An outbreak of necrotizing enterocolitis associated with a novel clostridium species in a neonatal intensive care unit.

An outbreak of necrotizing enterocolitis (NEC) occurred in 6 neonates within a 2-month period. Blood cultures from 3 of these neonates grew the same strain of what appears to be a novel clostridial species for which the name "Clostridium neonatale" has been proposed. A point-prevalence survey that used rectal swabs was performed in our intensive-care and intermediate-care nurseries, and it indicated that 20.8% of neonates carried this same "C. neonatale" strain despite having no evidence of NEC. In conclusion, we describe an outbreak of NEC associated with the novel species, and we suggest tha

Ileoileal intussusception and ileal stricture associated with necrotizing enterocolitis in a premature infant: report of a case.

A 630-g male infant developed presumed necrotizing enterocolitis (NEC) after fungal enteritis that resulted in subsequent fungal septicemia associated with pneumoperitoneum on the 9th day of life. Urgent percutaneous Penrose drainage was required. During the operation on the 14th day, an ileoileal intussusception and an ileal stricture were found with perforations on each oral side of the lesions. The distinction between NEC and intussusception in premature infants when they coexist can be difficult, as these conditions share common symptoms. The relation between ischemic injury and dysmotilit

Risk factors for necrotizing enterocolitis in preterm infants: a 1:1 matched case-control study.

IMPACT: This study demonstrates that neonatal sepsis, blood transfusion, combined antibiotic use, asphyxia, and maternal gestational diabetes (GDM) are independent risk factors for NEC. By utilizing a 1:1 matched case-control design, we effectively eliminated the confounding effects of gestational age and birth weight, revealing critical perinatal drivers of the disease. The resulting prediction model (AUC = 0.937) provides a robust tool for early screening. These findings underscore the urgent need for enhanced antibiotic stewardship and rigorous maternal GDM management to reduce NEC incidenc

Serum vitamin A and E improve neonatal necrotizing enterocolitis through activation of SOD/GPx pathway.

BACKGROUND: Colitis, including necrotizing enterocolitis (NEC), is a common and serious disease in newborns. OBJECTIVES: This study aimed to investigate the specific mechanisms of vitamins A and E in neonatal NEC, given their known roles in inhibiting NF-κB, regulating intestinal flora and reducing inflammatory cytokines. METHODS: The study measured serum vitamin A and E levels in healthy and NEC newborns/mice. An NEC model was established and mice were treated with vitamins A/E, DDC (SOD/GPx inhibitor), or DDW (SOD/GPx activator). Serum vitamin levels and intestinal inflammatory factors wer

[Berberine attenuates necrotizing enterocolitis-associated brain injury via the TLR4/MyD88/NF-κB/NLRP3 pathway].

OBJECTIVES: To investigate the protective effects and underlying mechanisms of berberine (BBR) on brain injury related to neonatal necrotizing enterocolitis (NEC). METHODS: An NEC mouse model was established and mice were assigned to control, model, low-dose BBR (2.5 mg/kg), and high-dose BBR (5 mg/kg) groups (n=24 per group). Body weight and survival were recorded. Hematoxylin-eosin staining was performed to assess pathological damage in intestinal and brain tissues; Nissl staining was performed to evaluate neuronal injury in the hippocampus; immunohistochemistry was conducted to detect micr

Glucagon-like peptides agonists promote maturation of intestinal organoids derived from neonates with necrotizing enterocolitis.

PURPOSE: Necrotizing enterocolitis (NEC) majorly affects premature infants, causing not only necrosis and inflammation but also feeding intolerance and gastrointestinal dysmotility, hinting at gut hormone secretion impairment. Particularly critical is the gestation period before 26 weeks where intestinal hormonal activity is partially developed, rendering preterm neonates highly susceptible to NEC. Emerging evidence suggests a role of gut hormones, especially glucagon-like peptides (GLP) in ileum development. Herein, the aim of this study was to determine the effect of modulating GLP signaling

Neonatal leucocyte cell population data: reference intervals and relevance for detecting sepsis and necrotizing enterocolitis.

BACKGROUND: Timely diagnosis of neonatal sepsis is crucial but remains challenging. Cell Population Data (CPD) provide high-resolution phenotyping of leukocytes, offering potential for sepsis detection. We aimed to establish neonatal CPD reference intervals and explore their capacity to detect sepsis and necrotizing enterocolitis (NEC). METHODS: CPD from neutrophils, monocytes, and lymphocytes were analyzed in hospitalized newborns. Reference intervals (5-95th percentiles), at birth and during the first 28 days, were derived from newborns without conditions potentially impacting CPD (referenc

A Multi-Center Survey of Necrotizing Enterocolitis Prevention Strategies in Very Low Birth Weight Infants.

OBJECTIVE: To compare the prevention practices of necrotizing enterocolitis (NEC) across 17 neonatal intensive care units (NICUs) in China. METHODS: A web-based survey was sent to 17 level 3 NICUs in China on September 21, 2023, to evaluate the prevention strategies for NEC. RESULTS: All 17 Neonatal Intensive Care Units (NICUs) responded to the survey. There was significant variation in the initial empirical use of antibiotics for early-onset sepsis, late-onset sepsis, and NEC among different NICUs. Out of the 17 NICUs, only 5 (29.4%) used donor human milk. Additionally, 15 (88.2%) NICUs per

Assessment of inflammatory biomarkers to identify surgical/death necrotizing enterocolitis in preterm infants without pneumoperitoneum.

BACKGROUND: Necrotizing enterocolitis (NEC) is a life-threatening disease that affects premature infants. However, the role of inflammatory biomarkers in identifying surgical/death NEC without pneumoperitoneum remains elusive. PURPOSE: We aimed to verify the value of platelet-to-lymphocyte ratio (PLR) and the combination of white blood cell (WBC), absolute neutrophil count (ANC), absolute lymphocyte count (ALC), neutrophil lymphocyte ratio (NLR), PLR, C reactive protein (CRP) and procalcitonin (PCT) in predicting the severity of NEC, and to construct a model to differ surgically NEC from non-

Macrophage-derived IGF-1 protects the neonatal intestine against necrotizing enterocolitis by promoting microvascular development.

Necrotizing enterocolitis (NEC) is a deadly bowel necrotic disease of premature infants. Low levels of plasma IGF-1 predispose premature infants to NEC. While increasing evidence suggests that defective perinatal intestinal microvascular development plays a role in NEC, the involved mechanism remains incompletely understood. We report here that serum and intestinal IGF-1 are developmentally regulated during the perinatal period in mice and decrease during experimental NEC. Neonatal intestinal macrophages produce IGF-1 and promote endothelial cell sprouting in vitro via IGF-1 signaling. In vivo

[Necrotizing enterocolitis after an isolated event of supraventricular tachycardia. Case report of atypical neonatal presentation].

Supraventricular tachycardia (SVT) is the main tachyarrhythmia in the newborn (NB) that requires urgent resolution. Necrotizing enterocolitis (NEC) is the most common gastrointestinal emergency that mainly affects premature infants. Although these conditions are recognized as distinct pathologies, literature reports suggest that episodes of SVT may predispose patients to NEC secondary to disturbances in mesenteric blood flow and a decrease in tissue perfusion. We present here the clinical case of a premature neonate who developed NEC after an isolated SVT event with low cardiac output.

Probiotics for Prevention of Severe Necrotizing Enterocolitis: Experience of New Zealand Neonatal Intensive Care Units.

Introduction: Necrotizing enterocolitis (NEC) affects mainly preterm infants, has a multifactorial etiology and is associated with intestinal dysbiosis and disordered immunity. Use of probiotics for prophylaxis is beneficial with studies indicating reduction in NEC ≥ stage 2, late onset sepsis (LOS) and mortality. However, not all studies have shown a reduction, there are questions regarding which probiotic to use, whether infants <1,000 g benefit and the risk of probiotic sepsis. All neonatal intensive care units in New Zealand (NZ) use probiotics and contribute to an international database (

Bifidobacterium adolescentis protects against necrotizing enterocolitis and upregulates TOLLIP and SIGIRR in premature neonatal rats.

BACKGROUND: Necrotizing enterocolitis (NEC) is a serious gastrointestinal disorder that is often seen in premature infants. Probiotics decrease the risk of NEC; however, the mechanism by which probiotics work is not clear. The goal of this study was to evaluate the preventive effect of Bifidobacterium adolescentis in an NEC rat model. METHODS: Sprague-Dawley neonatal rats were obtained by caesarean section after 20-21 d gestation and randomly divided into the following 3 groups: dam fed (DF), formula fed (FF), and formula + B. adolescentis (FB). Those in the FF and FB groups developed NEC aft

Necrotizing enterocolitis and respiratory distress syndrome as first clinical presentation of mitochondrial trifunctional protein deficiency.

BACKGROUND: Newborn screening (NBS) for long-chain 3-hydroxy acyl-CoA dehydrogenase (LCHAD) deficiency does not discriminate between isolated LCHAD deficiency, isolated long-chain keto acyl-CoA (LCKAT) deficiency and general mitochondrial trifunctional protein (MTP) deficiency. Therefore, screening for LCHAD deficiency inevitably comprises screening for MTP deficiency, which is much less amenable to treatment. Furthermore, absence of a clear classification system for these disorders is still lacking. MATERIALS AND METHODS: Two newborns screened positive for LCHAD deficiency died at the age of

Importance of early ileostomy closure to prevent chronic salt and water losses after necrotizing enterocolitis.

Six infants with necrotizing enterocolitis were discharged after periods of prolonged hospitalization (two to nine months) with intact ileostomies. Their initial hospitalization was complicated by feeding difficulties, chronic diarrhea, sepsis, rickets, and developmental delay. All were rehospitalized within three months, with severe acidosis and dehydration after a presumed viral-type illness. Each had large-volume ileostomy output, which was rich in electrolytes and bicarbonate. A prolonged recovery phase (two to eight months) again was punctuated with episodes of diarrhea, problems in start

Identification of key mitochondria-related genes in necrotizing enterocolitis using single-cell hdWGCNA and experimental verification.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe gastrointestinal disorder in preterm infants. The interplay between mitochondrial metabolism and immune inflammation in its development is not fully understood. METHODS: Single-cell data were analyzed using dimensionality reduction, clustering, and the high-dimensional weighted gene co-expression network analysis (hdWGCNA) algorithm to identify key gene modules in monocytes. GSE46619 was integrated with the MitoCarta3.0 to identify mitochondria-associated differentially expressed genes (MitoDEGs). Acyl-CoA synthetase long-chain family me

Protective effects of enteral acetate supplementation against brain injury in a rat model of necrotizing enterocolitis.

BACKGROUND: Necrotizing enterocolitis (NEC) is a severe gastrointestinal disease in preterm infants, often leading to brain injury. The microbiota-gut-brain axis (MGBA) plays a key role, with short-chain fatty acids (SCFAs) emerging as potential therapeutic agents. This study explores the impact of SCFAs, particularly acetate, in mitigating NEC-related brain injury. METHODS: A NEC rat model was established via overfeeding, hypoxia, and asphyxia. Intestinal injury, SCFA levels, systemic inflammation, and neuroinflammation were assessed through histology, gas chromatography-mass spectrometry, E
