The role of ventromedial prefrontal cortex in the recovery of extinguished fear.

Conditioned fear responses to a tone paired with footshock extinguish when the tone is presented repeatedly in the absence of shock. Rather than erase the tone-shock association, extinction is thought to involve new learning accompanied by inhibition of conditioned responding. Despite much interest in extinction from a clinical perspective, little is known about the neural circuits that are involved. Although the prefrontal cortex has a well established role in the inhibition of inappropriate behaviors, previous reports have disagreed as to the role of the ventromedial prefrontal cortex (vmPFC…

[Analysis of fear structure and intensity at women in prolonged pregnancy].

41 women in prolonged pregnancy were involved into anonymous researches. By the use of survey created in IVth Obstetrics and Gynaecological Department of Silesian Medical School and STAI Spielberger questionnaire and AS Cattell questionnaire and K test MPI Eyseneck Scale--the structure, level of intensity and crystallisation fear was examined. The expectation and wishes of women were known. The general psychological characteristic was done and dominant strategies of coping with difficult situations were isolated. Higher fear levels, mostly of reactive genesis and crystallised mainly on the pre…

Frequency and comorbidity of social phobia and social fears in adolescents.

This report presents findings on the frequency, comorbidity and psychosocial impairment of social phobia and social fears among 1035 adolescents, aged 12-17 years. The adolescents were randomly selected from 36 schools in the province of Bremen, Germany. Social phobia and other psychiatric disorders were coded based on DSM-IV criteria using the computerized Munich version of the Composite International Diagnostic Interview. Seventeen (1.6%) of the adolescents met the DSM-IV criteria for social phobia sometimes in their life. More girls than boys received the diagnosis of social phobia and the …

Evidence for a non-associative model of the acquisition of a fear of heights.

Theories that fear results from previous traumatic experience (i.e. conditioning theories) have enjoyed widespread support for over half a century. Recent research, however, has cast doubt on the validity of these models in some specific phobias. Two studies on the etiology of height phobia have obtained findings consistent with a non-associative, evolutionary explanation of fear acquisition (Menzies and Clarke, 1993a, Behaviour Research and Therapy, 31, 355-365; Menzies and Clarke, 1995a, Behaviour Research and Therapy, 33, 795-805). Unfortunately, the retrospective nature of these studies li…

No evidence for opioid-mediated analgesia induced by phobic fear.

The influence of anxiety on acute pain sensation was investigated, studying the relative contribution of endogenous opioids and attentional mechanisms. Thirty-six spider phobics received mildly painful electrical stimulation, while anxiety and focus of attention were manipulated within subjects. The opioid antagonist naloxone or placebo was administered between subjects to examine an analgesia owing to anxiety-induced endorphinergic activity. In contrast to earlier findings, attention towards pain failed to increase pain as opposed to distraction from pain, probably owing to a less effective a

Fear, anxiety and perceived control in children of agoraphobic parents.

Sixteen children (M = 11 years) of agoraphobic parents were compared with 16 children of parents with no history of psychopathology, matched on age, gender and socioeconomic status. The majority (68%) of children of agoraphobic parents met DSM-III-R diagnostic criteria, anxiety disorders being most common. They reported more fear and anxiety and less control over various risks than did comparison children. Group's perceptions of the prevalence and their vulnerability to these risks did not differ. Agoraphobic mothers reported more separation anxiety than did comparison mothers, and maternal se…

Obsessive-compulsive disorder and fear of AIDS contamination in childhood.

Two cases of obsessive-compulsive disorder (OCD) in prepubertal children are described in which the predominant obsessive ruminations relate to fear of acquired immune deficiency (AIDS) contamination. The findings in these cases are discussed in relation to known demographic and clinical profiles of OCD in children and adolescents. Fear of AIDS contamination in childhood OCD may reflect the effect of the current emphasis on AIDS in potentially vulnerable children with environmental influences giving the symptoms their specific content.

Phobias. How to help patients overcome irrational fears.

When phobias compromise a person's normal lifestyle or cause ongoing anxiety, they must be dealt with. Irrational, persistent fears may lead to isolation, depression, even substance abuse. What are the features of agoraphobia? Social phobia? Simple phobia? How common are these anxiety disorders? The authors answer these questions and discuss differential diagnosis and treatment options.

Sleep apnea following a pharyngeal flap: a feared complication.

Pharyngeal flaps are often used to correct velopharyngeal insufficiency. They produce a permanent partial obstruction of the velopharyngeal space. Respiratory obstruction and obstructive sleep apnea have been reported following this surgery. We undertook a study to find out the incidence of sleep apnea associated with pharyngeal flap surgery. Forty-one children (aged 2 to 22 years) admitted for a pharyngeal flap underwent a polysomnographic recording prior to their surgery. One child with Steinert's disease showed some episodes of obstructive apnea, and the surgery was canceled. Forty children

Fear of future illness, chemical AIDS, and cancerphobia: a review.

A review of the literature indicates that fear of future illness shares many characteristics with a wide variety of conditions. Concepts such as hypochondriasis, adjustment disorders, generalized anxiety, and mass hysteria must be considered. Also, factors such as reporting bias, influential others, collective behavior, political groups, and litigation appear to induce symptom reporting from patients complaining of fear of illness and cancerphobia. Cancerphobia may result in impairment ranging from avoidance behaviors to cognitive dysfunctions. Various treatment modalities have been used succe…

A cerebellar-vestibular explanation for fears/phobias: hypothesis and study.

To clarify and test the cerebellar-vestibular (CV) basis of fears/phobias, responses of 4000 learning disabled children, adolescents, and adults with neurological and electronystagmographic (ENG) evidence of CV-dysfunction were analyzed for anxiety-related symptoms. Of this sample, 64.6% indicated fears/phobias; females were significantly more predisposed; mixed-handedness was significantly related to fears of heights and reduced vestibular response or asymmetric vestibular functioning. Also, adults had a higher incidence of the specific fears/phobias characterizing agoraphobia than children a…

Fear of senility: the nurse's role in managing reversible confusion.

1. Acute illnesses in the elderly often cause reversible confusion. 2. Agitated and unsafe behavior is an attempt by the confused elderly to maintain self-image and physical and psychological integrity. 3. Ebersole and Hess' humanistic framework can guide the nurse to prioritize and meet the needs of the confused elderly.

Fear of obesity. A cause of short stature and delayed puberty.

We evaluated 201 children for short stature or delayed puberty or both. Fourteen of them (nine boys and five girls, aged 9 to 17 years) fit a pattern of growth failure due to malnutrition, which was the result of a self-imposed restriction of caloric intake arising from a fear of becoming obese. All 14 patients underwent a complete history, physical examination, diagnostic laboratory evaluation, and psychiatric assessment. They were all below the fifth percentile for weight, and 11 of them were also below the fifth percentile for height. The deficit of weight for height ranged from 5 to 23 per…

Interictal psychopathology in patients with ictal fear: a quantitative investigation.

We assessed the hypothesis that psychopathology is overrepresented in patients with temporal lobe epilepsy (TLE) who experience ictal fear. Eleven such patients were matched with two control groups: patients who had TLE without ictal fear and patients who had generalized epilepsy. On several MMPI scales, the TLE-ictal fear group scored significantly higher than the two control groups, which did not differ significantly from each other. Furthermore, the TLE-ictal fear group had more MMPI clinical scales that were pathologically elevated, had significantly more patients with histories of admissi…
