Reduced head-neck offset in nontraumatic osteonecrosis of the femoral head.

INTRODUCTION: Risk factors for nontraumatic osteonecrosis of the femoral head have in common that they trigger intravascular coagulation and thus lead to devascularization of the femoral head. In part of the patients, however, no risk factors seem to be evident. Mechanical reasons contributing to nontraumatic osteonecrosis have not been discussed so far. We hypothesized that recurrent traumatization of the vessels supplying the femoral head by a cam-type mechanism as in femoroacetabular impingement could add to intravascular coagulation. We, therefore, asked whether structural abnormalities at

Bilateral atraumatic femoral neck fractures resulting from transient osteoporosis of the hip.

A woman in her mid-30s presented to the orthopaedic team, unable to mobilise, shortly after her caesarean section. On questioning, she reported 10 weeks of atraumatic right hip pain. A radiograph revealed a displaced right subcapital neck of femur fracture. An MRI confirmed this, as well as identifying a minimally displaced left subcapital neck of femur fracture. She underwent a right total hip replacement and internal fixation of the left hip. A dual energy X-ray absorptiometry (DEXA) scan showed severe osteoporosis, and a diagnosis of transient osteoporosis of the hip was made. She was seen

Impact of postoperative haemoglobin on length of stay post fractured hip repair in patients with standardised perioperative management.

INTRODUCTION:: Perioperative blood transfusion is not without risk and effort should be made to limit patients' exposure to allogeneic blood. However, there is conflicting data regarding the impact of anaemia on postoperative recovery in patients with repaired hip fractures. It is hypothesised that for a given baseline functional status and fracture type, lower postoperative haemoglobin will increase rehabilitation time and prolong total length of hospital stay. METHODS:: This is a retrospective study on data collected prospectively on patients entered into the Clinical Pathway aged >65 years

Femoral neck fracture: a complication of femoral nailing.

A fracture of the shaft of the femur in adults is common after road traffic accidents. Until recently it was not known that a fracture of the femoral neck can occur while fixing the femoral shaft fracture with an intramedullary nail. We report three patients in whom femoral neck fractures occurred during femoral nailing. Fractures of the femoral neck were detected by routine postoperative radiographs of the hip joint. All the three patients were treated non-operatively and the fractures healed without complication. The cause of the femoral neck fracture seems to be forceful use of an awl in th

[On the biomechanics of the hip: relevance of femoral anteversion for hip contact force and loading using a short-stemmed prostheses].

Short-stemmed hip implants were established in total hip arthroplasty in the last years. Also patients with secondary osteoarthritis of the hip with pathological anteversion of the femoral neck are treated increasingly using this method. Therefore an investigation was performed to analyze the resulting hip contact force and femoral loading in the proximal femur at the solid model of the "standardized femur". Two different situations of femoral component anteversion were simulated. Increased hip contact forces and an increase of medial and lateral cortex loads result in the anteverted model. Wi

Unilateral late-onset tibia vara associated with bilateral proximal femoral growth disturbance in monozygotic twins: case report.

Clinical and histopathological similarities and rare association of Blount's disease with various proximal femoral physeal affections (i.e. adolescent coxa vara and slipped capital femoral epiphysis) are well known. Association of tibia vara with another epiphyseal disease of the proximal femur has not been reported previously. In this paper, a monozygotic set of twins with concordant bilateral epiphyseal growth disturbance of the proximal femur and unilateral late-onset tibia vara is presented. Radiological characteristics of the affected knees revealed a wedging in the proximal tibial epiphy

Unilateral proximal bifurcation of the femur.

We present a case report of a boy with a unilateral bifurcation of the proximal femur with a bifid femoral neck and 2 femoral heads. The patient had no other musculoskeletal or neurovascular abnormalities. One femoral head articulated with the acetabulum but the nonarticulating, larger head was better developed. The articulating head and neck were resected and the nonarticulating head was reduced. This hip then continued to develop well. This is the first report of such a phenomenon in the scientific literature.

Brodie's abscess of the femoral neck simulating osteoid osteoma.

Subacute osteomyelitis (Brodie's abscess) is essentially a problem of diagnosis, and there may be considerable difficulty in distinguishing it from other benign and malignant bone lesions. Though reported in the metaphyseal region of the femur, Brodie's abscess is rarer in the femoral neck. The authors present a case of Brodie's abscess in the femoral neck, which clinico-radiologically simulated an osteoid osteoma. Retrospectively, the presence of a cortical sinus tract should have aroused suspicion.

Biomechanical analysis of impending femoral neck fractures: the role of percutaneous cement augmentation for osteolytic lesions.

BACKGROUND: Management of impending pathologic femoral neck fractures includes internal fixation, arthroplasty and megaprostheses. The study aim was to determine the augmentative effect of cement injection for minimally invasive treatment of femoral neck lesions. METHODS: Twenty-seven cadaveric femora received a simulated osteolytic lesion previously shown to decrease the femur's failure load by 50%. Specimens were allocated to three groups of nine and loaded to failure in simulated single-leg stance: (1) percutaneous cementation + internal fixation (PCIF); (2) percutaneous cementation (PC);

An osteoporotic hip fracture in a 14-year-old girl undergoing chemotherapy and operated for knee osteosarcoma.

Non-weight-bearing, pre- and postsurgical immobilization, neoadjuvant and adjuvant chemotherapy are known to act on bone turnover, causing osteoporosis over short and long time periods. Treatment of fracture insurgence is very difficult because it really depends on being able to choose the right time (i.e., when immunodeficiency is less important). We report a case of spontaneous neck femur fracture during adjuvant chemotherapy in a young girl treated with resection and prosthesis reconstruction for distal femur osteosarcoma. Possible prevention and the correct approach and surgical timing are

[Further experiences with transplantation of the head of the fibula].

We are presenting our experience with vascularized epiphyseal transplantation of the upper end of the fibula, based distally on the anterior tibial artery in two patients, one suffering from septic epiphysitis of the hip with complete loss of the head and neck of the femur, the other suffering from radial club hand following septic loss of the radius, excluding its proximal and distal articular segments. In the first patient, the fibular epiphysis bridged the bone defect and provided growth at the neck of the femur. This restored hip stability, reduced limb-length discrepancy, initiated some d

Spontaneous femoral neck fracture complicating a healed subtrochanteric fracture--a case report.

A rare case of spontaneous femoral neck fracture complicating a healed subtrochanteric fracture was encountered in an 85-year-old female. The subtrochanteric fracture was caused by a minor trauma with no other combined injury. Patient received close reduction and internal fixation two days later and the fracture healed in three months. Six months after operation, spontaneous ispilateral femoral neck fracture occurred without trauma. Although there were some reports about subcapital fracture or femur neck fracture complication after operation, this is the first case of spontaneous femoral neck …

Periprosthetic Fractures in Long Versus Short Proximal Femoral Nailing for Intertrochanteric Fractures: A 10-Year Single-Centre Cohort Study.

Background Neck of femur (NOF) fractures, including intertrochanteric fractures, are common fragility fractures seen in the elderly population and are often amenable to fixation with a proximal femoral nail (PFN). However, there is conflicting evidence regarding the incidence of peri-prosthetic fractures with each device. Several studies from the 1990s and a recent meta-analysis have shown a higher incidence of peri-prosthetic fractures in the short PFN cohort. Other studies have shown a lower reoperation rate with short PFNs, and no statistically significant differences have been quoted in th

Varus derotation osteotomy for persistent dysplasia in congenital dislocation of the hip. Proximal femoral growth and alignment changes in the leg.

We studied the morphological changes in the proximal femur in 42 patients (42 hips) who had had varus derotation osteotomy of the upper femur for residual acetabular dysplasia and congenital subluxation of the hip and who did not show deformity of the head of the femur. In 19 patients the alignment of the whole leg was examined. The femoral neck-shaft angle (FNSA) at the final examination was unrelated to that immediately after operation or to the state of the acetabulum at that time. The postoperative FNSA was not related to the final result but the CE angle obtained at surgery influenced the

Mechanical effect of changed femoral neck ante-version angles on the stability of an intertrochanteric fracture fixed with PFNA: A finite element analysis.

OBJECTIVE: Change of femoral neck ante-version angle postoperatively due to inadequate reduction could result in unsatisfying treatment outcome of intertrochanteric fracture. However, the influence of increased or decreased femoral neck ante-version on the biomechanical stability of the bone-implant complex has rarely been studied. METHODS: A finite element model of a complete normal human femur with normal femoral neck ante-version as 13° was established accurately by scanning a 64 year old female femur. The models of 31-A1.1 intertrochanteric fractures with different femoral neck ante-versi

Functional morphology of trabecular system in human proximal femur: a perspective from P45 sectional plastination and 3D reconstruction finite element analysis.

BACKGROUND: The trabecular architecture of proximal femur plays a crucial role in hip stability and load distribution and is often ignored in hip fracture fixation due to limited anatomical knowledge. This study analyses trabecular morphology and stress distribution, aiming to provide an anatomical foundation for optimising implant designs. MATERIALS AND METHODS: Twenty-one formalin-fixed human pelvises (twelve male, nine female) were prepared using P45 sectional plastination. They were sliced into 3 mm sections in the coronal, sagittal, and horizontal planes and then photographed. A 3D femur
