Human fascioliasis is a new parasitic health problem in Egypt. This paper reports a case of ectopic fascioliasis in the right side of the pleural cavity of a farmer.

Sequential serum samples of 13 patients with acute farmer's lung (FL) taken during a follow-up of 18-36 months, were tested for antibodies of immunoglobulin G (IgG), IgA, IgM and IgE classes against Thermoactinomyces vulgaris and Micropolyspora faeni, and compared with contemporary lung function parameters. 113 lung specimens from rats and mice were observed under both LM and TEM, after inhalation of gonidiospores of Beauveria bassiana for 18 months 78.8% of the 113 cases developed chronic interstitial pneumonia (IP). A new medium, which we propose to call R8, was developed for the isolation and enumeration of the thermophilic actinomycete, Saccharomonospora viridis. This article reports on an epidemiological survey of the presence of farmer's lung among 1054 farmers who grind moldy hay. The prevalence was 8.3-11.4%. The main pathogen was Thermoactinomyces vulgaris. The factors which influenced farmer's lung were the moldy state of the hay and the dust concentration. The epidemiological features of farmer's lung are analyzed and discussed and certain preventive measures are suggested.

Data are presented on 9 subjects with Farmer's lung encountered in 1981-85. In all cases dyspnea arose within several hours after exposure to the allergen and in 4 cases only a few minutes after exposure. The radiological picture was altered in 7 cases (6 miliary pictures and 1 case of medial and basal nodulation). In functional terms 6 restrictive and 3 mixed forms were identified. The actinomycetes Micropolyspora faeni and Thermoactinomyces vulgaris are important sources of antigens in extrinsic allergic alveolitis, the deuteromycete Aspergillus fumigatus is it to a lesser extent. A 17-year-old boy demonstrated clinical and immunological signs of exogenic allergic alveolitis (farmer's lung). The patient grew up in the country and suffered from recurrent "pneumonia." A solid phase radioimmunoassay technique was used to demonstrate antibodies of various immunoglobulin classes in sera against farmer's lung antigens. IgG, IgA, and IgM antibody activity in normal patients and in patients with farmer's lung were detected against Micropolyspora faeni.
