Morphology of myenteric plexuses in the human large intestine: comparison between large intestines with and without colonic diverticula.

BACKGROUND: Large intestines with diverticula exhibit functionally abnormal peristaltic activity and elevated luminal pressure that may indicate functional changes in the myenteric plexus; however, no studies have investigated the characteristics of either normal or diverticula myenteric plexuses. METHODS: Tissue specimens obtained from 93 colorectal cancer patients without diverticula, 14 patients with perforated diverticulitis, and 12 colorectal cancer patients with asymptomatic diverticula were included in this study. Myenteric plexuses and ganglion cells were counted per centimeter, and t

Role of CT in diagnosis and management of complications of diverticular disease.

The clinical complications of diverticular disease may be unclear. Seven patients with complicated diverticulitis of the transverse, descending, and sigmoid colon were examined with computed tomography. Exact knowledge of the anatomical relationships of the transverse, descending, and sigmoid colon and neighboring structures are a prerequisite for understanding and interpreting the extensions of peridiverticular disease. Computed tomography proved to be useful in evaluating the presence and extent of sequelae of perforations, and, as a consequence, influenced the planning and timing of treatme

Stercoral Colitis: A Diagnostic Challenge and Therapeutic Approach in an Elderly Patient With Chronic Constipation.

Stercoral colitis, although rare, remains a significant cause of acute colonic inflammation. It is characterized by the presence of fecaloma leading to fecal impaction and subsequent mucosal injury, ultimately resulting in colonic wall inflammation. This condition primarily affects elderly patients with chronic constipation and is associated with significant morbidity and mortality if not recognized and managed promptly. Given its rarity and varied presentation, stercoral colitis often poses a diagnostic challenge. The clinical manifestations can mimic other colonic pathologies, such as divert

Intraoperative colonic lavage and primary anastomosis in nonelective colon resection.

In selected individuals requiring emergency colon resection, intraoperative colonic lavage with primary anastomosis represents a safe alternative to staged reconstruction. This procedure achieves excellent mechanical preparation of the colon, facilitates safe anastomosis, and avoids the disadvantages associated with multistaged operations. At our institution, 25 patients requiring urgent segmental resection of the left colon have undergone intraoperative colonic lavage. Primary anastomosis without fecal diversion has been performed in 21 of these patients. Obstruction of the large intestine wa

Irritable bowel syndrome and colonic diverticular disease: overlapping symptoms and overlapping therapeutic approaches.

PURPOSE OF REVIEW: Irritable bowel syndrome (IBS) is a common symptomatic disorder in the Western world and colonic diverticula are also prevalent; however, relationships between IBS-type symptoms and diverticula have been a source of much debate. Our goal was to reassess these relationships in the light of new data. RECENT FINDINGS: On removing from consideration clinical scenarios which are directly related to diverticula (i.e., diverticulitis, diverticular hemorrhage, and complications of diverticulitis, such as stricture and fistula), relationships between IBS and diverticula can be seen

Segmental colitis associated with diverticulosis (SCAD): imaging features, diagnostic pitfalls, and multidisciplinary correlation.

Segmental colitis associated with diverticulosis (SCAD) is an under-recognized clinical entity characterized by chronic mucosal inflammation localized to a colonic segment affected by diverticular disease. Despite its increasing recognition, it remains frequently misdiagnosed due to clinical and radiographic overlap with acute diverticulitis and inflammatory bowel disease (IBD). This review explores the pathogenesis, clinical presentation, and imaging features of SCAD. We highlight the critical endoscopic and radiologic markers that differentiate SCAD from its mimics and emphasize the necessit

Spontaneous Colovesical Fistula With Benign Prostatic Hyperplasia and Prostatitis.

We present an unusual case of colovesical fistula formed by a single diverticulum manifesting as prostatitis and urinary tract infection (UTI) before causing further complications. Diverticulitis is caused by inflammation of the colonic diverticula and manifests as fever and left lower quadrant pain. The patient also developed recurrent pneumaturia; upon examination, a colovesical fistula was observed on computed tomography (CT), which was managed successfully by laparoscopic repair. This case highlights manifestations of colovesical fistula secondary to diverticular disease and the laparoscop

[Caecal diverticulitis in a young woman with suspected acute appendicitis].

Colonic diverticular disease is common amongst the western population in the left hemicolon while right-sided involvement is rare. This case concerns a 20-year-old female, who was hospitalized with suspected acute appendicitis. Perioperative findings included an intramural tumour at the base of the appendix, and an ileocaecal resection was performed. Histopathological examinations revealed a single inflamed diverticulum of the caecum. The case illustrates the pre- and perioperatively differential diagnostic problems of caecal diverticulitis, which can lead to more radical surgery than needed.

Sigmoid volvulus an update.

Sigmoid volvulus is the third most common cause of colonic obstruction in the United States after cancer and diverticulitis. Etiologic factors include anatomic variation, chronic constipation, neurologic disease, and megacolon. Management of sigmoid volvulus involves relief of obstruction and the prevention of recurrent attacks; the outcome depends on the population and selection of patients. Although volvulus is uncommon, it may be encountered during pregnancy and is a condition that poses significant risk to both mother and fetus requiring a management strategy that varies with each trimeste

Colonic diseases: the value of US examination.

The colon is affected by a number of diseases, mainly inflammatory, ischemic, and neoplastic conditions. Depending upon clinical indications endoscopy, US, CT, or other radiological methods are used for evaluation. The fact that US is frequently used as the initial imaging method in patients with non-specific clinical symptoms allows for greater influence in further diagnostic evaluation and with treatment, provided the investigator is familiar with the features of different intestinal diseases. This article will describe the anatomical characteristics of the colon, the US technique for examin

Alpha-1-antitrypsin deficiency (carrier) as possible risk factor for development of colonic diverticula. A multicentre prospective case-control study: the ALADDIN study.

AIM: Connective tissue changes due to ageing or diseases leading to changes in the colonic wall are one theory for the development of diverticula. Alpha-1-antitrypsin (A1AT), a protease inhibitor that protects connective tissue, possibly plays a role in the aetiology of diverticulosis. The aim of this study was to explore associations between the development of diverticula and A1AT deficiency. METHODS: This was a multicentre prospective case-control study. A total of 221 patients aged ≥ 60 years with acute abdominal pain undergoing abdominal CT were included and analysed. Patients with divert

Gallstone Ileus Mimicking a Colonic Tumor: A Case Report.

INTRODUCTION: Gall stone ileus is a rare complication of cholelithiasis which typically presents with obstruction in the small intestine. However, it can rarely mimic a mass when it presents in unusual sites like the sigmoid colon as in our case. CASE PRESENTATION: We present a 42-year-old woman with a history of bariatric surgery, diverticulitis status post sigmoid colectomy and decompensated cirrhosis complicated by hepatic encephalopathy who presented to the hospital with concern for altered mental status and was diagnosed with grade III hepatic encephalopathy due to lactulose non adherenc

Perforated Stercoral Ulceration of Transverse Colon in the Setting of Multiple Comorbidities: A Case Report.

Stercoral colitis is an inflammatory condition resulting from the long-term collection of fecal matter that may lead to colonic ulceration and perforation, consequently representing a surgical emergency. Herein, we present the case of a 65-year-old female who developed stercoral ulceration of the transverse colon, an uncommon location of occurrence. The patient had multiple comorbid conditions and risk factors for stercoral colitis that included a history of constipation, major depressive disorder, hypothyroidism, and a partial colonic resection due to an upper rectal perforation secondary to

Perforated Diverticulitis with Generalized Peritonitis: Low Stoma Rate Using a "Damage Control Strategy".

PURPOSE: Optimal surgical management of perforated diverticulitis of the sigmoid colon has yet to be clearly defined. The purpose of this study was to evaluate efficacy of a "Damage Control Strategy" (DCS). MATERIALS AND METHODS: Patients with perforated diverticulitis of the sigmoid colon complicated by generalized peritonitis (Hinchey III and IV) surgically treated according to a damage control strategy between May 2011 and February 2017 were enrolled in the present multicenter retrospective cohort study. Data were collected at three surgical centers. DCS comprises a two-stage concept: [1]

Giant colonic diverticulum: percutaneous computed tomography-guided treatment.

We present a case of inflamed giant sigmoid diverticulum, which was treated under computed tomographic guidance by dilating the neck and decompressing the inflamed diverticulum. The procedure also involved placement of an internal stent across the neck of the diverticulum, the purpose of which was to decompress the giant diverticulum into the sigmoid colon and prevent acute recurrence. In patients with inflamed giant colonic diverticulum, the percutaneous approach may be useful in treatment of diverticulitis and avoiding a staged Hartmann procedure with end colostomy.

Robotic versus laparoscopic elective colectomy for left side diverticulitis: a propensity score-matched analysis of the NSQIP database.

PURPOSE: Robotic surgery might have an advantage over conventional laparoscopy for colonic diverticulitis. We intend to compare both approaches in the elective management of left side diverticulitis. METHODS: The National Surgical Quality Improvement Program (NSQIP) database (2012-2014) was surveyed for patients undergoing elective left/sigmoid colectomy for diverticulitis. Patient demographics, co-morbidities, disease complexity, and intraoperative details were matched on propensity scores derived from logistic regression model. RESULTS: We identified 441 robotic and 6584 laparoscopic cases
