Cryptogenic stroke in a young adult due to pulmonary arteriovenous malformation.

Pulmonary arteriovenous malformations (PAVMs) are rare vascular anomalies that create a right-to-left intrapulmonary shunt, allowing embolic material to bypass the pulmonary capillary filter and enter the systemic circulation. This predisposes affected individuals to paradoxical embolic events, including transient ischaemic attack and ischaemic stroke. Although uncommon, PAVMs represent an important and potentially treatable cause of cryptogenic stroke in young adults. We report a young woman in her early 20s with a 4-year history of intermittent central cyanosis, digital clubbing and exertiona

The Outcomes of Percutaneous Mitral Balloon Valvuloplasty in Patients With Severe Rheumatic Mitral Stenosis and Left Atrial Appendage Thrombus: Systematic Review and Meta-Analysis.

Persistent left atrial appendage thrombus despite anticoagulation is common. Current guidelines advise against percutaneous mitral balloon valvuloplasty (PMBV) because of perceived thromboembolic risk, leaving surgical mitral valve replacement-an option associated with substantial morbidity-as the primary alternative. However, the procedural risk of PMBV in the setting of left atrial appendage thrombus remains poorly established. We conducted a systematic review and meta-analysis to assess this. Of the 2,136 studies identified in the initial search, 17 were included in the analysis, comprising

Timing surgery and hemorrhagic complications in endocarditis with concomitant cerebral complications.

BACKGROUND: To date, limited studies have been conducted regarding the safe timing of valvular repair for infectious endocarditis (IE) in patients with radiographic findings consistent with embolic stroke or infectious intracranial aneurysm (IIA). METHODS: A single-center, retrospective review of valvular surgeries for IE was performed (2011-2019). Outcomes for patients who underwent cranial image screening and those who did not were subsequently compared. RESULTS: 276 patients underwent valvular repair for IE; 186 (67.4%) were male. The mean age was 51.0 (17.4) years. Mean time from imaging…

Acute Cardioembolic Stroke in the Setting of Subtherapeutic Anticoagulation.

Acute ischemic stroke is a sudden neurological deficit secondary to decreased or lack of blood flow (perfusion) due to a thrombus or an embolus. Embolic strokes are ischemic strokes that occur due to a distal clot that results in hypoperfusion upstream. Cardioembolic strokes are embolic strokes due to a cardiac origin. Almost a quarter of ischemic strokes are of cardioembolic etiology. Here, we present the case of an 83-year-old female presenting with right-side weakness and aphasia who arrived 45 minutes after symptom onset. Cardioembolic stroke symptoms, diagnosis, treatment, and risk factor…

S-nitrosoglutathione reduces asymptomatic embolization after carotid angioplasty.

BACKGROUND: The major complication of carotid angioplasty is embolic stroke, which may occur after balloon inflation and deflation or in the early postintervention period. Platelet adhesion and aggregation to the angioplasty site with subsequent embolization seems to plays a major role in early postangioplasty embolization and stroke. During this period, asymptomatic embolic signals can be detected in patients by transcranial Doppler ultrasound despite aspirin and heparin treatment. S-nitrosoglutathione (GSNO) is a nitric oxide donor that appears to have relative platelet specificity. We evalu…

Dietary and other risk factors for stroke in Hawaiian Japanese men.

As part of an on-going longitudinal study, 7895 men of Japanese ancestry living on the island of Oahu, aged 45-68 and free of evidence of prior stroke at entry examination, have been followed by re-examinations and surveillance. During ten years of follow-up 154 men developed thromboembolic stroke, 65 developed intracranial hemorrhage, and 19 developed stroke of unknown type. There were 79 deaths attributed to stroke. The independent risk factors for thrombo-embolic stroke were elevated blood pressure, glucose intolerance, age, electrocardiographic evidence of left ventricular hypertrophy or s…

[Antithrombotic therapy for ischemic cerebrovascular accident].

Antithrombotic therapy should be considered in the acute phase of an ischaemic stroke in patients who are not candidates for thrombolysis. The recommended treatment is aspirin, 160 or 300 mg daily, associated in bed-ridden patients with prophylactic heparin therapy to avoid venous thromboembolic complications. Although not founded on scientific proof, high dose anticoagulants may be used instead of aspirin in special cases presumed to be at high risk of early recurrence of embolism or of extension of thrombosis. Secondary prevention in patients who have already suffered a non-embolic ischaemic

Circulating Microparticles in Patients with Symptomatic Carotid Disease Are Related to Embolic Plaque Activity and Recent Cerebral Ischaemia.

BACKGROUND AND PURPOSE: In order to assess the association of microparticles derived from activated platelets (PMP) or endothelial cells (EMP) with risk markers for recurrent embolic events in patients with symptomatic carotid artery disease, we studied the associations between PMP/EMP and three risk markers: plaque haemorrhage (PH), micro-embolic signals and cerebral diffusion abnormalities. METHODS: Patients with recently symptomatic high-grade carotid artery stenosis (60-99%, 42 patients, 31 men; mean age 75 ± 8 years) and 30 healthy volunteers (HV, 11 men; mean age 56 ± 12 years) were pro

Embolism from the aortic arch in patients with cerebral ischemia.

The pathogenic role of aortic arch atherosclerosis (AAA) in embolic stroke is not well understood. We investigated, prospectively, the prevalence and severity of AAA in patients with embolic stroke to ascertain its role as a risk factor for cerebral ischemia. We examined 100 consecutive patients who had experienced acute symptoms due to cerebral ischemia. Clinical examination, electrocardiogram, x-ray, ultrasound examination of craniocervical arteries, transesophageal echocardiography (TEE), cranial computerized tomography, and magnetic resonance imaging were undertaken. Seventy-five patients …

Central retinal artery occlusion in a patient with aortic valve papillary fibroelastoma: a case report.

BACKGROUND: Cardiac papillary fibroelastoma are rare benign tumours of the endocardium, often arising from heart valves. Despite their benign histology, papillary fibroelastoma carry high embolic potential and can lead to ischaemic events. In contrast to stroke, ocular embolization including central retinal artery occlusion is a rather rare first manifestation. CASE SUMMARY: We report a 61-year-old female with sudden painless monocular vision loss due to central retinal artery occlusion. Initial stroke and cardiovascular work-up revealed a very small floating mass (0.6 × 0.4 × 0.2 cm) on the

Treatment of stroke in aged male and female rats with Vepoloxamer and tPA reduces neurovascular damage.

UNLABELLED: Stroke is a leading cause of death and disability worldwide, mainly affecting the elderly. Unfortunately, current treatments for acute ischemic stroke warrant improvement. To date, tissue plasminogen activator (tPA) is of limited use in stroke patients mainly due to its narrow therapeutic window and potential for hemorrhagic complication. The adjuvant treatment with Vepoloxamer, a purified amphipathic polymer has been shown to enhance the thrombolytic efficacy of tPA treatment in young adult male rats after embolic stroke. However, most stroke patients are aged; therefore, the curr…

Role of carotid plaque analysis in therapeutic decision-making for carotid disease.

Carotid atherosclerosis is an important yet declining etiology of embolic stroke and cerebrovascular or retinal ischemic events with an embolic source. The severity of stenosis is the most important anatomic factor for deciding whether to use optimized medical therapy alone or interventional therapy. Recently, specific features of the atherosclerotic plaque have attracted increasing attention; for example, total plaque area, juxtaluminal black area, intraplaque hemorrhage, and mainly echolucent plaques are associated with high risk of late stroke, and these features now influence the therapeut…

Acute ischemic stroke in Tsutsugamushi: understanding the underlying mechanisms and risk factors.

BACKGROUND: Tsutsugamushi (scrub typhus) is an acute infectious febrile disease common in the Asia-Pacific region. Common symptoms of tsutsugamushi include lymphadenopathy, fever, and myalgia, and it rarely causes acute ischemic stroke (AIS). However, we hypothesized that tsutsugamushi infection could trigger AIS. METHOD: We retrospectively examined patients diagnosed with AIS within 2 weeks of tsutsugamushi diagnosis at three hospitals over a 15-year period. We categorized patients who developed AIS while being treated for tsutsugamushi as the case group and those (of similar age and sex) wh

Pregnancy, patent foramen ovale and stroke: a case of pseudoperipheral facial palsy.

The pathogenetic role of patent foramen ovale (PFO) in embolic stroke and its prognostic and therapeutic implications have not yet been clearly defined. Nonetheless, recent availability of non-invasive diagnostic techniques, such as the transcranial Doppler (TCD), has increased the frequency with which this anomaly is diagnosed. Here we present the case of a young woman affected by post-partum peripheral facial palsy: further exams disclosed not only its truncal-ischaemic origin, but also, significantly, the presence of PFO, as well as of anticardiolipin antibodies (acL). Given the increased e…

Short-duration atrial fibrillation in ischemic stroke: high risk despite subclinical burden-a prospective cohort study.

BACKGROUND AND PURPOSE: Atrial fibrillation (AF) episodes ≥ 30 s are currently considered clinically relevant in stroke diagnostics. However, shorter AF episodes may signal a significant embolic risk, especially in patients with embolic stroke of undetermined source (ESUS). This study investigates the prevalence, risk profile, and stroke severity associated with short-duration AF (SDAF < 30 s) across ischemic stroke subtypes. METHODS: We prospectively enrolled 714 consecutive patients with ischemic stroke or Transient ischemic attacks who underwent ≥ 48-hour ECG monitoring. AF episodes were cl…

Prevalence of Cardio-Embolic Brain Complications in Permanent and Paroxysmal Atrial Fibrillation Patients.

Background: Atrial fibrillation (AF) is the most frequent of all cardiac arrhythmias, with an increasing prevalence in the last 20 years. Cardio-embolic brain complications (CEBC) related to AF often occur or recur, even following appropriate treatment. Method: We conducted a retrospective study and analyzed the presence of stroke, dementia, and Parkinson’s disease (PD) in both paroxysmal and permanent AF patients. The records of 1111 consecutive admitted patients with primary diagnosis of AF at the Municipal Emergency University Hospital, Timisoara, between 2015 and 2016 were examined. Statis
