[The value of alkalizing treatments in severe diabetic keto-acidosis].

In 37 diabetic patients with severe acidosis (pH less than 7.0; [HCO3-] less than 5.0 mMol/l), administration of insulin was preceded by a rapid infusion of molar sodium bicarbonate in order to obtain partial correction of acidosis (pH approximately 7.20). 31 patients survived (83,8%); 6 patients died in cardio-circulatory failure associated in two cases with acute pulmonary edema. Initial administration of bicarbonate appears to be beneficial in preventing the deleterious effects of prolonged severe acidosis, such as cardiac arrhythmias, shock or acute pulmonary edema. Furthermore, partial co

Characterizing a Left Ventricular Pseudoaneurysm With Photon-Counting Detector CT.

BACKGROUND: Ventricular pseudoaneurysms are a rare but potentially fatal complication of myocardial infarction. Accurate differentiation from true aneurysms is crucial given the risk of rupture. CASE SUMMARY: A 57-year-old woman presented with acute chest pain and progressive dyspnea. Electrocardiography showed signs of chronic infarction, while echocardiography revealed an aneurysmal structure and pericardial effusion. Photon-counting detector computed tomography (PCD-CT) was able to identify a large inferoseptal pseudoaneurysm with mural thrombus, occlusion of the right coronary artery, myo

Cardiac lymphatics retain LYVE-1-dependent macrophages during neonatal mouse heart regeneration.

In adult mice, myocardial infarction (MI) activates the cardiac lymphatics, which undergo sprouting angiogenesis (lymphangiogenesis), drain interstitial fluid and traffic macrophages to mediastinal lymph nodes (MLNs). This prevents edema and reduces inflammatory/fibrotic immune cell content to improve cardiac function. Here we investigated the role of cardiac lymphatics and macrophage clearance across the neonatal mouse regenerative window. The response to injury revealed limited lymphangiogenesis and clearance of macrophages from postnatal day 1 compared to postnatal day 7 infarcted hearts. T

Cardiomegaly and Low Voltage Suggest Cardiac Involvement in a Patient With Hypereosinophilic Syndrome: A Case Report.

Hypereosinophilic syndrome is a heterogeneous group of disorders, the majority of which are idiopathic. Cardiac manifestations, particularly eosinophilic myocarditis and endomyocardial fibrosis, are a typical course of morbidity and mortality in hypereosinophilic syndrome. We present a case of a patient with asthma and idiopathic eosinophilia who presented with dyspnea and edema. Cardiac ultrasonography showed pericardial effusion and reduced left ventricular motion, which persisted despite heart failure therapy, although pulmonary congestion improved. The peripheral blood eosinophil count was

Cardiovascular dysfunction in patients with liver cirrhosis.

Hyperdynamic syndrome is a well-known clinical condition found in patients with cirrhosis and portal hypertension, characterized by increased heart rate and cardiac output, and reduced systemic vascular resistance and arterial blood pressure. The leading cause of hyperdynamic circulation in cirrhotic patients is peripheral and splanchnic vasodilatation, due to an increased production/activity of vasodilator factors and decreased vascular reactivity to vasoconstrictors. The term "cirrhotic cardiomyopathy" describes impaired contractile responsiveness to stress, diastolic dysfunction and electro

Effects of a Three-Day vs. Six-Day Exposure to Normobaric Hypoxia on the Cardiopulmonary Function of Rats.

In rats, normobaric hypoxia significantly reduced left ventricular (LV) inotropic function while right ventricular (RV) function was not impaired. In parallel, the animals developed pulmonary edema and inflammation. In the present study, we investigated whether cardiac function and pulmonary injury would aggravate after three and six days of hypoxia exposure or whether cardiopulmonary reactions to prolonged hypoxia would become weaker due to hypoxic acclimatization. Sixty-four female rats were exposed for 72 or 144 h to normoxia. They received a low-rate infusion (0.1 mL/h) with 0.9% NaCl solu

Myocardial edema underlies dynamic T-wave inversion (Wellens' ECG pattern) in patients with reversible left ventricular dysfunction.

BACKGROUND: The Wellens' electrocardiogram (ECG) pattern of dynamic T-wave inversion in the anterior leads is observed in clinical conditions characterized by reversible left ventricular (LV) dysfunction (stunned myocardium), either ischemic or nonischemic. The pathophysiologic basis of this ECG pattern remains to be elucidated. OBJECTIVE: The purpose of this study was to report the contrast-enhanced cardiac magnetic resonance (CE-CMR) findings in 4 cases of Wellens' ECG pattern associated with transient LV dysfunction from a variety of clinical conditions such as myocardial bridge, coronary

Centrally injected tityustoxin produces the systemic manifestations observed in severe scorpion poisoning.

In this work we submitted adult male Wistar rats to intracerebroventricular (icv) and iv microinjections of the fraction tityostoxin (TsTX) from the Tityus serrulatus scorpion venom, to address whether the CNS could account for the systemic alterations previously reported: cardiac arrhythmias, lung edema, and seizures. Animals were injected icv, total volume of 1.0 microl, with either sterile saline (n = 4) or differing doses of TsTX (1.74, n = 5; 0.174, n = 4; 0.087, n = 5; and 0.058 microg, n = 4). The peripheral effect of the highest dose of TsTX used (1.74 microg) was tested through iv inj

Comparative efficacy and safety of bumetanide and furosemide in long-term treatment of edema due to congestive heart failure.

Bumetanide and furosemide were compared for efficacy in reducing edema due to congestive heart failure in 28 patients (21 receiving bumetanide and seven receiving furosemide) in a long-term study for periods from one week to 18 months. In both groups the patients showed decreases in body weight, abdominal girth, edema, hepatomegaly, blood pressure, and heart rate. Commonly observed decreases frequently achieved statistical significance, more often with bumetanide, but the differences between treatments were rarely statistically significant. Both drugs were generally well tolerated. A breast no

The Heart-Brain Axis: Key Concepts in Neurocardiology.

The heart-brain axis involves complex interactions between the cardiovascular and nervous systems via a network of cortical and subcortical structures working with the autonomic nervous system and intracardiac nervous system. Heart-brain interactions may be divided into 2 broad categories: cardiac effects of neurological disease and neurological effects of cardiac disease. The pathogenesis of neurogenic cardiac effects is thought to involve a neurogenic cascade where sudden shifts in autonomic balance lead to an exaggerated catecholamine release. This can occur in acute neurological conditions

Role of Cardiac Imaging in Myocardial Infarction With Non-Obstructive Coronary Arteries.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) represents a dynamic and evolving clinical entity, defined by two key components: the presence of acute myocardial infarction and the absence of obstructive coronary artery disease. Contrary to earlier perceptions of MINOCA as a benign condition, accumulating evidence underscores its association with significant morbidity and major adverse cardiac events. Recent clinical guidelines advocate for a structured, three-step "traffic light" approach to differentiate non-cardiac causes of elevated cardiac troponins from ischemic an

Left ventricular inflow tract obstruction secondary to a myxoma in a dog.

This is the first description of a left ventricular inflow tract obstruction secondary to a myxoma in a dog. A 4 yr old, male fox terrier presented with a 1 mo history of cough and exercise intolerance. Expiratory dyspnea, pulmonary crackles, irregular cardiac rhythm, and a grade 4/6 pansystolic cardiac murmur over the left cardiac apex were the most important features on physical examination. The electrocardiogram revealed atrial fibrillation. Thoracic radiographs showed left-sided cardiac enlargement and mild pulmonary edema, especially in the hilar area. Two-dimensional transthoracic echoca

Role of Cardiac Magnetic Resonance in Inflammatory and Infiltrative Cardiomyopathies: A Narrative Review.

Cardiac magnetic resonance (CMR) has acquired a pivotal role in modern cardiology. It represents the gold standard for biventricular volume and systolic function assessment. Moreover, CMR allows for non-invasive myocardial tissue evaluation, highlighting tissue edema, fibrosis, fibro-fatty infiltration and iron overload. This manuscript aims to review the impact of CMR in the main inflammatory and infiltrative cardiomyopathies, providing details on specific imaging patterns and insights regarding the most relevant trials in the setting.

Cardiopulmonary bypass before general anesthesia in prosthetic valve thrombosis.

Valve obstruction is a lifethreatening complication of mechanical valve prostheses. Emergency operation is mandatory for patients who subsequently develop cardiogenic shock and severe pulmonary edema. In this severely compromised hemodynamic condition, cardiac arrest develops in most of the patients before surgery and just after general anesthesia induction. In one such case, we performed femorofemoral cardiopulmonary bypass with local anesthesia before general anesthesia induction and successfully replaced the thrombosed prosthetic valve, thus avoiding a catastrophic outcome.

Loratadine disrupts cardiovascular and swim bladder development in zebrafish.

The widespread presence of antihistamines such as loratadine in aquatic environments poses emerging ecotoxicological concerns, yet their developmental impacts remain poorly characterized. In this study, zebrafish embryos were exposed to loratadine (35-350 μg/L) to evaluate its developmental effects. By 2-3 days post-fertilization (dpf), exposed larvae exhibited marked cardiac defects, including pericardial edema, reduced heart rate and cardiac output, and fractional area change (p < 0.05). By 4-6 dpf, a complete failure of swim bladder inflation was observed in the exposed group, confirmed by
