Implementation enhanced recovery after surgery (ERAS) pathway in patients undergoing surgical treatment for spinal metastases: A multicenter, retrospective, propensity-score matched, cohort study.

PURPOSE: Enhanced recovery after surgery (ERAS) pathway is a multidisciplinary program that aims to improve patient outcomes after various types of surgery. Here, we investigated the effects of the implementation of ERAS pathways on patients with spinal metastases (SM). METHODS: This was a multicenter, retrospective propensity-score-matched cohort study. We recorded the intraoperative variables, postoperative variables, and complications. RESULTS: After we performed propensity score matching, we included 60 well-balanced pairs of patients in the study. With respect to intraoperative variable

Mobilization practices in critically ill children: a European point prevalence study (EU PARK-PICU).

BACKGROUND: Early mobilization of adults receiving intensive care improves health outcomes, yet little is known about mobilization practices in paediatric intensive care units (PICUs). We aimed to determine the prevalence of and factors associated with physical rehabilitation in PICUs across Europe. METHODS: A 2-day, cross-sectional, multicentre point prevalence study was conducted in May and November 2018. The primary outcome was the prevalence of physical therapy (PT)- or occupational therapy (OT)-provided mobility. Clinical data and data on patient mobility, potential mobility safety event

Postoperative Physical Therapy to Prevent Hospital-acquired Pneumonia in Patients Over 80 Years Undergoing Hip Fracture Surgery-A Quasi-experimental Study.

BACKGROUND: Hip fracture requiring surgical fixation is a common condition with high mortality and morbidity in the geriatric population. The patients are usually frail, and vulnerable to postoperative complications and delayed recovery. Few studies have investigated physical therapy methods to prevent hospital-acquired pneumonia (HAP) after hip fracture surgery. OBJECTIVE: To explore whether an intensified physical therapy regimen can prevent HAP and reduce hospital length of stay in patients aged 80 and older, following hip fracture surgery. PATIENTS AND METHODS: The inclusion criterion wa

Choice of three different intramedullary nails in the treatment of trochanteric fractures: Outcome, analysis and consideration in midterm.

The purpose of this study is to compare the results obtained using three different systems of osteosynthesis, developed for the surgical treatment of fractures of the trochanteric region of the femur, based on the principle intramedullary nailing: the Gamma nail, the Affixus nail and the ZNN nail. This is a retrospective study: 72 trochanteric fractures treated with the Gamma nail, 68 treated with the Affixus nail and 69 treated with the ZNN nail, between the years 2012 and 2014, with the prerequisite of a minimum follow-up of 18 months. The fractures were classified according to the AO system

Indwelling bladder catheterisation as part of intraoperative and postoperative care for caesarean section.

BACKGROUND: Caesarean section (CS) is the most common obstetric surgical procedure, with more than one-third of pregnant women having lower-segment CS. Bladder evacuation is carried out as a preoperative procedure prior to CS. Emerging evidence suggests that omitting the use of urinary catheters during and after CS could reduce the associated increased risk of urinary tract infections (UTIs), catheter-associated pain/discomfort to the woman, and could lead to earlier ambulation and a shorter stay in hospital. OBJECTIVES: To assess the effectiveness and safety of indwelling bladder catheterisa

Traumatic patella tendon rupture: early mobilisation following surgical repair.

This study aims to demonstrate that postoperative immobilisation in a cast is unnecessary following suture repair of the patella tendon and retinacula protected by a cerlage wire. We present 11 patients mean age 42 years evaluated at a mean of 26 months (range 14-38 months). The mean Lysholm knee score at review was 97 (range 92-100). The mean Insall Salvati measurement (length patella/length tendon (LP/LT)) was 1 (range 0.95-1.1). No patients had fixed flexion deformities or demonstrated an extensor lag and the average range of motion was 0-137 degrees. The difference between the operated and

Management of displaced intraarticular calcaneal fractures by using external ring fixation, minimally invasive open reduction, and early weightbearing.

Twenty-three patients with 25 intraarticular fractures of the calcaneus were treated during a 7-year period with minimally invasive open reduction of the posterior facet, external ring fixation, and early weightbearing. Skeletal traction and a minimally invasive lateral approach were used to elevate the posterior facet. Percutaneous wires, which were secured to an external ring fixator, were used to stabilize the reduction. The patients were encouraged to bear weight on the first postoperative day and during the time the fixator was in place. A retrospective chart review was performed and the

[Palmar plating with the locking compression plate for dorsally displaced fractures of the distal radius--first clinical experiences].

Injuries and irritation of extensor tendons are common problems in the treatment of fractures of the distal radius when plating is used via a dorsal approach. By the development of locking compression plates the possibility of palmar plating for dorsally displaced fractures of the distal radius is available. In this study our first clinical experiences using the 3.5 mm radius locking compression plate (LCP) are reported. Between February 2002 and September 2002 24 patients with dorsally displaced fractures of the distal radius were treated using a palmar approach with the LCP and included in a

External fixator in the management of trochanteric fractures in high risk geriatric patients--a friend to the elderly.

The best treatment option for trochanteric fracture in a geriatric high risk patient with all associated medical and surgical problems remains debatable. Conservative methods of treatment are associated with dangerous complications of prolonged recumbency while open reduction and internal fixation under anaesthesia significantly increases the mortality and morbidity rates. We treated 110 elderly patients who were unfit or high risk cases for anaesthesia and major surgery for internal fixation due to associated medical and surgical conditions, by external fixation under local anaesthesia. The a

Postural control after anterior cruciate ligament reconstruction and functional rehabilitation.

Total sagittal knee laxity and postural control in the sagittal and frontal planes were measured in 25 patients at a mean of 36 months (range, 27 to 44) after anterior cruciate ligament reconstruction and in a control group consisting of 20 uninjured age- and activity-matched subjects. Body sway was measured in the sagittal plane on a stable and on a sway-referenced force plate in single-legged stance, double-legged stance, or both, with the eyes open and closed. Postural reactions to perturbations in the sagittal and frontal planes were recorded in the single-legged stance with the eyes open.

An arthroscopic technique for the reduction and fixation of tibial-eminence fractures.

A technique of arthroscopic reduction and fixation of type III tibial-eminence fractures is described. After reduction of the fracture, polyglactin (Vicryl) suture was passed into the knee with the use of the arthroscope to fix the fracture. Early mobilization of the patients was begun after 2 weeks. Two patients (aged 10 and 13 years old), with 2 years follow-up after treatment of their fracture with this technique, had full range of motion of the knee with no sign of instability. Arthroscopic reduction and fixation of type III tibial-eminence fractures is a technique that can reduce hospital

Fracture-dislocation of the dorsal-lumbar spine. Acute operative stabilization by Harrington instrumentation.

Twenty-four patients (of whom 17 were paraplegic) with acute fracture dislocations of the dorsal-lumbar spine underwent stabilization with Harrington instrumentation and were followed for an average of 16 months. Immediate postoperative ambulation was achieved in 16 patients. There was little difference between compression and distraction with respect to reduction and maintenance of correction. Compression rods resulted in solid union in 15 of 16 patients, while distraction rods were successful in 6 of 8 instances. In contrast to reported series in which operative intervention has been avoided

Risk factors for early reoperation in patients after posterior lumbar interbody fusion surgery. A propensity-matched cohort analysis.

BACKGROUND: Reoperation is usually associated with poor results and increased morbidity and hospital costs. However, the rates, causes, and risk factors for reoperation in patients undergoing lumbar spinal fusion surgery remain controversial. This study aimed to identify the risk factors for early reoperation after posterior lumbar interbody fusion surgery and to compare the clinical outcomes between patients who underwent reoperation and those who did not. METHODS: We investigated a multicenter medical record database of 1263 patients who underwent posterior lumbar interbody fusion surgery b

KMT2B-related disorders in Austria: clinical features and long-term outcome after deep brain stimulation.

INTRODUCTION: Since its initial description in 2016, DYT-KMT2B has emerged as one of the most common genetic causes of early-onset dystonia. Subsequent reports have expanded its phenotypic spectrum, frequently including neurodevelopmental features. Deep brain stimulation of the globus pallidus internus (GPi DBS) has become a therapeutic mainstay; however, most published data are based on single cases or short-term observations, and long-term outcomes remain poorly characterized. METHODS: We report the clinical course and response to GPi DBS in nine patients with KMT2B variants prospectively f
