A survey of voiding dysfunction in children with attention deficit-hyperactivity disorder.

PURPOSE: Physicians treating attention deficit-hyperactivity disorder (ADHD) have long had the clinical impression that these children suffe … l group without ADHD. The DVSS consists of 10 questions that assess daytime incontinence, nocturnal enuresis, constipation, urgency, voiding frequency and dysuria, each scored from 0 to 4 (0-never, 1-almost never, 2-less than half the time, 3-about half the time, 4-almost every time) for a maximum total score of 40 (severest symptoms). Scores for patients …

A novel CLDN16 mutation in familial hypomagnesemia with hypercalciuria and nephrocalcinosis
.

Familial hypomagnesemia with hypercalciuria and nephrocalcinosis (FHHNC) is a rare autosomal recessive tubulopathy caused by mutations in either the CLDN16 or CLDN19 genes. Two children from a consanguineous family of Chinese presented with enuresis, polyuria, polydipsia, and failure to thrive. Laboratory studies revealed hypomagnesemia, hypercalciuria, sterile leukocyturia, microscopic hematuria, and renal insufficiency. Renal ultrasound showed bilateral medullary nephrocalcinosis and urolithiasis. Gene sequencing showed compound heterozygous, missense mutations c.416C>T (p.Ala139Val) and c.3…

Temporary urinary incontinence in the acute psychiatric patient without delirium or dementia.

In a prospective study of the 295 admissions in one year to an acute psychiatric ward, 14 subjects without delirium or dementia were found to have developed temporary urinary incontinence. When compared with matched controls (n = 56) the incontinent patients were found to be suffering from psychosis (P less than 0.0002) and to have a history of childhood enuresis (P less than 0.01). Compared with psychotic controls (n = 22), the incontinent patients were older (P less than 0.05), they had been exposed to a greater variety of treatments (P less than 0.01), and they had received more thioridazin…

Neuropsychiatric diagnosis and psychotropic medication prescription patterns in a mental hospital-based child and adolescent psychiatric service in Nigeria.

Mental hospital-based data on child and adolescent psychiatric services are uncommon in low-resource countries, although such data are important for service development and planning. Data are reported on neuropsychiatric diagnoses and psychotropic medication prescription in a child and adolescent psychiatric service in southeastern Nigeria. Schizophrenia-like and other psychotic disorders, followed by seizure disorders, were the most prevalent, while the least prevalent included autism spectrum disorders, enuresis, adolescent postpartum psychosis, and adjustment disorders, among others. Most f…

[Febrile Urinary Tract Infections: When are they an Emergency?].

Urinary tract infections should be suspected in every febrile infant until proven otherwise. While older children and adolescents mostly present with classical features such as dysuria, flank pain and/or abdominal pain, secondary enuresis or pollakiuria, diagnosing infants or newborns, who are incapable of describing or pinpointing pain, can be much more challenging. As such, an infection with gradual and subtle onset may result in fulminant disease and emergency situations. In children with urinary tract infections and known mechanical or functional obstructions of the urinary tract, adequate…

[Pronounced physiological pupillary hippus].

BACKGROUND: The term "hippus" describes pupillary oscillations of variable amplitude and frequency and may be physiological or part of a pathological entity. Thus, pupillary hippus is often the target of extensive clinical investigation. CASE REPORT: A 9-year-old boy was seen in our emergency room in March 1998. Examination showed distinct pupillary oscillations in the absence of any other pathological findings. We presented the patient to a pediatric neurologist. Apart from a history of enuresis and of motor tics in the neck region, the present neurological examination was normal. CONCLUSIO…

[Constipation in children].

Constipation in childhood is often unrecognized and diagnosed upon manifestations such as pain and pseudo-diarrhoea reflecting of a fecalith. It is sometimes complicated with urinary infection and can be a factor favouring enuresis. In the absence of deterioration in the general condition, it reflects modification of the propulsive functions of the colon or of the reservoir function of the rectum. In a such context, no further explorations are required. Treatment involves dietary recommendations such as good nutritional balance and sufficient fibre intake as well as mineral-based laxatives (pa…

Pediatric urinary incontinence.

A clear understanding of the current concepts, pathophysiology, and typical symptoms of voiding dysfunction and pure nocturnal enuresis usually allows the clinician to distinguish these troublesome but benign problems from true underlying pathological conditions of the urinary tract. If a careful history confirms that wetting is the only symptom, and the pattern of wetting is consistent with uninhibited bladder contractions, or if the wetting occurs only during sleep, initial empiric treatment rather than invasive investigation is our preference. On the other hand, if the wetting is associated…

Psychophysiological correlates of 'bipolarity'.

Four hundred patients with major affective disorders were divided into six groups based on lifetime composite evaluations of the intensity, frequency, and duration of their high energy-positive mood states. This division was termed degree of 'bipolarity'. Within the non-bipolar I patients (groups 1-4) the rating of 'bipolarity' was found to be significantly positively associated with enuresis, classical migraine, Raynaud's disease, learning disorders, fingernail biting, and episodic phenomena. The possible significance of this non-bipolar I 'bipolarity' continuum construct is discussed. A sign…

Incontinence: enhancing care in women's prisons.

BACKGROUND: There is no evidence on the prevalence of urinary and faecal incontinence symptoms in women prisoners. AIM: To explore the extent and management of bladder and bowel symptoms to inform prison health services and prison nursing practice. METHOD: An anonymous self-report questionnaire tailored to low levels of English literacy, and administered in one women's prison. RESULTS: Women prisons have a higher reported prevalence of urinary and faecal incontinence, constipation and nocturnal enuresis than community populations; this is an unrecognised health problem. CONCLUSION: Prison …

Evaluation of boys with daytime incontinence by combined cystourethroscopy, voiding cystourethrography and urodynamics.

Objects: Approximately, 1% of school children have daytime urinary incontinence. The symptoms may be caused by an overactive bladder (OAB). In the evaluation of boys with OAB complaints, one should consider a possible urethral cause. The aim of the study was to evaluate the value of a diagnostic regime with cystourethroscopy, voiding cystourethrography (VCUG) and urodynamic pressure-flow studies in boys with OAB complaints after unsuccessful urotherapy and pharmacological therapy. Materials and Methods: Seventy-five boys (5-14 years old) were investigated with cystourethroscopy and within 24 h

Urinary Bladder Dysfunction in Transgenic Sickle Cell Disease Mice.

BACKGROUND: Urological complications associated with sickle cell disease (SCD), include nocturia, enuresis, urinary infections and urinary incontinence. However, scientific evidence to ascertain the underlying cause of the lower urinary tract symptoms in SCD is lacking. OBJECTIVE: Thus, the aim of this study was to evaluate urinary function, in vivo and ex vivo, in the Berkeley SCD murine model (SS). METHODS: Urine output was measured in metabolic cage for both wild type and SS mice (25-30 g). Bladder strips and urethra rings were dissected free and mounted in organ baths. In isolated detrus…
