Clinical diagnosis of primary epiploic appendagitis: differentiation from acute diverticulitis.

BACKGROUND: Primary epiploic appendagitis (PEA) is an uncommon cause of abdominal pain that occurs either from appendageal torsion or spontaneous thrombosis of an appendageal draining vein. Primary epiploic appendagitis is frequently misdiagnosed as either appendicitis or diverticulitis, depending on its location. STUDY: Clinical and radiologic characteristics of 8 patients with PEA were retrospectively reviewed and compared with 18 patients with acute diverticulitis. RESULTS: Patients with PEA presented with lower abdominal pain of recent onset that was localized to the left (seven cases) a…

[Complication of acute diverticulitis: colo-vesical fistula (a clinical case)].

BACKGROUND: A patient presenting a colo-vescical fistula together with a number of other pathologies, and the absolute absence of a set scheme for the treatment of septic diverticular complications has prompted us to publish this case. PATIENT AND METHODS: The patient aged 80 was admitted urgently, diagnosed with intestinal occlusion. Generally poor conditions: renal insufficiency, abdomen globose and hypertympanic, systaltic mass in the epigastric site. The patient had experienced abdominal pains for about 4 months; severe tenesmo during evacuation: fever temperature as high as 38.5 degrees

Same admission colon resection with primary anastomosis for acute diverticulitis.

Current standard of care for complicated diverticulitis includes urgent resection with colostomy versus antibiotic treatment, followed by delayed resection with primary anastomosis at a second admission. In certain circumstances, it is possible to perform resection and anastomosis on the same admission for acute diverticulitis. A retrospective review was completed for patients undergoing surgery for diverticulitis from 1991 to 1998. Groups included: 1) sigmoid resection with primary anastomosis on same admission (n = 18); 2) resection with protective end colostomy (n = 16); and 3) in-patient a…

Acute colonic diverticulitis: prospective comparative evaluation with US and CT.

PURPOSE: To compare the accuracy of ultrasonographic (US) and computed tomographic (CT) findings for diagnosis of acute colonic diverticulitis. MATERIALS AND METHODS: US and CT were prospectively performed in 64 consecutive patients suspected of having acute colonic diverticulitis. Images were interpreted independently in a blinded fashion. Imaging data were compared with the final diagnosis, which was based on initial clinical and follow-up examination results (n = 64) and pathologic (n = 22), endoscopic (n = 21), and contrast enema (n = 15) examination findings. RESULTS: Final diagnosis wa…

[Intestinal perforation caused by chicken bone mimicking perforated colonic diverticulitis].

Intestinal Perforation can occur in less than 1% of cases of ingestion of foreign bodies. Clinical suspicion is very important to make the diagnosis since many medical conditions can simulate this pathology. We present a case of a 63 years old man who swallowed a chicken bone that perforated the sigmoid colon and produced a clinical picture mimicking a perforated colonic diverticulitis. At surgery, the bone was removed without any evidence of abscess formation. A Hartmann procedure was performed and we reanastomosed it 3 month later. The patient followed an uneventful postoperative course. We …

Prognostic factors from computed tomography in acute left colonic diverticulitis.

This prospective study examined factors which may predict a poor outcome (complications and recurrence) after a first attack of diverticulitis which has been successfully managed conservatively. Twenty-four of 107 patients who entered the study had a poor outcome: persistent diverticulitis (nine cases), recurrence (seven cases), colonic stenosis (six cases), residual parasigmoid abscess (one case) and colovesical fistula (one case). Eight of the 18 men aged 50 years or less had a poor outcome compared with 16 of the remaining 89 patients (P = 0.032). Twelve of 76 patients (16 per cent) with mi…

Hepatic abscess resulting from asymptomatic diverticulitis of the sigmoid colon.

I have described a case of pyogenic liver abscess secondary to completely asymptomatic diverticulitis. Because extensive diverticulitis may be present with minimal or no clinical symptoms, it should be strongly considered as the cause when dealing with a so-called "cryptogenic" liver abscess, and treatment should be planned accordingly.

[Pseudo-neoplastic sigmoid diverticulitis from radiology to diagnosis (author's transl)].

Pseudo-neoplastic sigmoid diverticulitis, a rare clinical form of pseudo-tumoural sigmoiditis, is characterised by the difficulty of its clinical, radiological, endoscopic and even per-operative diagnosis. None of these various phases in study may suffice to eliminate the possibility of an associated carcinoma. The diagnosis may be made by opening the operative specimen, but this is too late in time. The appearance of the bladder, rather than misleading the diagnosis, may provide arguments in favour of the benign nature of these pseudo-tumours.

Colonic intussusception masquerading as diverticulitis: a case report of a 31 year old diagnosed with colorectal carcinoma after multiple presentations to the ED for episodes of acute diverticulitis.

Colonic intussusception is a rare occurrence in adults, with few reported cases in the literature. Patients often present with nonspecific a … with left lower quadrant abdominal pain. Multiple computed tomography scans showed inflammation and diverticulitis of the mid-descending colon along with a short segment of colonic intussusception. A colonoscopy was performed due to concern for malignancy. A partially obstructing mass was found in the descending colon that could not be traversed. Biopsies reveal …

Acute diverticulitis in 2024: current evidence to prevent onset and recurrence.

INTRODUCTION: The worldwide increase in acute diverticulitis (AD) prevalence and the resulting growing economic burden on the healthcare system have driven the scientific community towards standardizing a methodological approach to obtain a prompt diagnosis, optimized treatment, and thus containing costs. By the analysis of currently available evidence, this review could provide effective strategies for efficient AD clinical management and highlights possible innovative therapeutic strategies. EVIDENCE ACQUISITION: A systematic literature search was conducted from October 2014 to October 2024…

Diet and Risk for Incident Diverticulitis in Women : A Prospective Cohort Study.

BACKGROUND: Patients with diverticulitis often attempt to control their diet with a particular focus on avoiding nuts and seeds. However, whether dietary patterns or dietary intake of nuts and seeds are associated with diverticulitis risk is poorly studied, particularly in women. OBJECTIVE: To determine whether select diets affect incident diverticulitis risk in women. DESIGN: Prospective cohort study. SETTING: Cohort study in the United States and Puerto Rico. PARTICIPANTS: Women aged 35 to 74 years at enrollment who responded to food frequency and diverticulitis questionnaires and had no…

Antibiotic Prescription for Acute Diverticulitis: Evaluation of Compliance With National Guidelines.

INTRODUCTION: Diverticulitis is a common surgical problem that can be categorised as uncomplicated or complicated diverticulitis. National guidelines for managing acute diverticulitis recommend that antibiotics should only be given to patients who are systemically unwell, have a background of immunosuppression or have complicated diverticulitis. We analysed the compliance with these guidelines at our hospital, identified gaps, and introduced appropriate measures to improve service provision. METHODS: This was a closed-loop audit. The first cycle was a retrospective analysis conducted between …

Fistula formation in recurrent sigmoid diverticulitis - a domain of laparoscopic surgery?

INTRODUCTION: Chronic sigmoid diverticulitis is the most common benign cause of sigmoid-bladder fistulas (SBF) and sigmoid-vaginal fistulas (SVF). This multicenter retrospective comparative study analyzed the perioperative and postoperative outcomes between laparoscopic and open surgical procedures. METHODS: The study included 101 patients from four German hospitals who underwent elective sigmoid resection for SBF, SVF, or combined fistulas between January 2010 and July 2024. Patient data were retrospectively analyzed, comparing outcomes based on the surgical approach. RESULTS: Of the 101 pa…

A rare concurrence of acute appendicitis with Meckel's diverticulitis: a case report.

BACKGROUND: Meckel's diverticulitis is the inflammation of Meckel's diverticulum (MD), a common congenital structural abnormality of the gastrointestinal tract. The simultaneous occurrence of Meckel's diverticulitis with acute appendicitis is extremely rare. CASE PRESENTATION: A 17-year-old male presented with acute abdominal pain and vomiting, accompanied by tenderness in the right iliac fossa. Laboratory investigations revealed neutrophilia and an elevated leukocyte count, while ultrasound findings were consistent with acute appendicitis. The patient underwent an emergency open appendectomy…

Less Is (Sometimes) More: Laparoscopic Peritoneal Lavage and Drainage for Diverticulitis.

Introduction: Laparoscopic lavage and drainage (LLD) emerged as an alternative to Hartmann's procedure (HP) for patients with diverticulitis and uncontained perforation. Although initially popular as a less invasive approach, its use in modern practice is in question. This summary will review the available literature to show techniques, outcomes, and indications. Methods: The literature was reviewed for relevant case studies, randomized trials, prospective series, retrospective analyses, and meta-analyses to define peritoneal lavage and determine the clinical outcomes of peritoneal lavage. Res…

Risk factors and outcomes of renal insufficiency after colectomy for diverticulitis.

BACKGROUND: Diverticulitis has significantly increased in prevalence in recent decades, requiring higher rates of colon resections. While current literature focuses on postoperative complications such as abscesses, wound infections, and anastomotic leaks, many are limited in showing details regarding the significant risk associated with developing renal insufficiency among diverticulitis patients undergoing colectomy. METHODS: We selected patients from the 2022 National Surgical Quality Improvement Program (NSQIP) Colectomy database who underwent colon resection for diverticulitis using Inter…
