Necrotizing fasciitis: an uncommon consequence of pressure ulceration.

Pressure ulcers may occur in patients with chronic illnesses, especially in those who are bed-bound or chair-bound. Local measures usually suffice to allow primary ulcer healing and support skin grafting or tissue transfer reconstruction. On rare occasions, however, pressure ulcers may progress to invasive infection and necrosis of adjacent soft tissues, possibly leading to necrotizing fasciitis. Early recognition and aggressive medical and surgical therapy are required to halt disease progression and prevent patient mortality. Two cases are presented to describe the severity of this soft-tiss

Cervicofacial necrotizing fasciitis. A devastating complication of blepharoplasty.

We report a case of cervicofacial necrotizing fasciitis that developed after blepharoplasty, an occurrence that, to our knowledge, has not previously been reported in the medical literature. A patient who presented to our institution 3 days after undergoing blepharoplasty of the upper eyelid was diagnosed as having fulminant fasciitis involving extensive areas of the face, scalp, and neck. We review the case in detail and discuss clinical and radiological clues to diagnosis, surgical and medical management, wound care, and subsequent scar contracture. This case emphasizes the need for individu

Necrotizing fasciitis: case study of a nursing dilemma.

Necrotizing fasciitis usually manifests as a low grade cellulitis that quickly deteriorates to a limb and life threatening soft tissue infection. Immediate surgical debridement is essential, after which wound management becomes the nurse's primary concern. Case #1 reports on a 72 year old female who, upon presenting to the ER with a "sore bottom," subsequently had these diagnoses: (1) anal-rectal abscess, (2) Fournier's gangrene, (3) ulcerative enterocolitis, (4) chronic blood loss/anemia, and (5) protein caloric malnutrition. After debridement, her anal-rectal wound extended from labia to lef

Periocular necrotizing fasciitis: a review of five cases.

OBJECTIVE: The purpose of the study is to display a spectrum of clinical presentations of periocular necrotizing fasciitis caused by group A streptococci and to discuss recent trends and treatment of this disease. DESIGN AND INTERVENTION: A case series of five patients (four female and one male) was seen between July 1990 and January 1995 in four university centers. All had clinical evidence of periocular necrotizing fasciitis and grew group A streptococci on wound cultures or had serologic evidence of streptococcal infection. Details of patient presentation, treatment, and outcome are examin

[Postoperative necrotizing fasciitis of the anterior abdominal wall].

Postoperative necrotizing fasciitis of the anterior abdominal wall is a serious and life-endangering complication of an acute progressive synergistic infective process. There is an absolute increase in its incidence rate attributable to a number of situations in modern life. Morphological and clinical studies are carried out on personal case material of 28 patients, followed up over a 3-year period. The presence of aerobic-anaerobic mixed polyinfection, consisting of average 3.75 bacterial species of which 1.43 aerobes and 2.32 anaerobes, is demonstrated microbiologically. Of the latter non-sp

Necrotizing fasciitis of the face without significant trauma.

Necrotizing fasciitis is a rare but well recognized clinical entity which most often occurs on the trunk, perineum or legs following surgery or trauma. The condition is much less common in the head and neck and it is particularly uncommon in the midface/periorbital region. In almost all the cases occurring in the neck the condition follows obvious dental or oropharyngeal sepsis and in all the cases of the scalp there is a history of previous surgery or trauma. However, necrotizing fasciitis of the midface/periorbital region may lack any obvious traumatic aetiology or may follow relatively mino

[Necrotizing fasciitis of the upper limb. 12 cases].

Twelve cases of necrotizing fasciitis or streptococcal cellulitis of the upper limb are reported. Four cases presented with a low grade aggressive for and one case was chronic. Seven fulminating cases resulted in two deaths. These different presentations are in fact different stages of the same disease which is a group A beta-hemolytic streptococcal necrotizing infection of the subcutaneous tissue. It is a medical emergency in which surgery is the main treatment. In cases seen early, surgery helps by making an early diagnosis by showing the typical appearance of the subcutaneous tissue and by

Rapidly progressive septic shock, associated with necrotizing fasciitis.

A case is presented of a 59-year-old male with rapidly progressive septic shock and necrotizing fasciitis. The patient was admitted in shock with an extensive skin lesion on the anterior chest wall. The history was relatively short and there was only a questionable history of preceding trauma. Necrotizing fasciitis was suspected from the appearance of the lesion. Antibiotics and anti-shock therapy were given but despite this, his condition deteriorated and he died from septic shock. At autopsy, the diagnosis of necrotizing fasciitis was confirmed. The importance of rapid diagnosis and primary

[Etiology and diagnosis of necrotizing fasciitis (author's transl)].

This is the report of three cases of necrotizing fasciitis, an acute and progressive disease involving the connective tissue, which may appear after infection with beta-hemolytic streptococci of group A (S. pyogenes). Painless ulcerations and necrosis along the fascial tissue are pathognomonic. Treatment consists in immediate and wide incisions to terminate the toxic effects due to the cell wall constituents of the streptococci together with an adequate chemotherapy. The laboratory diagnosis and the pathogenic mechanisms are discussed.

Treatment of necrotizing fasciitis caused by Staphylococcus epidermidis.

Postoperative necrotizing fasciitis with septicemia caused by Staphylococcus epidermidis was documented by cultures of the blood and wound biopsy specimen. Therapy consisted of surgical debridement, topical application of mafenide acetate dressings, and parenteral administration of cefazolin sodium. The combination effectively reversed the progression of infection and necrosis.

Factitial panniculitis and necrotizing fasciitis in juvenile diabetes.

A fifteen-year-old diabetic girl developed multiple inflammatory areas on the extremities and abdominal wall at the site of insulin injection. The bizarre clinical manifestations led to great difficulties in determining both its etiology and its treatment. However, three months after her initial hospitalization, we found that the lesions were probably secondary to self-injection with a chemical agent rather than a result of insulin allergy. If idiopathic recurrent dermal infection is observed, then factitial origin should be suspected.

Diagnostic performance of ultrasound for myositis and fasciitis, including necrotizing fasciitis, in pediatric patients.

OBJECTIVES: To demonstrate the diagnostic performance of ultrasound for abnormal magnetic resonance imaging findings in pediatric patients with suspected myositis and fasciitis. We compared sonographic findings between patients with and without necrotizing fasciitis. METHODS: In 21 pediatric patients, the presence or absence of abnormal T2-high signal intensity in subcutaneous fat, peripheral deep fascia, intermuscular fascia, and muscle was recorded. The extent of the disease based on ultrasound findings was recorded according to the previous locations. The vascular lumen within the affected

A case of necrotizing fasciitis initially misdiagnosed as cellulitis.

INTRODUCTION AND IMPORTANCE: Necrotizing Fasciitis (NF) is a life-threatening, rapidly progressive infection of the skin and underlying soft tissues. Bacterial pathogens induce a toxic-shock reaction that reduces vascular flow, causing thrombosis, sepsis, and tissue necrosis. Treatment consists of immediate IV antibiotics and oftentimes surgical intervention. We present a case of acute NF that was misdiagnosed as cellulitis. CASE PRESENTATION: A 17-year-old male was transferred to an emergency department from a rural hospital for further management of right lower extremity cellulitis and susp

Actinomyces europaeus as an emerging cause of necrotizing fasciitis.

Necrotizing fasciitis is a type of necrotizing soft tissue infection (NSTI) that can be polymicrobial or monomicrobial in origin. Polymicrobial infections typically involve anaerobes of the Clostridium or Bacteroides family. This case report highlights necrotizing fasciitis caused by an unusual culprit, Actinomyces europaeus, which is a gram-positive anaerobic filamentous bacillus that has only been documented in one prior report to cause NSTI. Currently, about half of the hospitals in the United States are equipped to perform antibiotic susceptibility testing for anaerobes, but less than one-
