Sports injuries of the elbow joint can be acute or can arise due to chronic overuse. Such overstress syndromes are common in overhead, throwing and racket sports activities. The present article introduces diagnostic algorithms developed from knowledge gained through motion analysis, clinical examination and further imaging. The diagnosis of the overtaxing syndromes mainly depends on these findings. Basically most of the disorders in the elbow joint can be treated conservatively. If this is not the case or if the conservative treatment does not succeed, arthroscopic techniques, ligament reconst…

Arthritis of the elbow resulting from either prior trauma or primary osteoarthritis is similar in that the end result is often a combination of pain and stiffness, but the location of the disorder can be different. Treatment decisions must be made on a case-by-case basis taking into account patient age, level of demand, and location and degree of degenerative changes. This article focuses primarily on the unique pathogenesis and general treatment rationale for primary osteoarthritis and posttraumatic arthritis of the elbow.

There are many bone and soft tissue injuries to the elbow, wrist, and hand that are treated surgically. The operative techniques can be complicated and their indications may change. This article reviews the common injuries at the elbow, wrist, and hand, the indications for surgical management, the current and previous operative techniques used, the expected postoperative appearance on different imaging modalities, and the important potential complications of each technique.

An 11-month-old child presented with what initially appeared to be 2 simple problems, a small elbow effusion followed 1 week later by croup. This case history is presented to remind emergency medicine physicians of the importance of index of suspicion and recognition of the difference between common events occurring in a normal host versus common events occurring in an abnormal host.

Injuries to the shoulder and elbow are common in athletes involved in sporting activities that require overhead motion of the arm. An understanding of the forces involved in the throwing motion, the anatomic structures most at risk, and the magnetic resonance imaging appearances of the most common associated injuries can help to improve diagnostic accuracy when interpreting imaging studies in these patients.

Supra- and intercondylar elbow fractures are rare in children. The treatment of these fractures is controversial. The purpose of this report is to discuss diagnosis and treatment of this unusual injury. Our study looked at nine patients between six and 15 years old (average age: 11.5). In four patients, the fracture was T-condylar and in five patients, the fracture was epiphyseal-diaphyseal. Communition was noted in five cases. All fractures are secondary to high-energy trauma. All patients were treated by open reduction and internal fixation. Three patients had associated nervous lesions. Pat…

OBJECTIVE: To retrospectively reviewed the operative therapy of the terrible triad of the elbow.
METHODS: From October 2003 to September 2007, 10 cases of terrible triad were treated, with an elbow dislocation and an associated fracture of both the radial head and the coronoid process. There were 3 males and 7 females with the age of 18-66 years. The injury was caused by traffic accidents in 4 cases, falling from a height in 4 cases, and tumbling in 2 cases. The coronoid process fractures of the patients were 5 cases of type I, 3 cases of type II and 2 cases of type III according to Regan-Mor…

The excellent and good rate was 88.9%. Three patients had light pain of elbow, 1 patient had moderate pain and the other 14 had no pain. The range of elbow motion was from 110 to 140 degrees (mean 130 degrees). The pronation averaged 75 degrees (35-85 degrees). The supination averaged 80 degrees (65-89 degrees). Compared with …

An anatomical study of the brachial portion of the radial nerve with surgical implications is proposed. Thirty specimens of arm from 20 fresh cadavers (11 male, 9 female) were used to examine the topographical relations of the radial nerve with reference to the following anatomical landmarks: acromion angle, medial and lateral epicondyles, point of division between the lateral and long heads of the triceps brachii, lateral intermuscular septum, site of division of the radial nerve into its superficial and posterior interosseous branches and entry and exit point of the posterior interosseous br

The shoulder and elbow represent two of the most commonly injured joints in the adolescent population. Specific injuries vary by sport and can involve various structures, depending on the mechanism of injury. Unlike the adult shoulder, the immature skeletal structure of the adolescent athlete can lead to several unique injuries. By understanding the special demands placed on the immature shoulder, the sports physician can more effectively treat the resultant injury. This article reviews the diagnosis and management of unique injuries to the shoulder and elbow in the adolescent athlete.

Despite the growing use of hinged external fixators of the elbow, there are no studies regarding the complications associated with their application. The purpose of this study is to report our experience with complications with this procedure. Between 1998 and 2005, we reviewed the records of 100 consecutive patients who were treated with hinged external fixators (including 433 pin sites). Complications specifically related to pin placement were recorded. There were 15 patients with minor complications (15%) involving 21 pins (4.8%) and 10 patients with major complications (10%) involving 29 p…

Between 1987 and 2005, 165 patients with rheumatoid arthritis of the elbow, four with psoriatic arthritis, and eight with osteoarthrosis of the elbow were treated with total elbow arthroplasty using 126 semiconstrained GSB III elbow replacement prostheses; 46 semiconstrained Coonrad-Morrey prostheses; 24 Souter-Strathclyde unconstrained, unlinked prosthesis; four constrained link hinge prostheses; two custom-made prostheses; and one Pritchard elbow replacement. All implant procedures relieved pain and improved functionality. The complication rate was 34.4%. Revision surgery was needed in 27.2%…

This report presents my guidelines for surgeon training and development of skills in the art of elbow arthroscopy. As elbow arthroscopy becomes more common, it is important to achieve greater levels of experience that will minimize the risk of complications because indications and techniques are growing in number and complexity. Indications for elbow arthroscopy that require diagnosis before open surgery include evaluation of instability; removal of loose bodies, spurs, and plica; irrigation and debridement of infection or open joints; evaluation for arthrofibrosis (with open nerve protection)…

Septic arthritis of the shoulder, elbow, or wrist is not commonly described in the literature. We evaluated the micro- biology of these infections and the diagnostic role of joint aspirate findings. This retrospective study includes 52 patients, diagnosed from 1994 to 2004, with septic arthritis of the shoulder (n = 17), elbow (n = 23), or wrist (n = 12). The mean age was 44.3 years, and 54% of patients (28/52) had comorbidities. The prevalence of Staphylococcus aureus was 76%. Ninety-six percent of aspirates with positive cultures had a polymorphonuclear differential count greater than 85% of…

MRI is a useful diagnostic method for evaluating nerve disease at the shoulder and elbow. MRI can depict the normal anatomy of the nerves, confirm and identify the cause of the neuropathy, identify the site of entrapment based on muscle denervation patterns, and detect unsuspected space-occupying lesions. MRI can also narrow down the differential diagnosis of nerve disease, such as in the case of suprascapular nerve syndrome versus Parsonage-Turner syndrome, or radial tunnel syndrome versus lateral epicondylitis. Large prospective studies with surgical correlation, however, are still necessary…
