Delayed effects of noise on the ear.

The possibility that a delayed onset hearing loss may occur after noise exposure was investigated in 20 normotensive (N) and 20 spontaneously hypertensive (SH) Wistar rats. Half of the rats of each strain were first exposed to noise at 3 months of age and half were first exposed at 12 months of age. All rats were exposed for a total of 3 months to a noise of 100 dB Leq (lin) (a narrowband noise, sweeping from 3 to 30 kHz at a rate of 0.5 Hz). Hearing thresholds were determined with a behavioral technique, and also on certain selected animals by brainstem audiometry, using pulses of 1/3-octave …

Polytomography and congenital anomalies of the ear.

Conventional radiography and even high resolution computerized axial tomography leave much to be desired in demonstrating the minutiae of the anatomy of the temporal bone. Multidirectional tomography remains the examination of choice radiographically in cases of congenital anomalies of the temporal bone. Details of the inner, middle and even external ear in the presence of atresia are more clearly delineated by this method than any other to data. Such information is of inestimable value to the surgeon in the diagnosis and surgical correction of anomalies of the temporal bone. Important feature…

Experience with tympanoplasty for atelectatic ears.

Atelectasis is, unfortunately, still a common sequela of otitis media with effusion (OME)--including serous otitis media (SOM) and mucoid otitis media (MOM). When atelectatic ears provide a functional deficit, i.e., a conductive loss of sufficient magnitude or a residual conductive loss after aerating the middle ear with ventilation tube or early or strongly suspected attic cholesteatoma, a simple corrective procedure has been used which has stood the test of time. The procedure as well as experience with 51 patients (60 ears) will be described and discussed. The procedure consists of re-estab…

Fragile bones and fragile ears.

Conductive, sensorineural and mixed hearing loss occur in osteogenesis imperfecta in autosomal dominant inheritance pattern. Hearing loss is generally due to the middle and inner ear pathology of osteogenesis imperfecta and only occasionally to the coincidental association of otosclerosis and osteogenesis imperfecta. Two lesions cause the conductive hearing loss of osteogenesis imperfecta: (1) functional ossicular discontinuity due either to stapes superstructure fracture or fibrous replacement, or (2) thick, crumbly, lightly fixed stapes footplate. Cochlear hair cell loss, stria vascularis at…

Vascular anomalies of the middle ear.

The patient presenting with a red mass behind the eardrum and a pulsating tinnitus may well have a vascular tumor. One must be ever mindful, however, that the mass may represent a congential vascular anomaly. The most common one seen is the uncovered jugular bulb in which the vessel extends superiorly into the middle ear to or above the incudostapedial joint. More rare is the uncovered and posteriorly displaced carotid artery. The purpose of this paper is to review the literature on the subject of vascular anomalies of the middle ear and temporal bone and to discuss the diagnosis and managemen…

Electrocochleography of ears with mumps deafness.

Electrocochleography was performed on 16 affected ears of 15 deaf patients whose deafness had resulted from the mumps. Although pure-tone audiometry showed no response in each case, the cases of mumps deafness can be classified into the following three types of cochlear impairment according to the cochlear microphonic (CM) response: (1) no action potential (AP) response but a well-developed CM response, with impairment at the neural level and probable functioning of Corti's organ; (2) absence of both the AP CM responses, with severe impairment of both the neural regions and Corti's organ; (3) …

Treatment of carcinoma of the middle ear.

31 patients with carcinoma of the middle ear were observed over a period of 15 years. Polytomography was used to determine the local extent of the disease. Of 22 patients given radical therapy, 15 were treated by mastoidectomy followed by radiation therapy. The 5-year survival rate was 40%, which is in agreement with the findings of several other authors. Patients treated by irradiation alone had a poor survival rate.

Admittance tympanometry in otosclerotic ears.

Twenty-eight surgically confirmed otosclerotic ears were evaluated to determine whether tympanometric admittance measurements could differentiate otosclerosis from a normal population. Subjects were tympanometrically measured at test conditions B220, B660, G220 and G660 Hz. When comparing mean curve peak amplitude to normative standards, 20 ears showed significantly low admittance tympanograms. Further investigation revealed no significant differences in curve width or pressure of the curve peak. Of the remaining eight otosclerotic ears, all produced diphasic "W"-notched tympanometric configur…

Diving injuries to the inner ear.

Most of the previous literature concerning otologic problems in compressed gas environments has emphasized middle ear barotrauma. With recent increases in commercial, military, and sport diving to deeper depths, inner ear disturbances during these exposures have been noted more frequently. Studies of inner ear physiology and pathology during diving indicate that the causes and treatment of these problems differ depending upon the phase and type of diving. Humans exposed to simulated depths of up to 305 meters without barotrauma or decompression sickness develop transient, conductive hearing lo…
