Electrical stimulation of the trigeminal tract in chronic, intractable facial neuralgia.

In this paper the treatment of patients with chronic, intractable trigeminal neuralgia by invasive electrical stimulation of the Gasserion ganglion is reviewed. Two different surgical techniques are employed in this treatment. Most frequently, a method similar to the traditional technique for percutaneous glycerol and radiofrequency trigeminal rhizolysis is used: a small percutaneous stimulation electrode is advanced under fluoroscopic control through a thin needle via the foramen ovale to the Gasserian cistern. Some neurosurgeons use an open surgical technique by which the Gasserian ganglion

Therapeutic electrical stimulation of the hypoglossal nerve in obstructive sleep apnea.

BACKGROUND: Hypoglossal nerve stimulation has been demonstrated to relieve upper airway obstruction acutely, but its effect on obstructive sleep apnea is not known. OBJECTIVE: To determine the response in obstructive sleep apnea to electrical stimulation of the hypoglossal nerve. METHODS: Eight patients with obstructive sleep apnea were implanted with a device that stimulated the hypoglossal nerve unilaterally during inspiration. Sleep and breathing patterns were examined at baseline before implantation and after implantation at 1, 3, and 6 months and last follow-up. RESULTS: Unilateral hyp

Functional electrical stimulation on chronic and acute hemiplegic shoulder subluxation.

OBJECTIVE: The present study investigated and assessed the effectiveness of a functional electrical stimulation (FES) program in the management of acute and chronic shoulder subluxation. DESIGN: By their postonset duration, hemiplegic subjects with subluxation participating in the study were placed into a short-duration group and a long-duration group. Subjects in each group were further assigned randomly to either a control subgroup or an experimental subgroup. The experimental subgroups of both short and long duration received FES therapy in which supraspinatus and posterior deltoid were in

Identification of the target neuronal elements in electrical deep brain stimulation.

The aim of this study is to identify the primary neuronal target elements in electrical deep-brain stimulation, taking advantage of the difference in strength-duration time constant (tau(sd)) of large myelinated axons ( approximately 30-200 micros), small axons ( approximately 200-700 micros) and cell bodies and dendrites ( approximately 1-10 ms). Strength-duration data were measured in patients suffering from Parkinson's disease or essential tremor and treated by high-frequency stimulation in the ventral intermediate thalamic nucleus or the internal pallidum. Threshold voltages for the elimin

Central electrical stimulation of the auditory pathway in neurofibromatosis type 2.

The auditory brainstem implant (ABI) is a viable treatment option for patients with neurofibromatosis type 2 (NF2) whom either vestibular schwannomas or the surgery used to remove them has rendered totally deaf. This device stimulates the central auditory pathways in a manner similar to the cochlear implant in individuals with a total hearing loss. A multichannel model with transcutaneous signal transmission is in use experimentally in both the United States and Europe. Of 14 patients implanted with the ABI in a European pilot study, 13 received auditory sensations at initial tuning. Surgical

An experimental pain model based on electric stimulations of the colon mucosa.

BACKGROUND: Pain intensity and distribution related to diseases of the gut are important diagnostic indicators in gastroenterology. Experimental pain models provide a unique possibility for standardized activation of the nociceptive system, but only few human models exist. METHODS: An experimental pain model based on electric stimuli in the human colon was developed and applied. Eleven patients who were referred for surveillance colonoscopy due to earlier polyps in the colon were included. None had any abdominal pain complaints. The following areas were stimulated with 'single', 'repeated', o

Use of electrical stimulation in brain-injured patients: a case report.

After failure of other therapeutic measures, electrical stimulation was applied to promote gait rehabilitation in a patient with severe brain injury and complete left hemiplegia. The favourable results reported in the literature were confirmed. Despite the long interval between injury and institution of electrical stimulation, independent ambulation was quickly restored.

Threshold-distance measures from electrical stimulation of human brainstem.

Threshold current levels for electrical stimulation of a single human brainstem via an auditory prosthesis are compared with postmortem measures of the distance between the electrode and stimulated structures. The results compare well with the summary of threshold-distance measures from animal experiments compiled by Ranck. The correspondence between the human and animal data gives confidence that the extent of current spread (distance to stimulable neural units) can be well estimated from the current level at threshold for 200 microseconds/phase biphasic pulses. This is of particular interest

Influence of electrically stimulated ankle plantar flexors on the swinging leg.

The influence of functional electrically stimulated ankle plantar flexors on the swinging lower extremity was studied in incomplete spinal cord injured persons. Stimulation sequences with different time and frequency parameters were delivered to ankle plantar flexors and knee extensors and to the peroneal nerve. The results of kinematic assessment showed that stimulated calf muscles provide noticeable forward and upward propulsion to the swinging leg.

Electrical stimulation of sacral spinal nerves for treatment of faecal incontinence.

Functional deficits of the striated anal sphincteric muscles without any apparent gross defect often result in a lack of ability to postpone defaecation by intention or in faecal incontinence in response to increased intra-abdominal or intra-rectal pressure. We applied electrostimulation to the sacral spinal nerves to increase function of the striated muscles of the anal sphincter. Of three patients followed for 6 months, two gained full continence and one improved from gross incontinence to minor soiling. Closure pressure of the anal canal increased in all. Preliminary data indicate that anal

Dermatome mapping in the rat hindlimb by electrical stimulation of the spinal nerves.

In rats treated with Evans blue (i.v.), electrical stimulation of the lumbar spinal nerves produced an extravasation zone in the skin. Stimulation of L1 produced extravasation in the lower abdomen; that of L2, in the cranial region of the hindlimb; that of L3, in the ventral region of the hindlimb and the medial paw; that of L4, in the lateral region of the hindlimb and the middle paw; that of L5, in the caudal region of the hindlimb and the lateral paw; and that of L6, in the caudal region of the hindlimb and the scrotum. Since the substance P antagonist FK224 inhibited the extravasation caus

External electrical stimulation and bracing for treatment of spondylolysis. A case report.

Evidence indicates that osseous healing of acute spondylolysis can occur without surgery, although no existing data supports a particular regimen that optimizes healing. This article presents a case study of a 17-year-old athletic male student who presented with acute spondylolysis and who was treated with intermittent bracing and daily external electric stimulation. The patient was treated with a thoracic lumbar sacral orthosis to which an external bone growth stimulator was added. Computer tomography scans performed throughout the treatment process and described in this report illustrate the…

Tinnitus suppression by electrical promontory stimulation in sensorineural deaf patients.

In 62 patients, whose hearing acuity was normal or profoundly impaired, tinnitus suppression was attempted through electrical promontory stimulation (EPS) with a Promontory Stimulator (Cochlear Co.). The causes of tinnitus were idiopathic sudden deafness, ototoxicity, noise induced hearing loss, labyrinthitis, Meniere's disease, acoustic neuroma and unknown origin. Reduction of tinnitus was achieved in 46 of 68 ears (67.6%). Of these, EPS was most effective in cases of noise induced hearing loss (100.0%; 3/3), followed by those of idiopathic sudden deafness (87.5%; 14/16), Meniere's disease (8

Electrically stimulated gracilis sphincter for treatment of bladder sphincter incontinence.

Correction of total urinary incontinence due to sphincter damage is done with an artificial sphincter prosthesis or urinary diversion. In this pilot study we used graciloplasty around the bladder neck followed by electrical stimulation of this muscle with an implanted stimulator, which could be switched off and on by a magnet. Stimulus variables could be changed externally. With the stimulator on, urethral pressures of about 50 cm H2O were obtained. Of three patients who underwent the procedure, two became continent and one improved but remained incontinent. Dynamic graciloplasty can restore u

Masseteric facilitation induced by electrical stimulation of rat orofacial tissues.

The effects of a conditioning electrical shock applied to the periodontium of the lower incisor or the glabrous area of the lower lip on the jaw-closing reflex in the anesthetized, non-paralysed rat were studied. The masseteric reflex was triggered by stimulation of the mesencephalic nucleus as a test shock and was recorded from the masseter muscle. There was facilitation of the jaw-jerk reflex, which culminated at an interval of 10-15 ms between the conditioning and the test shocks. This facilitation was not suppressed by digastric excision or by blocking a possible rebound closing reflex evo

[Diaphragmatic pacing by electrical stimulation of the phrenic nerve--a case report].

A four-year-old boy had traffic accident and was transferred to our hospital with cardiorespiratory arrest, on March 24, 1986. After resuscitation, quadriplegia and apnea were sustained. Computed tomography (CT) scan on the day of admission only demonstrated subarachnoid hemorrhage and plain roentgenograms of the skull and cervical spine showed no abnormalities. He opened his eyes to verbal order one week later, but apnea and quadriplegia sustained. CT scan obtained on that day demonstrated no abnormalities but roentgenogram of the cervical spine showed atlanto-axial dislocation. On May 26, 19

Direct current electrical bone growth stimulation for spinal fusion.

An electronic bone growth stimulator (EBGS) had been devised to facilitate lumbosacral fusion in the early 1970s, using a totally implanted device which delivers a steady 5 microA at each of four cathodes. The use of the device is reserved for patients in whom painful and abnormal motion is demonstrated to be the major cause of low-back pain. An initial series (I-A) of 82 patients demonstrated a successful fusion rate of 91.5% compared with a 80.5% fusion rate in 159 patients treated without the EBGS. A second prospective randomized controlled trial series (II) was begun using only "difficult
