Preliminary study on ultrasound-based quantitative assessment of epiphyseal ossification angle for bone age evaluation in children.

BACKGROUND: Bone age (BA) assessment is crucial for evaluating pediatric growth, yet the standard radiographic method involves ionizing radiation. This study aimed to investigate the feasibility of a novel, quantitative ultrasound parameter-the epiphyseal ossification angle (OA)-as a non-invasive alternative for BA evaluation, and to examine its correlation with radiographically determined BA. METHODS: In this prospective study, 201 Chinese Han girls aged 5-14 years (mean age: 9.24±2.15 years) underwent both left wrist radiography and ultrasonography of the distal radius, distal ulna, and med

Computational mechanobiological model combining epiphyseal, apophyseal, and appositional growth and inner bone remodeling of the juvenile femur.

In silico models for simulating bone growth based on mechanical or non-mechanical epigenetic factors are widely used. In this study, a well-known mechanobiological model, which states that octahedral shear stress accelerates longitudinal bone growth and hydrostatic stress retards it, is applied to a finite element model of the femur of an 8-year-old boy. Proximal and distal epiphyseal plates as well as the growth plate of the greater trochanter, cartilaginous growth at the femoral isthmus, and appositional bone growth are included in the model. Furthermore, changes in the density of the cancel

Evaluation of femoro-epiphyseal acetabular roof index for assessing hip instability in hip disorders in female patients.

INTRODUCTION: The Femoro-Epiphyseal Acetabular Roof (FEAR) index is a proposed radiographic marker of hip instability, particularly in borderline developmental dysplasia of the hip (BDDH). Its diagnostic value in other hip morphologies and its association with patient-reported outcomes (PROMs) remain unclear. This study evaluated the FEAR index for assessing hip instability across different acetabular morphologies by analyzing its associations with radiographic measures obtained from standard anteroposterior pelvic and false-profile radiographs, dynamic ultrasonography, and PROMs. MATERIALS AN

Peroneal Nerve Dysfunction Following Vascularized Fibular Epiphyseal Transfer (VFET): A Prospective Case Series Study.

BACKGROUND: In skeletally immature individuals, vascularized epiphyseal transfer allows for reconstruction of complex bony defects while preserving longitudinal growth and providing a reliable articular surface for effective joint function. The shape and size of the proximal fibular epiphysis allow for gradual remodeling of the articular surface, facilitating favorable reconstruction and rehabilitation over time. Moreover, the proximal fibula is highly expandable, with less weight-bearing function, contributing to low donor-site morbidity. However, peroneal nerve dysfunction-related foot drop

Curdlan/chitosan NIR-responsive in situ forming gel: An injectable scaffold for the treatment of epiphyseal plate injury.

Premature closure of the epiphyseal plate inducing by the formation of bone bridges after epiphyseal plate injury, can lead to limb shortening and angular deformity, causing adverse effects on the growth and development of adolescents. Therefore, preventing the formation of bone bridges has become the primary task for children with epiphyseal plate fractures. In our study, a novel near-infrared (NIR)-responsive bone repair scaffold (CGCB), namely black phosphorus (BP)-loaded in-situ gel based on curdlan (CUD), β-glycerophosphate (GP) and chitosan (CS), was developed. In vitro studies confirmed

Exome Sequencing Identifies a Novel Splicing Variant in COL9A3 Resulting in Multiple Epiphyseal Dysplasia: A Case Report.

BACKGROUND: Multiple epiphyseal dysplasia (MED, OMIM #600969) is sometimes a mild skeletal dysplasia with diverse clinical findings, includi … initially suspected. Radiographs revealed sclerotic changes and fragments in the femoral and tibial epiphyses, suggesting destructive arthropathy. Genetic testing identified a COL9A3 variant (c.148-1G>C), as well as CTLA4 deficiency. The COL9A3 gene produces type IX collagen, and mutations in this gene can disrupt collagen folding or its interaction with ot …

Evaluating Skeletal Maturity at Time of Surgical Correction of Pectus Excavatum Based on Medial Clavicle Epiphyseal Ossification.

BACKGROUND: Surgical correction of pectus excavatum (SCOPE) is dependent upon chest wall pliability with optimal timing prior to complete skeletal maturation. Measures of skeletal maturity are not readily available for operative planning; therefore, surgeons use age as proxy despite patient-specific rates of skeletal maturation. We aimed to determine whether preoperative skeletal maturity is associated with postoperative pain as surrogate for chest wall pliability. METHODS: Children ≤18 years who underwent SCOPE from 2020 to 2022 were retrospectively identified. Preoperative CT within 3 month

Leptin accelerates BMSC transformation into vertebral epiphyseal plate chondrocytes by activating SENP1-mediated deSUMOylation of SIRT3.

Bone marrow mesenchymal stem cells (BMSCs) are capable of multidirectional differentiation, and engrafted BMSCs can be used to replace damaged chondrocytes for treatment of intervertebral disc disease. However, chondroblast differentiation of implanted BMSCs is inhibited by the anoxic environment of the articular cavity. Here, we found that leptin enhanced the transformation of BMSCs into chondrocytes under hypoxic conditions. BMSCs isolated from mice were cultured in medium supplemented with leptin under hypoxia. The expression of MFN1/2 and OPA1 were increased only in BMSCs cultured in an an

What Is the Association Among Epiphyseal Rotation, Translation, and the Morphology of the Epiphysis and Metaphysis in Slipped Capital Femoral Epiphysis?

BACKGROUND: Contemporary studies have described the rotational mechanism in patients with slipped capital femoral epiphysis (SCFE). However, there have been limited patient imaging data and information to quantify the rotation. Determining whether the epiphysis is rotated or translated and measuring the epiphyseal displacement in all planes may facilitate planning for surgical reorientation of the epiphysis. QUESTIONS/PURPOSES: (1) How does epiphyseal rotation and translation differ among mild, moderate, and severe SCFE? (2) Is there a correlation between epiphyseal rotation and posterior or

Epiphyseal Split Depression Tibial Plateau Fracture with Preservation of Physeal Growth in a Pediatric Patient: A Case Report.

CASE: A 14-year-old skeletally immature boy sustained a lateral split depression tibial plateau fracture without extension through the physis. He underwent open reduction internal fixation with all-epiphyseal instrumentation. CONCLUSION: Tibial plateau fractures that spare the physis are rare injuries, with no reports previously in the literature of operative treatment without physeal arrest. Careful all-epiphyseal reduction and fixation is a viable option with good-to-excellent outcomes and return to sporting activities.

Inactivation of Lkb1 in postnatal chondrocytes leads to epiphyseal growth-plate abnormalities and promotes enchondroma-like formation.

Liver serine-threonine kinase B1 (LKB1) is a tumor suppressor that has been linked to many types of tumors. However, the role of LKB1 in cartilaginous tumorigenesis is still poorly understood. In this study, we find that cartilage-specific, tamoxifen-inducible Lkb1 knockout results in multiple enchondroma-like lesions adjacent to the disorganized growth plates. We showed that chondrocytes retain an immature status caused by loss of Lkb1, which may lead to the dramatic expansion of growth-plate cartilage and the formation of enchondroma-like lesions. Additionally, increased mammalian target of

Endoscopic Surgery under Fluoroscopic Guidance Is Useful for Diagnosing and Treating Epiphyseal Osteomyelitis Caused by Mycobacterium Species.

Osteomyelitis caused by Mycobacterium species may be difficult to diagnose and treat. We report a case of treatment for osteomyelitis caused by Mycobacterium species in the epiphysis of the right proximal tibia. A 28-month-old boy presented to a hospital with symptoms of fever and right knee pain. He had been vaccinated with Mycobacterium bovis Bacille Calmette-Guérin (BCG) at five months of age. The epiphyseal radiolucent lesion had increased in size and extended to the metaphysis through the physis on a plain radiograph of the right proximal tibia. Surgical drainage and curettage of the lesi

Epiphyseal growth plate architecture is unaffected by early postnatal activation of the expression of R992C collagen II mutant.

Spondyloepiphyseal dysplasia (SED) exemplifies a group of heritable diseases caused by mutations in collagenous proteins of the skeletal system. Its main feature is altered skeletal growth. Pathomechanisms of SED include: changes in the stability of collagen II molecules, inability to form proper collagen fibrils, excessive intracellular retention of mutant molecules, and endoplasmic reticulum stress. The complexity of this pathomechanism presents a challenge for designing therapies for SED. Our earlier research tested whether such therapies only succeed when applied during a limited window of

Articulated Hip Distraction for Impingement of the Deformed Femoral Head in a Patient with Multiple Epiphyseal Dysplasia: A Case Report.

CASE: A 14-year-old boy with multiple epiphyseal dysplasia (MED) presented with right hip pain and stiffness. Radiographs revealed a deep notch at the superolateral part of the femoral head, which abutted the lateral corner of the acetabulum. Articulated hip distraction (AHD) was used to reduce the femoral head and obliterate the notch. At the 4-year follow-up, the radiographs showed a well-preserved joint space, and the patient had functional improvement. CONCLUSION: AHD may be an effective way to address and obliterate a superolateral femoral head notch and promote regeneration of the femor

Surgical correction of coxa vara: Evaluation of neck shaft angle, Hilgenreiner-epiphyseal angle for indication of recurrence.

BACKGROUND: Coxa vara is a hip deformity in which the femoral neck-shaft angle decreases below its normal value. Standard surgical treatment for this condition is corrective valgus osteotomy. Appropriate correction of the Hilgenreiner-epiphyseal angle is important to prevent recurrence. The purpose of this study is to: 1) evaluate the recurrence of the deformity at the latest follow up; and 2) find the appropriate angle of correction associated with the lowest recurrence. METHODS: 34 hips in 31 patients who underwent surgery for treatment of coxa vara from 2005 to 2014 were included. Patient-

New insights into the biomechanics of Legg-Calvé-Perthes' disease: The Role of Epiphyseal Skeletal Immaturity in Vascular Obstruction.

OBJECTIVES: Legg-Calvé-Perthes' disease (LCP) is an idiopathic osteonecrosis of the femoral head that is most common in children between four and eight years old. The factors that lead to the onset of LCP are still unclear; however, it is believed that interruption of the blood supply to the developing epiphysis is an important factor in the development of the condition. METHODS: Finite element analysis modelling of the blood supply to the juvenile epiphysis was investigated to understand under which circumstances the blood vessels supplying the femoral epiphysis could become obstructed. The
