BACKGROUND: Ménière's disease (MD) mainly refers to the endolymphatic hydrops in membranous labyrinth of the inner ear. Application of the m … MMP9 was down-regulated. Immunohistochemistry confirmed that CHMP1A and MMP9 were expressed in the endolymphatic sacs of MD patients and in the inner ear of adult mice.
CONCLUSIONS AND SIGNIFICANCE: Endocytosis may be involved in the pathogenesis of sporadic MD, furthermore CHMP1A, VPS4A, FCN3 and MMP9 could be considered as potential biomarkers.

Two temporal bone-eustachian tube (ET) specimens, one from a 34-gestational week-old male newborn and the other from a 3-month-old female infant, both with trisomy 22, were studied histopathologically. Both cases demonstrated severe inner-ear anomalies including hypoplastic cochlea, atrophy of the saccular macula, and dislocated endolymphatic sac and vestibular aqueduct. In addition, they had several anomalies in the ET including widely developed medial lamina of the ET cartilage, undeveloped lateral lamina of the ET cartilage, and absence of attachment of the tensor veli palatini muscle to th…

This article reviews recent imaging developments in various diseases of the membranous labyrinth. Membranous labyrinth is the initial receptor for acoustic stimuli. Exciting progress has been made in the imaging evaluation of the normal and diseased labyrinth. Structures of the membranous labyrinth, such as the cochlea, vestibule, semicircular canals, and endolymphatic duct and sac can be visualized on routine MR scans. Using high-resolution MR imaging techniques, researches have been able to visualize the cochlear duct, scala tympani, scala vestibuli, and even the otolithic mass. The sensitiv

Recent biochemical studies revealed that ric-8A encodes a guanine nucleotide exchange factor for a subset of Galpha proteins. Ric-8 is a key component of a signaling network in C. elegans that regulates neurotransmitter secretion and also plays a role in centrosome-mediated events during early embryogenesis. Here we show that during the early development in mice (E9.5-E12.0) ric-8 (synembryn) is expressed in the developing nervous system such as the cranial ganglia, neural tube, sympathetic chain and dorsal root ganglia. Ric-8 is also found in the lens, vomeronasal organ, and endolymphatic sac…

One, 2, and 4 months after surgical obliteration of the endolymphatic sac, the sequence of degenerative changes in the organ of Corti of the guinea pig was studied. The block surface technique with interference differential (Nomarski) microscopy was used for this investigation to study the morphological changes in the organ of Corti. The hair cell loss was calculated and mapped in cytocochleograms. One month postoperatively a minimal loss of only outer hair cells was observed in the apical cochlear turn. At 2 months a progression of outer hair cell loss was seen, which proceeded in the 4-month…

Pigmented guinea pigs were chronically implanted bilaterally with a platinum electrode on each round window. After recovery the endolymphatic sac was destroyed and the duct blocked on one side only; the other side was employed as a control. The round window response thresholds on both sides were recorded several times per week over a three month period. There were three main results. A sensitivity loss of up to 20 dB was observed for frequencies between 250 Hz and 6.4 kHz within two weeks post-op. At the end of three months the threshold elevation for these frequencies was as much as 50 dB. On…

Some cochlear fibre response properties have been measured in two GPs approximately one year after induction of endolymphatic hydrops (by surgical obliteration of the endolymphatic sac and duct). These animals are considered as models of the effects of hydrops in Menière's disease, and the purpose of the study was to examine any modifications of fibre response properties which may underly auditory symptoms of the disease in man. Neurones towards more apical cochlear regions (with low characteristic frequencies) showed the greatest deterioration in tuning properties; on average, in the 1-6 kHz …

BACKGROUND: Endolymphatic sac decompression has shown limited success for the treatment of Ménière's disease (MD). We have published a novel technique with very promising results for the treatment of refractory MD: the Endolymphatic Duct Blockage (EDB) with two titanium clips. One of the challenges of this technique was an increased incidence of intraoperative Cerebrospinal Fluid (CSF) Leak.
OBJECTIVE: To assess the effect of intraoperative CSF Leak on short-term surgical outcomes.
METHODS: Retrospective comparative study in a tertiary care center (61 patients). MD patients who underwent EDB…

Tumors of the inner ear and adjacent structures often present with hearing loss, tinnitus and vertigo due to compression of the traversing cranial nerves. More than 90% of the tumors of the inner ear with or without expansion into the cerebellopontine angle are histologically diagnosed as vestibular schwannomas. Less common tumorous lesions include ectopic meningiomas located in the petrous bone, glomus tympanicum paragangliomas or endolymphatic sac tumors (ELST) originating in the vestibular recess. Most tumors are sporadic, but hereditary disorders have to be considered. Bilateral vestibular…

This study was based on observations on 14 normal guinea pigs and 17 guinea pigs in which the left endolymphatic duct and sac had been surgically obliterated to induce endolymphatic hydrops. Three groups of hydropic animals were tested 1, 2 and 3 months after surgery. The Ca2+ concentration and endocochlear potential (EP) were recorded by means of double-barrelled ion selective micro-electrode in the third turn of the cochlea. The results indicated that a decrease in EP and an increase in Ca2+ in hydropic cochlea, a negative correlation was present. In the control ear, the values of Ca2+ and E

The prognostic value of electrocochleography and of the glycerol dehydration test in Meniere's disease is well recognized. Abnormal electrocochleographic waveforms carry a poorer prognosis. After adequate dehydration, significant threshold changes for better or worse predict the likely course of events following endolymphatic sac surgery. Meniere's disease frequently becomes bilateral. Vestibular aqueduct tomography in patients with Meniere's disease gives very different results, in both ears, compared with those in control patients. The acetazolamide cochlear hydration test can help detect la…

The role of CT and MR studies in the evaluation of patients with Meniere's disease is to exclude other pathologic conditions that could masquerade as Meniere's disease clinically. This article is divided into three sections corresponding to the three major symptoms of Meniere's disease. Each section presents examples of CT and MR pathologic conditions that could mimic these clinical findings: "sensorineural hearing loss" covers acoustic neuromas and other cerebellopontine angle tumors, inner ear anomalies, otosclerosis, and more; "vertigo" includes examples of labyrinthitis, endolymphatic sac …

Forty-six cases in whom endolymphatic drainage surgery was performed between 1986 and 1992 were followed up for more than 2 years. The results of numerical values (1985 AAOHNS) revealed complete control of definite spells in 26 cases; substantial control of definite spells in 18 cases; limited control of definite spells in 1 case; and insignificant control of definite spells in 1 case. Hence, endolymphatic drainage surgery for Meniére's disease was effective. There are, however, cases who cannot be operated on because of old age or underlying diseases. In these, we injected gentamycin sulfates
