The Safety and Interventions for Redirecting Non-Urgent Patients From Emergency Departments to Primary Care Services: A Scoping Review.

BACKGROUND: Emergency departments (EDs) experience overcrowding, resulting in delayed care for life-threatening conditions, increased healthcare costs, and prolonged waiting times. Non-urgent patients contribute to ED overcrowding, and redirecting these patients to primary care (PC) is a strategy to reduce pressure on emergency services. However, the safety and procedural components of redirection have not been comprehensively described due to limited evidence. This scoping review aimed to identify the safety outcomes and intervention components associated with redirecting non-urgent patients

Can pharmacare reduce suicide-related behaviors? A quasi-experimental study in Ontario youth.

BACKGROUND: Canada's Ontario Health Insurance Plan Plus (OHIP+), implemented in 2018, provided universal prescription drug coverage to youth under 25. Because youth face elevated risks of suicide-related behaviors (SRBs) and frequently encounter cost-related barriers to accessing mental health medications, this study evaluated whether OHIP + impacted emergency department (ED) visits for SRBs among Ontario youth. METHODS: We used linked data from the 2016 Canadian Census and the National Ambulatory Care Reporting System, identifying SRB-related ED visits between April 2016 and March 2020. A si

Emergency medicine updates: Acute respiratory distress Syndrome.

INTRODUCTION: Acute respiratory distress syndrome (ARDS) is a critical condition associated with severe morbidity and mortality. There are several components of management, and it is important for emergency clinicians to be aware of these as they manage critically ill patients who may face extended time in the emergency department. OBJECTIVE: This paper evaluates evidence-based updates concerning ARDS. DISCUSSION: ARDS is a condition associated with diffuse, severe lung inflammation resulting in hypoxemic respiratory failure. While there have been several definitions, the most recent concept

RICsy Business: A Tutorial on Rapid Infusion Catheter Placement and Utilization in Emergency Medicine.

BACKGROUND: In emergency medicine (EM), high-volume fluid resuscitation is often crucial to stabilize critically ill patients. This tutorial aims to provide emergency physicians with a comprehensive guide on the placement and utilization of Rapid Infusion Catheters (RICs)-a large bore peripheral IV inserted by modified Seldinger technique-in emergencies. DISCUSSION: We introduce the RIC as a resuscitation tool, describe a clinical scenario ideal for RIC placement, and provide a step-by-step guide for RIC placement. We briefly discuss the pathophysiology of hypovolemic states and how rapid cor

Small Rare Earth Magnets Adhered to Pharyngeal Tissue in a Pediatric Emergency Department Patient.

BACKGROUND: Small rare earth magnets pose a known health risk to children and many cases of ingestion and aspiration with associated complications have been described. More unusual, but also seen in children, are retained foreign bodies in the oropharynx that require extraction. CASE REPORT: We present the case of a 3-year-old boy with persistent left-sided sore throat 1 h after ingestion of several 3-mm spherical rare earth magnets. No foreign bodies were visible in the oropharynx on examination; however, a chest radiograph revealed two adjacent magnets within the lower pharyngeal space, as

Approaching acute pain in emergency settings; European Society for Emergency Medicine (EUSEM) guidelines-part 2: management and recommendations.

BACKGROUND: In Europe, healthcare systems and education, as well as the clinical care and health outcomes of patients, varies across countries. Likewise, the management of acute events for patients also differs, dependent on the emergency care setting, e.g. pre-hospital or emergency department. There are various barriers to adequate pain management and factors common to both settings including lack of knowledge and training, reluctance to give opioids, and concerns about drug-seeking behaviour or abuse. There is no single current standard of care for the treatment of pain in an emergency, with

Recommendations for the Emergency Department Prevention of Sport-Related Concussion.

Sport-related concussion refers to the subset of concussive injuries occurring during sport activities. Similar to concussion from nonsport mechanisms, sport-related concussion is associated with significant morbidity, including migrainous headaches, disruption in normal daily activities, and long-term depression and cognitive deficits. Unlike nonsport concussions, sport-related concussion may be uniquely amenable to prevention efforts to mitigate these problems. The emergency department (ED) visit for sport-related concussion represents an opportunity to reduce morbidity by timely diagnosis a

Active epistaxis at ED presentation is associated with arterial hypertension.

Epistaxis and hypertension are frequent in the general population, but an association is still controversial. Aim of this retrospective cohort study was to test if active epistaxis at emergency department (ED) presentation is associated with hypertension. Patients with active epistaxis at ED presentation (n = 271; 73%) were compared with controls without active bleeding (n = 101; 27%). By multivariate logistic regression modeling we found that active epistaxis was independently associated with history of hypertension (odds ratio 2.8 [95% CI 1.4 to 5.6; P =.004] adjusted for age, gender, rhinit

Emergency room admissions, meteorologic variables, and air pollutants: a path analysis.

Daily hospital emergency room admissions at hospitals located within 8 km of Los Angeles Basin monitoring stations at Long Beach, Lennox, Azusa, and Riverside, California, were examined for correlations with pollutant and meteorologic variables for 1974-1975. By conventional correlation and regression with lagged and temporospatial analysis, the authors could not distinguish effects of pollution by particulate sulfate from those due to meteorologic variables and oxidant. The authors use a variety of structural models and path analysis to estimate "direct" effects on emergency room admissions o

Lessons Learned From Helene: The Role of a Rural Community Hospital in Disaster Response After a Major Hurricane.

Hurricane Helene struck Western North Carolina in September 2024, leaving an entire region without power, clean water, or internet connectivity for more than a week. A small, rural, community hospital and its emergency department, which normally sees 84 patients per day, suddenly became the sole source of medical care for an estimated 200,000 residents and experienced a 70% surge in patient volume. Although structural damage to the hospital was limited, the complete loss of digital connectivity rendered electronic medical records, laboratory information systems, radiology, pharmacy, and routin

Pediatric and Adult Fracture Incidence: A Decreasing Trend With Increasing Hospital Admissions.

Large-scale studies examining fracture trends and epidemiological data are lacking. The purpose of this study was to evaluate the incidence of fractures presenting to US emergency departments using the National Electronic Injury Surveillance System. A total of 7,109,078 pediatric and 13,592,548 adult patients presenting to US emergency departments with a fracture between 2008 and 2017 were analyzed for patterns. Fractures accounted for 13.9% of pediatric injuries and 15% of adult injuries. Among children, fracture incidence was highest in the group 10 to 14 years old and most frequently involv

Interventions for preventing and reducing the use of physical restraints of older people in general hospital settings.

BACKGROUND: Physical restraints, such as bedrails, belts in chairs or beds, and fixed tables, are commonly used for older people in general hospital settings. Reasons given for using physical restraints are to prevent falls and fall-related injuries, to control challenging behavior (such as agitation or wandering), and to ensure the delivery of medical treatments. Clear evidence of their effectiveness is lacking, and potential harms are recognised, including injuries associated with the use of physical restraints and a negative impact on people's well-being. There are widespread recommendation

Intoxicated persons showing challenging behavior demand complexity interventions: a pilot study at the interface of the ER and the complexity intervention unit.

Intoxicated persons showing challenging behavior (IPCBs) under influence of alcohol and/or drugs frequently have trouble finding appropriate acute care. Often IPCBs are stigmatized being unwilling or unable to accept help. Separated physical and mental healthcare systems hamper integrated acute care for IPCBs. This pilot aimed to substantiate the physical, psychiatric, and social health needs of IPCBs visiting the emergency room (ER) during a 3-month period. All ER visits were screened. After triage by the ER physician, indicated IPCBs were additionally assessed by the consultation-liaison-psy

Digitising wound care: a cost-consequence analysis of the Wound Care Command Centre™ in Australia.

BACKGROUND: Chronic wounds pose considerable financial challenges for healthcare systems globally, with most cases requiring hospital care and extended lengths of stay, particularly due to delayed access to treatment. To address this, Sydney Local Health District (LHD) in Australia launched the Wound Care Command Centre™ in 2023, utilising a digital application for timely access to wound care and to reduce the burden on hospitals. This study evaluates the cost consequences of this Centre by comparing healthcare service use under this new model of care compared to service use under standard cli
