[Electroconvulsive therapy in a patient with Huntington's disease and depression: a case-report].

We present the case of a 60-year-old woman with Huntington’s disease who was successfully treated with electroconvulsive therapy (ECT) for a recurrent depressive disorder presenting with acute suicidality. Depression is the most common neuropsychiatric disorder in Huntington’s disease and ranks among the most important risk factors for suicide within this population. Depressive symptoms in individuals with Huntington’s disease are generally treated in accordance with established guidelines for major depressive disorder in the general population. However, the etiology and path…

Improvement in Quality of Seizures in Electroconvulsive Therapy With Zotepine: A Case Report.

Regular symmetric high-amplitude seizure waves, postictal suppression, constant seizure duration, and sympathetic nervous system excitation are used to evaluate the efficacy of electroconvulsive therapy (ECT) for seizures. Seizure quality is rated from 0 to 3 (higher scores indicate better quality) using seizure quality categories (SQC). We report a case in which zotepine (ZTP) improved seizure quality. A woman in her 50s developed schizophrenia in 1993. In December 2021, her delusions, anxiety, and agitation worsened, and she was admitted to our hospital. ECT was started in November 2022 but …

The Role of Electroconvulsive Therapy in Posttraumatic Stress Disorder: A Systematic Review.

Posttraumatic stress disorder (PTSD) is associated with a high burden of disability and mortality. Despite standard treatments with antidepressants and/or psychotherapy, remission is often difficult to achieve. Electroconvulsive therapy (ECT) is an effective treatment for mood disorders but is currently not recognized as a treatment modality for PTSD. The literature about its potential role in the management of PTSD is growing. Thus, we aim to systematically review the available evidence for the role of ECT in PTSD.Adhering to the Preferred Reporting Items for Systematic Reviews and Meta-analy…

Network integration and segregation changes in schizophrenia: impact of electroconvulsive therapy.

BACKGROUND: Studies have confirmed brain network topology disruption in schizophrenia (SZ). Electroconvulsive therapy (ECT) rapidly improves acute psychiatric symptoms, yet the exact mechanism by which it impacts brain network topology in SZ patients remains unclear. This study aims to explore topological changes in SZ patients' whole-brain functional networks during ECT, ultimately elucidating implicated neurological mechanisms. METHODS: This study collected resting-state functional magnetic resonance imaging (rs-fMRI) data from 53 patients with schizophrenia before and after ECT, as well as…

Attitudes and Knowledge of Electroconvulsive Therapy: An Exploration of Medical Students' Perspectives.

AIMS: To explore knowledge and attitudes of medical students before and after exposure to a technology-enhanced learning and teaching (TELT) session about electroconvulsive therapy (ECT). BACKGROUND: Adequate knowledge of and attitudes toward ECT is essential for medical students. However, the impact of teaching on knowledge and attitudes is infrequently studied. METHODS: Fourth- and fifth year medical students at the University of Glasgow were provided with an online questionnaire before and after engaging in a remote TELT session, led by a consultant psychiatrist with expertise in ECT. The…

Predictors of cognitive changes in patients with schizophrenia undergoing electroconvulsive therapy.

INTRODUCTION: Previous studies on the effects of electroconvulsive therapy (ECT) on cognition in schizophrenia have been inconclusive. This study aimed to identify factors that may predict cognitive improvement or deterioration in patients with schizophrenia after-ECT. MATERIALS & METHODS: Patients with schizophrenia or schizoaffective disorder with predominantly positive psychotic symptoms, who were treated with ECT at the Institute of Mental Health (IMH), Singapore, between January 2016 and January 2018, were assessed. Montreal Cognitive Assessment (MoCA), Brief Psychiatric Rating Scale (BP…

The ethical imperative of trauma-sensitive care for electroconvulsive therapy (ECT).

BACKGROUND: The empirical literature on the lived experience of electroconvulsive therapy (ECT) is limited, divergent, and largely focused on the experiences of procedures, (adverse) effects, information provision, or decision-making. AIMS: This study aimed to investigate aspects related to the lived experience and meaning-making of people who have had ECT. METHOD: In-depth interviews with 21 women (21-65 years old) were analyzed using interpretative phenomenological analysis (IPA). FINDINGS: A subgroup of nine participants described more negative experiences with ECT. A common factor for t…

Reviewing the neurobiology of electroconvulsive therapy on a micro- meso- and macro-level.

BACKGROUND: Electroconvulsive therapy (ECT) remains the one of the most effective of biological antidepressant interventions. However, the exact neurobiological mechanisms underlying the efficacy of ECT remain unclear. A gap in the literature is the lack of multimodal research that attempts to integrate findings at different biological levels of analysis METHODS: We searched the PubMed database for relevant studies. We review biological studies of ECT in depression on a micro- (molecular), meso- (structural) and macro- (network) level. RESULTS: ECT impacts both peripheral and central inflamma…

Examining the clinical effectiveness of continuation and maintenance electroconvulsive therapy in schizophrenia.

OBJECTIVE: There are few studies that examine the effectiveness of Continuation/Maintenance Electroconvulsive Therapy (C/M-ECT) in schizophrenia, despite the documented effectiveness of acute ECT treatment. We aimed to investigate the clinical effectiveness of C/M-ECT for in-patients with Schizophrenia in a naturalistic setting. We examined the medical records of 46 in-patients who were diagnosed with Schizophrenia and had received C/M-ECT belonging to non-acute extended care service in a public psychiatry hospital in Sydney, Australia. The focus of analysis was on 138 treatment cycles (71 acu…

Historical Highlights on Electroconvulsive Therapy Practice in Iraq: A Brief Narrative.

Electroconvulsive therapy (ECT) is an established treatment option for severe psychiatric disorders. Electroconvulsive therapy practice began in Iraq in 1946, primarily to treat patients with schizophrenia. Since then, it has developed and expanded to treat mood disorders and major depression. Initially, both modified and unmodified ECT was administered to patients in Ibn Rushd and Al Rashad mental hospitals in Baghdad. The ongoing political turmoil from 3 wars, economic sanctions, and internal conflicts halted the development of mental health services across the country; ECT provision was not…

Effects of Electroconvulsive Therapy on Glycemic Control in Type 1 Diabetes.

Electroconvulsive therapy (ECT) is a treatment modality for refractory depression and other severe psychiatric diseases. Depression is a common comorbid condition of diabetes. Yet, evidence regarding the effect of ECT on glycemic control in patients with diabetes is limited and conflicting, with reports of both exacerbation and amelioration of hyperglycemia. A 52-year-old Caucasian man with a history of type 1 diabetes mellitus (T1DM) was admitted for ECT therapy in the setting of worsening depression refractory to medical treatment. Pre-admission glycemic control was poor. He had significant …

Intercorrelation of physiological seizure parameters and hormonal changes in electroconvulsive therapy.

OBJECTIVE: Physiological parameters that predict electroconvulsive therapy (ECT) effectiveness may reflect propagation of the induced epileptic seizure. As an indication of seizure propagation to the diencephalon, we here examined the correlation between prolactin increase after ECT and clinical seizure evaluation parameters, focusing on peak heart rate. As a proxy for peripheral endocrine stress response, we examined the correlation to postictal cortisol increase. METHODS: Participants were consecutively recruited from clinical ECT patients (n = 131, age 18-85 years). The first ECT session i…

Successful electroconvulsive therapy for depression in a man with cerebral amyloid angiopathy.

Cerebral amyloid angiopathy (CAA) is a condition characterised by accumulation of amyloid beta protein (Aβ) in the wall of cerebral blood vessels which increases the risk of intracranial haemorrhage and contributes to cognitive impairment. We describe the case of a man around the age of 70 with 'probable' CAA according to the modified Boston criteria and severe depression whose depression was treated successfully with electroconvulsive therapy (ECT). To the best of our knowledge, there are no earlier published reports of ECT in a patient with CAA. We briefly discuss possible safety measures fo…
