Exposure and response prevention therapy augmented with naltrexone in kleptomania: a controlled case study using galvanic skin response for monitoring.

BACKGROUND: Kleptomania is a disease that shares features with obsessive compulsive spectrum disorders (OCD) and with substance abuse disorders (SAD). This is underlined by therapeutic approaches in kleptomania ranging from cognitive behavioural therapy and selective serotonin reuptake inhibitors that are effective in OCD, and opioid antagonists that are currently being used in SAD. However, almost no literature exists about exposure and response prevention (ERP) therapy in kleptomania. Furthermore, there is a clear lack of objective markers that would allow a therapeutic monitoring. AIM: To

Impulse Control Disorders - The Continuum Hypothesis.

The group Parkinson Inside Out is composed of health professionals and academic researchers who have been diagnosed with Parkinson's Disease. In our discussions we try to make use of both our inside perspective as patients, and our outside perspective as professionals. In this paper, we apply the two perspectives to the Impulse Control Disorders. These impulsive behaviour patterns are thought to be relatively uncommon side effects of some of the medication used in dopamine replacement therapy. The phenomenon is usually described as relatively rare (<15%), and mainly confined to patients with s

Skin picking in a non-clinical sample of young Polish adults. Prevalence and characteristics.

INTRODUCTION: The aim of the study was to examine the prevalence and characteristics of skin picking behaviors in a sample of young Polish adults. METHODS: Five hundred and thirty-four participants completed measurements of skin picking frequency and severity. They also retrospectively rated the intensity of affective states experienced before, during and after skin picking episodes. RESULTS: In total, 46.07% of the participants endorsed some forms of skin picking, and the prevalence of skin picking disorder (SPD) in the study sample amounted to 7.67%. The characteristics of skin picking epi

Emotion dysregulation and symptoms of anorexia nervosa: the unique roles of lack of emotional awareness and impulse control difficulties when upset.

OBJECTIVE: Extant research suggests that individuals with anorexia nervosa (AN) have deficits in emotion regulation across a variety of domains. The current study investigated associations between specific difficulties with emotion regulation and the core symptoms of AN. METHOD: Participants were 192 patients with AN presenting to an intensive eating disorder treatment facility. Emotion regulation was assessed using the multidimensional Difficulties in Emotion Regulation Scale, and associations with body mass index (BMI) at admission, eating disorder cognitions, objective binge eating, subjec

Gambling on the stock market: an unexplored issue.

OBJECTIVE: Stock market investment (SMI) is one of the most socially acceptable types of gambling, which, however, can turn into a gambling problem. Because it is barely examined, we compared a series of clinical, psychopathologic, and personality variables in SMI gambling patients (both as primary and secondary problem) with a group of traditional pathologic gamblers (PGs). METHOD: A total sample of 1470 PGs (1376 patients without SMIs [PG-SMI], 76 patients with SMI as a secondary gambling problem [PG+SMI], and 18 patients with SMI as a primary gambling problem [SMI+PG]) participated in this

Screening hypersexuality in Parkinson's disease in everyday practice.

OBJECTIVE: The purpose of this study was to develop a short and reliable measure of hypersexuality that could be used in everyday practice in patients with Parkinson's disease (PD). DESIGN: The original questionnaire containing twenty-five-items, the Sexual Addiction Screening Test (SAST), was shortened and tested in a PD population. METHODS: Successive reductions were performed until a final set of items satisfied the model fit requirements. The testing phase consisted of administering the SAST questionnaire to 159 PD patients. It included i) acceptability, ii) dimensionality construct vali

Executive functions in a virtual world: a study in Parkinson's disease.

In Parkinson's disease executive functions are altered. We used a Virtual Reality version of the Multiple Errand Test in order to evaluate decision-making ability in 12 patients and 14 controls. Patients with Parkinson's disease, even if not-demented, showed strategies full of errors, suggesting that impulse control disorder, very frequent in course of disease, could precede cognitive dysfunctions.

Impulsivity, attribution and prison bullying: bully-category and perpetrator-victim mutuality.

The current study explores bullying behaviours among adult male prisoners, examining its relationship with aggression attribution and impulsivity. Employed are two separate methods of analysis to determine how this may influence results. Participants were 102 prisoners. All completed a revised version of the Direct and Indirect Prisoner behaviour Checklist (DIPC-R), the Barratt Impulsivity Scale: Version II (BIS-12) and the Expressive Aggression Scale (EXPAGG). Analysis included categorical analysis with prisoners placed into one of four groups (pure bully, pure victim, bully/victim and not-in

Famous people with Gilles de la Tourette syndrome?

Virtually no neurologist nor psychiatrist today can be unaware of the diagnosis of Gilles de la Tourette syndrome (GTS). Although the eponymous description by Dr. Georges Gilles de la Tourette was published in 1885, familiarity with this syndrome has been achieved only recently. In this article, the two most renown accounts of exceptional individuals retrospectively diagnosed with GTS are critically analyzed: British lexicographer Samuel Johnson and Austrian musician Wolfgang Amadeus Mozart. In both cases, clinical descriptions have been retrieved from written documents predating Gilles de la

The material culture of suicidal fantasies.

In this study, the author examined the content of impulsive suicidal fantasies among U.S. undergraduates. Of 546 participants, 45.6% reported incidents in which they fantasized about killing themselves. Most fantasies (95%) included explicit references to suicide methods, and most of those fantasies (98%) involved human-made tools or materials (e.g., drugs, cutlery, firearms). The methods reported in most suicidal fantasies seem to have been suggested by the mass media. The findings suggest that suicidal thoughts do not occur without explicit and detailed information about suicide methods invo

Correlates of restraint use in an institutional population: a replication.

BACKGROUND: Reducing inappropriate restraint is an important mission of services for people with intellectual disabilities (IDs). METHODS: In this study, 52 clients who had been restrained were compared with 52 other clients, individually matched on age, gender and level of IDs. Participants were mostly adults with severe and profound IDs in an institutional setting. RESULTS: The two groups differed on six Diagnostic Assessment for the Severely Handicapped-II (DASH-II) and three Aberrant Behaviour Checklist (ABC) scales. However, discriminant functional analysis demonstrated that only DASH-I

Clinical phenomenology of episodic rage in children with Tourette syndrome.

OBJECTIVE: Episodic rage of unknown etiology causes significant morbidity in children with Tourette's syndrome (TS). Using modified Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) criteria for intermittent explosive disorder (IED), we developed a screen and symptom questionnaire to explore rage attack phenomenology and to preliminarily investigate whether symptom clusters can identify clinical subgroups of TS children with rage attacks. METHODS: 48 children with TS between ages 7 and 17 years consecutively presenting with rage attacks completed the Rage Attacks Screen and Quest

Treating impulsivity and anxiety in the suicidal patient.

It has been found that while over 90% of people who commit suicide have a psychiatric illness at the time, over 50% are under active psychiatric or mental health care. How can suicide risk be detected and preventive treatment provided? Both communication of suicidal ideation or intent and prior suicide attempts have been shown to be risk factors, which should be assessed in patients with psychiatric illness. Research shows that suicidal ideation is often not communicated to professionals or is denied by patients just prior to suicide and, when present, is often useful not as an immediate risk

Problem and probable pathological gambling: considerations from a community survey.

OBJECTIVE: To investigate the nature and extent of gambling problems in a region of Canada in which legalized gambling activities were expanded during the 1990s. METHOD: A standardized telephone interview was conducted with a random sample of 738 community-dwelling adults (response rate 74%) in Winnipeg, Manitoba. RESULTS: According to traditional classification criteria, the lifetime prevalence of "probable pathological gambling" was 2.6%. A further 3.0% of the sample met criteria for traditionally defined "problem gambling," and evidence suggests that both types of gamblers share several c

Low platelet monoamine oxidase activity in pathological gambling.

Decreased platelet monoamine oxidase (MAO) activity has been reported in association with sensation-seeking personality type and in some mental disorders associated with a lack of impulse control. Pathological gambling itself has been related with both sensation-seeking and reduced impulse control. Platelet MAO activity was investigated in 15 DSM-III-R pathological gamblers from our outpatient clinic. Gamblers had a significantly lower platelet MAO activity than a group of 25 healthy controls. The range of MAO levels in gamblers was also significantly shorter than in controls. In controls, pla

[Difficulties in learning new ideas in children with specific mental disturbances].

AIM: Results of research on the learning of ideas by children with specific mental disturbances are presented in the article below. In the introduction, there are some psychological concepts related to the learning of ideas, and psychopathological concepts concerning the subject of research. METHOD: In the further part of the article, Andrzej Lewicki's views are presented. His concept of abstraction (i.e. positive and negative abstraction) constitutes a development of classical views deriving from the Wurzburg school. RESULTS: In the article, there are the results of a clinical experiment wi
