A case report of perforated duodenal ulcer in a ten year old Holstein cow is presented. On three occasions, sudden anorexia and rapidly progressing abdominal fluid distension were associated with metabolic alkalosis, hypochloremia and hypokalemia. Rumen fluid at the time of the second episode was acidic and contained an excessive amount of chloride ion. An abdominal mass dorsal to the abomasum involving the pylorus and several loops of small bowel was identified but not corrected at surgery. Necropsy confirmed a 1.5 cm diameter duodenal ulcer 6 cm distal to the pylorus.

A case of successful treatment of a bleeding duodenal varix in a patient with portal hypertension and compensated cryptogenic cirrhosis (Child A) is reported. The 42-year-old man had a history of recurrent gastrointestinal hemorrhage over 14 years. In 1966 he underwent a portocaval shunt operation. Angiography in 1968 revealed a thrombosis of the shunt as well as of the splenic vein. Splenectomy was performed because of hypersplenism. In 1980 bleeding from esophageal varices occurred and was treated by sclerotherapy. Seven weeks after sclerotherapy massive bleeding from a duodenal varix occurr

A case of congenital duodenal diaphragm has been presented. The clinical features of protracted vomiting, epigastric pain and discomfort after meals with a gastric splash occurring at any age, the biochemical features of profound hypokalaemia and hypochloraemic alkalosis, and the radiological demonstration of a diaphragm have been shown to be of diagnostic significance. The operation of vagotomy and gastroenterostomy was unsuccessful in this case, and a literature review suggests that duodenotomy with excision of the diaphragm is the preferred method of treatment.

Seven patients with duodenal injuries following penetrating or blunt upper abdominal trauma are presented. In five, debridement, primary closure and drainage were performed; the postoperative course was uneventful. One other patient developed complications as a results of multiple trauma not directly related to duodenal injury. The seventh patient suffered from duodenal and small bowel fistulas, which closed after prolonged nonoperative treatment. All patients survived. Early diagnosis through accurate exploration and appropriate surgical interventions is essential to reduce morbidity and mort

Over the years, the aeromedical disposition of aircrew members with bleeding duodenal ulcer has been a vexing problem because two essential questions cannot be categorically answered: Is the risk of rebleeding less with medical or with surgical treatment? In either case, is the risk of rebleeding low enough to permit, with reasonable prudence, the resumption of aircrew duties by the aviator? To try to answer these questions, the author has reviewed a portion of the literature and a number of medical records of patients with bleeding duodenal ulcer. With this information, the author draws his c

Cholecystokinin (CCK)-like activities in the duodenal mucosa were measured by bioassay in patients with duodenal ulcer, cholelithiasis and some other gastrointestinal diseases as well as in healthy normal subjects. The mean CCK-like activity of the duodenal mucosa in patients with duodenal ulcer and cholelithiasis was 0.405 Ivy dog units per milligram and 0.376 Ivy dog units per milligram of dry weight of the duodenal mucosa (IDU/mg d.w.), respectively. Both these figures are significantly higher than 0.180 IDU/mg d.w. in normal subjects (p less than 0.005, p less than 0.005, respectively). It

A case is described of gangrenous inflammation of a duodenal diverticulum, into the posterior wall of which, the common bile duct and pancreatic duct were found to open. Treatment consisted of resection of the necrotic diverticulum and restoration of continuity between the papilla of Vater and bowel by means of a pedicle graft from the anterior wall of the duodenum.

Duodenal gastrin release in the dog was studied after mucosal antrectomy with intact duodenal innervation, following which basal gastrin levels fell. Acetylcholine at pH 7 but not at pH 1.5 and insulin hypoglycemia but not 2-deoxyglucose release duodenal gastrin. Glycine (pH7), physiologic saline (pH 7), and balloon distention failed to release duodenal gastrin. These findings suggest differences in release characteristics between canine antral and duodenal gastrin, and point out species differences between man and dog in respect to release of duodenal gastrin.

The surgical treatment of duodenal ulceration of the gastrointestinal tract is reserved for specific complications of the ulcer, namely perforation, hemorrhage, obstruction of the gastric outlet, intractability, and recurrence following previous surgery. Though all persons with an ulceration share at least one symptom of the ulcer diathesis, the treatment of a specific patient's problem demands individualization so that the best-suited operative procedure will be performed. If patients are appropriately selected for surgical procedures, the success ratio is maximized, and the frequency of unde

The results of the surgical correction of congenital duodenal anomalies in infants free of severe prematurity, mongolism or other intestinal anomalies are most favorable. Several uncommon defects involving the second part of the duodenum, which required special attention to the anatomic proximity of the ampulla of Vater and the terminal portion of the common bile ducts have been encountered. The recognition of this intimate relationship may prevent inadvertent injuries to these structures, further improving the results of operative procedures for these difficult congenital malformations.
