorded in response to rare, so-called "No-Go" stimuli presented during a continuous performance test. Brain activity was quantified with two indic

Psychiatric, psychosocial, and physical health correlates of co-occurring cannabis use disorders and nicotine dependence.

BACKGROUND: Several gaps in the literature on individuals with co-occurring cannabis and tobacco use exist, including the extent of psychiatric, psychosocial, and physical health problems. We examine these gaps in an epidemiological study, the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC), of a large, nationally representative sample. METHODS: The sample was drawn from Wave 2 NESARC respondents (N=34,653). Adults with current cannabis use disorders and nicotine dependence (CUD+ND) (n=74), CUD only (n=100), and ND only (n=3424) were compared on psychiatric disorders,

The relationship among three models of personality psychopathology: DSM-III-R personality disorder, TCI scores and DSQ defences.

BACKGROUND: Current systems of describing personality pathology have significant shortcomings. A polydiagnostic approach is used to study the relationship between psychological, psychoanalytical and psychopathological models of personality. METHODS: The subjects were 256 patients enrolled in treatment studies of major depression and bulimia nervosa. Subjects were assessed using the Temperament and Character Inventory (TCI), the Defense Style Questionnaire (DSQ) and the Structured Clinical Interview for DMS-III-R personality disorders (SCID-II). RESULTS: Subjects had high rates of DSM-III-R p

[Differentiation and therapy of delinquent behavior in borderline disorders. Psychodynamic approach].

In the literature about borderline patients, self-destructive dependency needs are described, as well as oral-aggressive root conflicts. Using case studies, the author points out that in persons with borderline personality organisation, self-destructive forms of delinquency are as likely to manifest themselves as aggressive forms are. The first form is connected with sexual abuse, the second with physical abuse. Therefore, delinquency should be differentiated according to the traumatic experiences. Finally, depending on the direction of delinquency-either directed against oneself or against ot

Alcohol use moderates the relationship between symptoms of mental illness and aggression.

Diagnosis of mental illness (MI) inconsistently predicts aggressive behavior although co-occurrence of substance use appears to increase the frequency of aggression in MI populations. We propose that alcohol use should moderate the relationship between mental disorders marked by deficits in self-control and aggression and victimization. In the present study, alcohol use, physical aggression perpetration, physical aggression victimization, injury and psychiatric symptoms were assessed in a sample of 297 substance use disorder patients (102 women; Mage = 38.9, SD = 20.2) recruited from a residen

Psychopathology in methamphetamine-dependent adults 3 years after treatment.

INTRODUCTION AND AIMS: Although psychiatric symptoms are frequently observed in methamphetamine (MA) users, little is known about the prevalence of psychiatric disorders in MA-dependent individuals. This is the first study to examine the association of psychiatric disorders with substance use and psychosocial functioning in a large sample of MA users 3 years after treatment. We predicted that psychiatric diagnoses and severity would be associated with substance use and poorer overall functioning over the 3 year post-treatment course. DESIGN AND METHODS: Participants (N = 526) received psychos
