Submaximal exercise metrics: Looking prior to VO₂max : in Heart Failure.

Maximal oxygen uptake (VO₂max) remains the benchmark for cardiopulmonary exercise testing (CPET) in chronic heart failure (CHF). However, VO₂max and its surrogate, VO₂peak, often fail to capture the full spectrum of functional impairment, especially in patients unable to reach maximal effort. This review delineates submaximal CPET-derived metrics that offer valuable prognostic and diagnostic insights beyond VO₂max and are less dependent on peak effort. Guideline-endorsed parameters such as ventilatory anaerobic threshold (VAT) and the minute ventilation–carbon dioxide production slope (VE/VCO₂

Cardiopulmonary Exercise Testing in Advanced Heart Failure Management.

Cardiopulmonary exercise testing (CPET) permits the assessment of gas exchange, electrocardiogram, and hemodynamic patterns throughout exercise, providing a window into multi-organ physiologic reserve during exercise. CPET provides risk stratification and informs management of advanced heart failure (HF). Increasingly, CPET is combined with echocardiography, or invasive right heart catheterization, which enables high-resolution assessment of cardiac and extracardiac limitations to exercise. CPET also represents a cornerstone in the evaluation process for advanced HF interventions. This review

The Invasive Cardiopulmonary Exercise Test: A Practical Guide.

The invasive cardiopulmonary exercise test (iCPET) provides a comprehensive, simultaneous evaluation of an individual's cardiovascular, respiratory, and metabolic response to exercise. The test is uniquely suited for the evaluation of exercise intolerance, as well as the deep phenotyping of disease states including pulmonary arterial hypertension and post-coronavirus disease symptomatology. Despite an expanding list of clinical and research applications, both the complexity of the test and a lack of familiarity with how the test is performed have been barriers to the widespread use of iCPET. Impaired physical ability in patients with transfusion-dependent β-thalassaemia: Can regular physical activity be a countermeasure?

Transfusion-dependent β-thalassaemia (TDβT) is a genetic disorder characterised by reduced or absent β-globin chain synthesis, resulting in chronic anaemia. Treatment consists of regular blood transfusions and chelation therapy to limit iron overload and its negative effects on organs (e.g. heart, lungs and liver). Despite improved life expectancy with transfusions and chelation therapy, TDβT patients experience poor quality of life and impaired exercise capacity (mainly due to muscle deconditioning and reduced cardiac inotropism). The aims of this review are (i) to discuss the role of organ d

Cardiopulmonary functional capacity in Taiwanese children with ventricular septal defects.

BACKGROUND: Children with ventricular septal defects (VSDs) are considered to have no difference in cardiopulmonary functional capacity with healthy children of the same age; however, studies have shown contradictory findings. The aim of this study was to assess whether Taiwanese children with VSDs exhibited cardiopulmonary deficits. METHODS: This is a retrospective cohort study with the data collected from January 2010 to December 2021. All patients and controls (age-, sex-, and body mass index -matched) underwent cardiopulmonary exercise testing (CPET) and pulmonary function test. RESULTS:

The motivation for physical activity is a predictor of VO2peak and is a useful parameter when determining the need for cardiac rehabilitation in an elderly cardiac population.

BACKGROUND: Exercise-based cardiac rehabilitation (CR) is an essential contributor to a successful recovery for elderly cardiac patients. The motivation for physical activity is a psychological parameter seldom described in secondary prevention, and it is plausible that motivation contributes to the differential effect of CR. PURPOSE: To investigate if motivation, measured using the behavioural regulation in an exercise questionnaire (BREQ-2), predicts VO2peak in elderly cardiac patients before and after CR. METHODS: A prospective cohort study of elderly ischemic cardiac patients and patient

Persistent Exertional Dyspnea and Perceived Exercise Intolerance After Mild COVID-19: A Critical Role for Breathing Dysregulation?

OBJECTIVE: After mild COVID-19, a subgroup of patients reported post-acute-phase sequelae of COVID-19 (PASC) in which exertional dyspnea and perceived exercise intolerance were common. Underlying pathophysiological mechanisms remain incompletely understood. The purpose of this study was to examine outcomes from cardiopulmonary exercise testing (CPET) in these patients. METHODS: In this observational study, participants were patients who were referred for the analysis of PASC after mild COVID-19 and in whom CPET was performed after standard clinical workup turned out unremarkable. Cardiopulmonary reserve examined with cardiopulmonary exercise testing in individuals with chronic kidney disease: A systematic review and meta-analysis.

BACKGROUND: Patients with chronic kidney disease (CKD) often present reduced physical activity and exercise tolerance due to factors relevant to co-existing disturbances of the cardiac, nervous and muscular systems. Cardiopulmonary exercise testing (CPET) is used for clinical evaluation of exercise limitation and related symptoms (i.e., dyspnea, fatigue) in several medical fields. OBJECTIVES: This is a systematic review and meta-analysis of studies using CPET technology to examine cardiopulmonary reserve in individuals with versus without CKD. METHODS: Literature search involved PubMed, Web …

Hemodynamics for the Heart Failure Clinician: A State-of-the-Art Review.

Heart failure (HF) fundamentally reflects an inability of the heart to provide adequate blood flow to the body without incurring the cost of increased cardiac filling pressures. This failure occurs first during the stressed state, but progresses until hemodynamic derangements become apparent at rest. As such, the measurement and interpretation of both resting and stressed hemodynamics serve an integral role in the practice of the HF clinician. In this review, we discuss conceptual and technical best practices in the performance and interpretation of both resting and invasive exercise hemodynam

Cardiopulmonary exercise testing in interstitial lung diseases and the value of ventilatory efficiency.

Interstitial lung diseases (ILDs) are diverse parenchymal pulmonary disorders, primarily characterised by alveolar and interstitial inflammation and/or fibrosis, and sharing pathophysiological similarities. Thus, patients generally harbour common respiratory symptoms, lung function abnormalities and modified exercise adaptation. The most usual and disabling complaint is exertional dyspnoea, frequently responsible for premature exercise interruption. Cardiopulmonary exercise testing (CPET) is increasingly used for the clinical assessment of patients with ILD. This is because exercise performanc

Residual Exertional Dyspnea in Cardiopulmonary Disease.

In cardiopulmonary medicine, residual exertional dyspnea (RED) can be defined by the persistence of limiting breathlessness in a patient who is already under the best available therapy for the underlying heart and/or lung disease. RED is a challenge to the pulmonologist because the patient (and the referring physician) assumes that the "lung doctor" should invariably provide a successful plan to fight the symptom. After presenting a simplified framework to understand the neurobiological underpinnings of dyspnea in cardiorespiratory disease, I discuss the seeds of RED associated with 1) increas

Approaches to improving exercise capacity in patients with left ventricular assist devices: an area requiring further investigation.

Introduction: Left ventricular assist device (LVAD) implantation has become a well-established treatment option for patients with end stage heart failure (HF) who are refractory to medical therapy. While LVADs implantation does effectively improve hemodynamic performance many patients still possess peripheral pathological adaptations often present in end-stage HF. Therefore, increased attention has been placed on investigating the effects of exercise training for patients with LVADs to improve clinical outcomes. However, the available evidence on exercise training for patients with LVADs is li

Gut dysbiosis is associated with the reduced exercise capacity of elderly patients with hypertension.

Hypertension is a global health issue, and a reduced exercise capacity is unavoidable for older people. According to recent clinical studies, the intestinal microbiota play a crucial role in the pathogenesis of many human diseases. We investigated whether specific alterations in the gut microbiota contribute to the reduced exercise capacity of elderly patients with hypertension. This study enrolled 56 subjects, and all patients performed a cardiopulmonary exercise test and underwent fecal bacteria sequencing (16 s ribosomal RNA V4 region). According to peak oxygen uptake values, patients were

Physical capacity in LVAD patients: hemodynamic principles, diagnostic tools and training control.

Over time left ventricular assist devices (LVAD) have become an alternative to heart transplantation because of enormous technical development and miniaturization. Most patients present a significant improvement in clinical conditions and exercise capacity. Nevertheless, exercise tolerance remains markedly limited even after LVAD implantation compared to a control group. The complex physiological and hemodynamic changes in LVAD patients, both at rest and during exercise, are not yet understood, or at least not completely. It is the aim of the present paper to describe the current state of scien…

Submaximal exercise intensities do not provoke variations in plasma magnesium concentration in well-trained euhydrated endurance athletes with no magnesium deficiency.

The purpose of this study was to assess the effect of exercise intensity during an incremental exercise test on plasma Mg concentration in well-trained euhydrated athletes. Twenty-seven well-trained endurance athletes carried out a cycloergometer test: after a warm-up of 10 min at 2.0 W·kg(-1), the workload increased by 0.5 W·kg(-1) every 10 min until exhaustion. Oxygen uptake (VO(2)), blood lactate concentration ([La(-)](b)), catecholamines, and plasma Mg were measured at rest, at the end of each stage and at 3, 5 and 7 minutes post-exercise. Urine specific gravity (U(SG)) was analyzed before

[Ergometry separates sleep apnea syndrome from obesity-hypoventilation after therapy positive pressure ventilation therapy].

In contrast to the obstructive sleep apnoea syndrome (OSA) the obesity-hypoventilation syndrome (OHS) is characterized by persistent hypercapnia during the day. After positive pressure ventilation (PPV) patients with OHS the daytime blood gases normalize after a short time. The aim of this study was to investigate whether blood gases at the end of a standardized exercise test separate both OSA and OHS after 3 months of PPV. Fourteen patients with OHS (12 males, 53.2 +/- 9.5 years, BMI: 41.7 +/- 9.6 kg/m2, PCO2: 50.7 +/- 4.5 mmHg) and 28 patients with severe OSA (27 males, 54.5 +/- 8.3 years, B
