Frozen elephant trunk in acute aortic type a dissection: risk analysis of concomitant root replacement.

OBJECTIVES: To analyse whether full root replacement increases perioperative risks in patients who undergo frozen elephant trunk for acute aortic dissection. METHODS: Between March 2013 and December 2019, 115 patients underwent emergency frozen elephant trunk for acute dissection. Patients without root replacement were assigned to group A, while patients with concomitant full root replacement to group B. RESULTS: Mean age was 50.8 (12.5) years and 85 (73.9%) patients were male. Preoperative malperfusion was present in 49 (42.6%) patients. In group B, 27 (41.5%) patients received composite ro

[A case of concomitant aortic stenosis and aortic arch aneurysm: a combined endovascular approach].

Aortic valve stenosis and aortic aneurysmal disease are increasingly prevalent with advancing age. When associated, their treatment is very challenging. A female patient with previous Tirone-David procedure presented to our hospital with acute heart failure. She was diagnosed with severe aortic stenosis, aneurysm of the aortic arch and the descending thoracic aorta. She underwent successful concomitant aortic arch TEVAR and transcatheter aortic valve repair, with optimal acute and mid-term result. Our case demonstrates that a careful pre-procedural planning, along with a good cooperation betwe

A new device as an open stent graft for extended aortic repair: a multicentre early experience in Japan.

OBJECTIVES: Open stent grafting for extended aortic repair has been widely carried out around their world. We reported the effectiveness of a new device as an open stent graft for extended aortic repair. METHODS: A new device was used as an open stent graft in this study. The graft part of the device has a woven structure made of Nitinol wire, a superelastic/shape-memory alloy. The subjects of this study were patients aged 20-84 with aneurysms (n = 38) or aortic dissection (n = 22) in the thoracic aorta, including the distal aortic arch and the proximal descending aorta. This study was a mult

Branched stent-grafts deployed in the true and false lumen to exclude a post dissecting thoracoabdominal aneurysm.

The aim of this case report was to present an endovascular treatment of a residual post-dissection Crawford type III thoracoabdominal aneurysm (TAA). A 60-year-old man, who had suffered from acute type B aortic dissection (TBAD) 8 years ago and had already been treated for both descending thoracic and infrarenal aortic aneurysm with open repair, presented with a 61mm post-dissection TAA. The aneurysm was successfully excluded with a staged fully endovascular procedure by employing two multibranched custom-made stent grafts deployed into the false lumen. The first stent-graft with a proximal mo

Treating the thoracic aorta in Marfan syndrome: surgery or TEVAR?

Marfan syndrome (MFS) is an inherited disorder of connective tissue that is historically associated with high mortality due to disorders of the cardiovascular system. Over the past 40 years, surgery to the aortic root and thoracoabdominal aorta has resulted in a significantly prolonged life expectancy. More recently, techniques for endovascular repair of the aortic complications of MFS have been described. In this article we review the relative merits of open and endovascular approaches to thoracic aortic complications in MFS, comparing outcomes from contemporary literature. A reasonably large

The vertebral artery is unlikely to be the sole source of vascular complications occurring during stellate ganglion block.

INTRODUCTION: Stellate ganglion block (SGB) is commonly performed for upper extremity complex regional pain syndrome and other conditions. Known complications of stellate block include Horner's syndrome, hoarseness, hematoma formation, airway compromise, immediate seizure (presumably from vertebral artery injection), and death. A previous arterial anatomy study demonstrated other vessels, eg, the ascending and deep cervical arteries, reinforcing the blood supply of the spinal cord and brain stem. The potential role of these vessels in the pathogenesis of seizures or hematoma during SGB has not

Blood Vessel Topography of the Feet in Selected Species of Birds of Prey and Owls.

Birds of prey and owls are susceptible to diseases of and traumatic injuries to their feet, which regularly require surgical intervention. A precise knowledge of the blood vessel topography is essential for a targeted therapy. Therefore, the metatarsal and digital vasculature was examined in eight species of birds of prey and owls. The study included contrast micro-computed tomography scans and anatomical dissections after intravascular injection of colored latex. In all examined species, the dorsal metatarsal arteries provided the main supply to the foot and their branching pattern and number

Anatomic variations in femoral head circulation.

The purpose of this cadaveric study was to clearly define the blood supply to the femoral head to help further reduce the incidence of iatrogenic avascular necrosis (AVN). Thirty-five hips of twenty-eight fresh cadavers were injected with colored silicone. Anterior and posterior dissection was performed to assess the vessels contributing to femoral head vascularity. The medial femoral circumflex artery (MFCA) was found to be the main blood supply to the hip in twenty-nine specimens; the inferior gluteal artery (IGA) was found to be the main blood supply in six. The MFCA consistently provided a

Current Management of Uncomplicated Type B Aortic Dissection.

Aortic dissection is the most common thoracic aortic emergency and is associated with significant morbidity and mortality. Initial complications are dependent on reduction of sheer stress against the aortic wall to protect against rupture and minimize progression of the aortic wall injury. In patients with dissection starting at or distal to the left subclavian artery (Stanford type B), initial management includes strict blood pressure and heart rate control with monitoring for any complications such as malperfusion, rupture, or hemodynamic instability. Following the acute dissection event, su

Spontaneous Coronary Artery Dissection: Rediscovering an Old Cause of Myocardial Infarction.

Spontaneous coronary artery dissection is an uncommon, but not insignificant cause of acute coronary syndrome that overwhelmingly affects middle-aged women. The pathophysiology of coronary dissection appears to be an outside-in mechanism, where the initiating event is not an intimal tear but rather the formation of an intramural hematoma, which compromises blood flow by reducing the arterial lumen. Considering this mechanism, it is clear to see how intracoronary imaging techniques, such as optical coherence tomography and intravascular ultrasound, are most accurate in the diagnosis. However, t

Endovascular Treatment Options for Chronic Dissections.

Aortic dissections are classified based on the location of the entry tear. A major concern in chronic aortic dissections is aneurysmal degeneration. Aneurysmal degeneration can lead to aortic expansion and potentially death. False lumen patency is a predictor of outcomes in type B aortic dissections. Thoracic endovascular aortic repair (TEVAR) can be used to treat chronic aortic dissections. TEVAR aims to occlude the false lumen to promote thrombosis and favorable aortic remodeling. However, its benefit may be limited in chronic dissection. Several techniques have been developed to treat chron

[Complicated with Spinal Cord Injury After Frozen Elephant Trunk Procedure for Stanford Type A Acute Aortic Dissection].

We report two cases complicated with spinal cord injury after the frozen elephant trunk procedure for Stanford type A acute aortic dissection. The two patients experienced dissection from the ascending aorta to the iliac arteries, and posterior false lumens were seen from Th8 to L2 in both cases. After an emergent surgery, both patients showed paraplegia and underwent immediate spinal fluid drainage. Symptoms improved to paraparesis in both patients, who were subsequently transferred to rehabilitation hospital. If a posterior false lumen is present from Th8 to L2, an increased risk of complica

Giant dissecting aneurysm of the middle cerebral artery (MCA): report of four cases-successfully treated by microsuture technique.

INTRODUCTION AND IMPORTANCE: This study aims to evaluate the microsuture technique in treating giant dissecting aneurysms of the middle cerebral artery (MCA) trunk, particularly in cases involving perforators-a key surgical challenge. METHODS: This reconstruction of the MCA trunk was performed in four patients with giant dissecting aneurysms using a microsuture technique. The fragile wall of the dissecting aneurysm was meticulously excised while preserving the perforating arteries originating from the aneurysm body. Subsequently, a bypass was established from the internal maxillary artery (IM

Placement of stent grafts with different calibers into the true and false lumens of anastomotic aneurysms after descending aortic replacement for aortic dissection.

Anastomotic aneurysms present as a life-threatening emergency after descending aortic replacement for aortic dissection. Thoracic endovascular aneurysm repair (TEVAR) has been performed since the early 2000s for complicated cases in which re-thoracotomy cannot be adopted. We report the case of a 57-year-old male patient, during a 5-year follow-up after descending aortic replacement for aortic dissection, developed aneurysm expansion around the false lumen on the peripheral side of the artificial graft. Considering the risk and the patient's desires, we opted to perform TEVAR with different cal

Aortic Root Reinforcement Combined with Vascular Grafts Eversion and Built-in Procedure (XJ-Procedure) for Acute Type A Aortic Dissection Surgery.

INTRODUCTION: Current root reinforcement methods for acute type A aortic dissection (ATAAD) risk the tearing of endothelial tissue by sutures. This study proposed a novel technique for aortic root reinforcement and evaluated its effectiveness. METHODS: Patients who diagnosed with ATAAD and had mild to moderate aortic root involvement, combined with aortic arch involvement undergoing Sun's procedure in the First Affiliated Hospital of Xi'an Jiaotong University from January 2020 to December 2021, were retrospectively enrolled. They were divided into two groups according to their surgical proced
