Implementation and validation of a modeling framework to assess personal exposure to black carbon.

Because people tend to move from one place to another during the day, their exposure to air pollution will be determined by the concentration at each location combined with the exposure encountered in transport. In order to estimate the exposure of individuals in a population more accurately, the activity-based modeling framework for Black Carbon exposure assessment, AB(2)C, was developed. An activity-based traffic model was applied to model the whereabouts of individual agents. Exposure to black carbon (BC) in different microenvironments is assessed with a land use regression model, combined

A statistical method was used for the meta-analysis of tests for latent TB in the absence of a gold standard, combining random-effect and latent-class methods to estimate test accuracy.

OBJECTIVE: Because of the lack of a gold standard, the diagnostic performance of tests for the detection of latent tuberculosis infection (LTBI) is not known. However, statistical methods can be used to estimate the accuracy from the studies reporting the concordance among the tests. STUDY DESIGN AND SETTING: We developed a random-effect latent-class model to estimate performance characteristics of three LTBI diagnostic tests: tuberculin skin test (TST, at 10-mm cutoff), QuantiFERON-TB gold (QFG), and TSPOT-TB from the studies evaluating agreement among the tests. RESULTS: Nineteen studies w

Fine-scale identification of the most likely source of a human plague infection.

We describe an analytic approach to provide fine-scale discrimination among multiple infection source hypotheses. This approach uses mutation-rate data for rapidly evolving multiple locus variable-number tandem repeat loci in probabilistic models to identify the most likely source. We illustrate the utility of this approach using data from a North American human plague investigation.

Fluctuations in climate and incidence of coccidioidomycosis in Kern County, California: a review.

Coccidioidomycosis (Valley Fever) is a fungal infection found in the southwestern United States, northern Mexico, and some places in Central and South America. The fungi that cause it (Coccidioides immitis and Coccidioides posadasii) are normally soil dwelling, but, if disturbed, become airborne and infect the host when their spores are inhaled. It is thus natural to surmise that weather conditions, which foster the growth and dispersal of Coccidioides, must have an effect on the number of cases in the endemic areas. This article reviews our attempts to date at quantifying this relationship in

The United States' potential blood donor pool: estimating the prevalence of donor-exclusion factors on the pool of potential donors.

BACKGROUND: Efforts to ensure donor and recipient safety have reduced the population of eligible voluntary blood donors. The current method for determining eligible blood donors in a population using only age as the criterion for excluding donors poorly reflects the large constellation of factors known to cause donor deferrals. An epidemiologic model has been developed to determine the prevalence of donor exclusions and thus improve the estimate of total eligible blood donors in the nation. STUDY DESIGN AND METHODS: Epidemiologic databases were selected to enumerate the population prevalence

Multivariate models of self-reported health often neglected essential candidate determinants and methodological issues.

BACKGROUND AND OBJECTIVE: Self-reported health is an important indicator of overall well-being that may be influenced by diverse parameters. We intended to evaluate the variety of candidate determinants used in models of self-reported health (SRH) and to examine the methodological problems encountered in multivariate models used in recent studies in this field. STUDY DESIGN AND SETTING: Medline searches identified articles published in 2002 in which SRH was included as an outcome, at least one other variable was used as a determinant of SRH, and the study population was not defined by the pre

Capture-recapture, epidemiology, and list mismatches: several lists.

In applying capture-recapture methods for closed populations to epidemiology, e.g., in the estimation of the size of a diabetes population, one comes up against the problem of list errors due to mistyping or misinformation. This problem has been studied for just two lists by Seber, Huakau, and Simmons (2000, Biometrics 56, 1227 1232) using the concept of tag loss borrowed from animal population studies. In this article, we discuss a similar method that can be extended to an arbitrary number of lists. The methods are applied to an example.

A discussion of potential exposure metrics for use in epidemiological studies on human exposure to radiowaves from mobile phone base stations.

There is currently a high level of concern in many countries that exposure to radiowaves from mobile phone base stations may be hazardous to health. When investigating such suggested risks, epidemiologists need to define an exposure metric that can reliably discriminate between exposed and unexposed groups of people. We conducted a feasibility study to investigate if either short-term measurements of electric field strength, calculations of electric field strength, or distance from nearby mobile phone base stations could be used to develop a metric reflecting an individual's exposure to radiow

Calculation of the frequency of congenital rubella syndrome in Poland.

The annual number and incidence of congenital rubella syndrome (CRS) in Poland was evaluated by means of a methematical model and on the basis of serological studies. In its principles this model describes the infectious kinetics of rubella by the formula y = 1--e-at, where "y" is the rate of immune persons, "a" is a parameter representing the transfer of susceptibles to immunes and "t" is age. The calculated incidence of congenital rubella syndrome resulted in average values of 0.15 to 0.38%, i.e. 15 to 38 CRS cases among 100,000 children born to mothers primarily infected by rubella virus du

Epidemiology of a thermonuclear bomb-burst over Nashville, Tennessee: a theoretic study.

A thermonuclear bomb explosion over any city in the world would have a devastating effect on the population and environment. For those who survive, with or without injuries, life would become primitive with little or no uncontaminated food or water, and with inadequate housing, fuel, and medical care, resulting in a breakdown of family and interpersonal relationships. This theoretic study of the potential outcome of a thermonuclear bomb-burst over Nashville, Tennessee, discusses epidemiologically the wide range of medical and psychologic effects from the direct trauma of blast and fire, widesp

Methodological problems arising from the choice of an independent variable in linear regression, with application to an air pollution epidemiological study.

In epidemiological studies using linear regression, it is often necessary for reasons of economy or unavailability of data to use as the independent variable not the variable ideally demanded by the hypothesis under study but some convenient practical approximation to it. We show that if the correlation coefficient between the "practical" and "ideal" variables can be obtained, then a range of uncertainty can be obtained within which the desired regression coefficient of dependent on "ideal" variable may lie. This range can be quite wide, even if the practical and ideal variables are fairly wel

Exploring the effect of density dependent infection rate in a within host viral infection model.

The most common viral dynamics models for analyzing viral infections assume a well-mixed spatial distribution between viral particles and un … ls that treat the cellular infection probability as a constant. Building on previous work that used epidemiological models, which introduced Saturated Incidence, Beddington-DeAngelis, and Crowley Martin models to simulate spatial heterogeneity in population-level models, we implemented these density-dependent models in within-host viral kinetics models to understand how …

A spatial multiscale mathematical model of Plasmodium vivax transmission.

The epidemiological behavior of Plasmodium vivax malaria occurs across spatial scales including within-host, population, and metapopulation levels. On the within-host scale, P. vivax sporozoites inoculated in a host may form latent hypnozoites, the activation of which drives secondary infections and accounts for a large proportion of P. vivax illness; on the metapopulation level, the coupled human-vector dynamics characteristic of the population level are further complicated by the migration of human populations across patches with different malaria forces of (re-)infection. To explore the int

Prediction of Dengue Incidence in the Northeast Malaysia Based on Weather Data Using the Generalized Additive Model.

INTRODUCTION: Dengue, a vector-borne viral illness, shows worldwide widening spatial distribution beyond its point of origination, namely, the tropical belt. The persistent hyperendemicity in Malaysia has resulted in the formation of the dengue early warning system. However, weather variables are yet to be fully utilized for prevention and control activities, particularly in east-coast peninsular Malaysia where limited studies have been conducted. We aim to provide a time-based estimate of possible dengue incidence increase following weather-related changes, thereby highlighting potential deng

Bladder Cancer Burden in the USA: Population Scenarios for 2040.

BACKGROUND AND OBJECTIVE: Bladder cancer is the sixth most common cancer among men and is expensive to manage. We independently developed three microsimulation models that describe its natural history and explain epidemiological trends. We projected bladder cancer burden in the USA through 2040 to inform workforce planning. METHODS: We calibrated the models to the Surveillance, Epidemiology and End Results (SEER) program incidence data and standardized key inputs. For White men, the highest-incidence subgroup, the models inferred unobservable epidemiological metrics, including lifetime risks

A Probabilistic Synthesis of Malaria Epidemiology: Exposure, Infection, Parasite Densities, and Detection.

The epidemiology of Plasmodium falciparum malaria presents a unique set of challenges due to the complicated dynamics of infection, immunity, disease, and detection. Studies of malaria epidemiology commonly measure malaria parasite densities or prevalence, but since malaria is so complex with so many factors to consider, a complete mathematical synthesis of malaria epidemiology has been elusive. Here, we take a new approach. From a simple model of malaria exposure and infection in human cohorts as they age, we develop random variables describing the multiplicity of infection (MoI) and the age

Improved Variable Selection Algorithm Using a LASSO-Type Penalty, with an Application to Assessing Hepatitis B Infection Relevant Factors in Community Residents.

OBJECTIVES: In epidemiological studies, it is important to identify independent associations between collective exposures and a health outcome. The current stepwise selection technique ignores stochastic errors and suffers from a lack of stability. The alternative LASSO-penalized regression model can be applied to detect significant predictors from a pool of candidate variables. However, this technique is prone to false positives and tends to create excessive biases. It remains challenging to develop robust variable selection methods and enhance predictability. MATERIAL AND METHODS: Two impro
