The economic implications of cyclooxygenase-2-specific inhibitors.

Rheumatoid arthritis and osteoarthritis are prevalent and costly conditions. A large proportion of the direct costs associated with these conditions relates to management of iatrogenic side effects. The cyclooxygenase (COX)-2-specific inhibitors lead to equivalent control of pain and disability compared with traditional NSAIDs. However, the COX-2-specific inhibitors have significant potential to reduce health-care costs, principally through the reduction of side effects. These cost savings are most likely to be realized through reductions in costs associated with dyspepsia and upper gastrointe

Social economic development in the prevention of global blindness.

AIMS: To assess the relation between a country's economic developmental status and its prevalence of blindness. METHODS: Available epidemiological data on worldwide visual loss and its causes compiled by the World Health Organization were reviewed. Findings were compared with economic development data from the involved countries and regions. Analysis was completed in view of the socioeconomic status of each country and region. RESULTS: Analysis of the global distribution of blindness indicates a trend of higher prevalence existing in developing countries with lower per capita income. Prevent

A model for economic comparison of swine insemination programs.

Optimal artificial insemination schedules are those that result in a high farrowing rate and litter size, while minimizing costs of semen and labor by avoiding unnecessary inseminations. A simulation model programmed in a commercial spreadsheet was developed to permit comparison of alternative schedules. Farrowing rate and litter size for a particular schedule were dependent on the timing of insemination relative to the time of ovulation. Economic return was calculated by multiplying the number of pigs born per bred sow by $33.00 and subtracting the cost of producing a litter of pigs and raisi

Geriatric intertrochanteric hip fractures: an economic analysis.

Hip fractures have a high economic and social cost to society. Rising healthcare costs, coupled with managed healthcare systems, have forced a close inspection of healthcare expenditures. To evaluate the impact of geriatric intertrochanteric hip fractures upon a hospital system, an economic cost-analysis was undertaken. An analysis was made of financial and hospital data of elderly patients who sustained an intertrochanteric hip fracture and subsequently underwent open reduction and internal fixation. Increasing patient age did not correlate with length of stay, overall hospital cost, or net h

Prophylactic use of factor VIII: an economic evaluation.

BACKGROUND: Since the introduction of exogenous factor VIII therapy, several studies have explored the clinical benefits of prophylactic use of factor VIII. Little research, though, has focused on the economic aspects of this regimen. We conducted a cost analysis using data from the Orthopedic Outcomes Study, a prospective, cross-national study of the clinical outcomes associated with different patterns of factor VIII utilization to examine the health care costs incurred and expenditures averted in patients receiving on-demand versus prophylactic use of factor VIII in hemophilia. METHODS AND

Economic considerations in the treatment and management of Alzheimer's disease.

Cost issues and formal economic evaluations of treatment for Alzheimer's disease (AD) are reviewed. The annual cost of AD in the United States is estimated to be $100 billion. Each case of AD is associated with total costs of up to $195,000, with a substantial portion of this attributable to lost patient and caregiver productivity and to family out-of-pocket expenses. Only a few studies have evaluated the economic impact of therapeutic interventions in AD. In one study of tacrine, dosages greater than 80 mg/day significantly delayed nursing-home placement. In another study, treatment that impr

The economic burden of lymphatic filariasis in northern Ghana.

The socio-economic impact of lymphatic filariasis was investigated in a rural community in northern Ghana, as part of a multi-country study. The entire community was followed up for a period of one full calendar year, to document episodes of acute adenolymphangitis (ADL), treatment seeking and the cost of treatment. Cases of chronic filarial disease identified at the start of the study were also visited every 3 months to monitor treatment seeking and the costs of treatment. The subjects, who were mainly subsistence farmers, were found to be materially very poor. The incidence of ADL, which las

Tumour serum markers: clinical and economical aspects.

Tumour serum markers represent one of the most interesting challenges in modern oncology. Although know for many years, tumour markers did not receive clinical attention until the '80s. Despite their widespread use, the identification of a tumour marker which is highly sensitive as well as specific for a certain type of cancer, and can be assayed by simple, reproducible and cheap methods, remains elusive. This review deals with the clinical use of the Prostate Specific Antigen (PSA). PSA is biochemically a glycoprotein, is the most valuable tool available for the diagnosis and staging of prost

Economic losses caused by foodborne parasitic diseases.

Fragmentary data indicate that zoonotic parasites cause human illnesses with medical costs and productivity and disability losses totalling billions of dollars annually. Food is an important vehicle for some of these parasitic diseases. The cost to public health is not reflected in the priorities given to these parasitic diseases in either research or public health planning. In this article, Tanya Roberts, Darwin Murrell and Suzanne Marks discuss the cost of toxoplasmosis, taeniasis, cysticercosis, trichinellosis and other foodborne parasitic diseases.

Economic impact of injuries associated with electrical events.

The employees injured as a result of an electrical event are younger and less experienced than a peer control group selected at random. The average cost of injuries associated with electrical events to industry is $49,823 plus the costs of unaccounted for expense factors.

Economic and demographic research on malaria: a review of the evidence.

The primary objective of malaria control is to reduce deaths and illness from the disease. Elimination of infection is far more difficult. However, in most developing countries where malaria is endemic, there are few reliable data on levels, differences and trends in malaria mortality, and its relative importance as a cause of death or illness in endemic communities. The large-scale efforts and resources being devoted to delivery of essential care to mothers and young children in most countries make it important to have a better understanding of the extent of cause-specific child mortality lev

Economic analysis of vaccination applied to ovine listeriosis.

An economic model for determining whether vaccination against a disease would be beneficial financially on individual farms is proposed; it is based on four pieces of information: the costs of a disease case including its treatment, the cost of vaccinating each animal including veterinary fees, the expected incidence of the disease, and the efficacy of the vaccine. The model was applied to ovine listeriosis, which is a serious disease problem in Norway. Vaccination appeared to be beneficial for the average sheep flock of 100 ewes which might expect two or more cases of listeriosis per year. Fu

Focus on oral ciprofloxacin; clinical and economic considerations.

Ciprofloxacin, a recently released oral fluorinated quinolone structurally related to nalidixic acid, joins norfloxacin as the second drug of this class to be released. Ciprofloxacin has a wide spectrum of antimicrobial activity and importantly demonstrates little cross resistance to non-quinolone drug classes (e.g. ureidopenicillins, cephalosporins, monobactams, carbapenems, aminoglycosides). Unlike other antibacterial classes such as the beta-lactams or aminoglycosides, ciprofloxacin does not suffer from transferable plasmid-mediated (i.e. R-factor) antibiotic resistance. Against gram-positi

[Health consequences of the economic development of tropical countries].

If the usefulness of industrial development of the Third World is not really discussed, nevertheless it appears more and more necessary to evaluate carefully its advantages and disadvantages on the health status and the well being of the population concerned. Indeed, the evil consequences are well known: new outbreak of bilharziosis, malaria, onchocerciasis, but also arbovirosis, tuberculosis and S.T.D. On the other hand, we must consider the positive aspects of economic development: improvement of hygiene, constructions of health centers, increased food production. In order to minimize the dr

Therapeutic and economic value of a normal coronary angiogram.

A significant number of patients with severe angina or intractable atypical chest pain referred for coronary arteriography are found to have normal coronary vessels. To determine what therapeutic or economic benefit may be derived from these studies, we analyzed the data of 72 consecutive patients with normal vessels referred for cardiac catheterization because of severe chest pain. The clinical status and hospitalizations were analyzed for the 2 year period before and the 2 year period after angiography. There were no deaths or myocardial infarctions. Although 47 were thought to have angina a
