Pathological pregnancies. Results of amniotic fluid studies and fetal outcome.

Late amniocenteses (greater than 20 weeks' gestation) were performed in 114 pregnancies with no a priori genetic risk, but referred because of abnormal clinical and/or ultrasound findings suggesting fetal malformations. Reasons for referral included polyhydramnios (51 cases), oligohydramnios (15 cases), fetal growth retardation (FGR) (16 cases) and abnormal fetal ultrasound findings excluding anencephaly (32 cases). In 42 of these cases, referral was motivated by a combination of the above abnormal findings. When polyhydramnios was the sole anomaly (25 cases), 5 fetuses were malformed (20%), a

Sinusoidal fetal heart rate pattern associated with congenital hydrocephalus. A report of 2 cases.

Two cases of hydrocephalus with an intrapartum sinusoidal heart rate pattern are presented. In the first patient, the diagnosis of toxoplasmosis during pregnancy and congenital hydrocephalus was made antenatally. Intrapartum the fetal heart showed intermittent periods of a sinusoidal pattern, alternating with a normal heart rate pattern. At birth there were signs of congenital toxoplasmosis. In the second patient there was a sinusoidal heart rate pattern associated with late decelerations and a loss of beat-to-beat variation. A brief review of the literature shows that the debate still continu

Fetal heart patterns. A study of the anencephalic fetus.

The anencephalic fetus is an interesting model for the study of the fetal heart rate (FHR). Such a fetus could serve as a model for the examination of the effect of "intracranial" pressure on the FHR, because its central nervous system (CNS) is directly exposed to intraamniotic pressure changes. The FHR was monitored in 10 anencephalic fetuses of normal, uncomplicated pregnancies and of gestational age between 35 and 42 weeks. Silent-pattern heart rate was observed in 6 of the monitored anencephalic fetuses. No other consistent heart rate pattern was obtained. A relatively high incidence of ex

Fetal breathing: its physiologic and clinical implications.

The respiratory movements the human fetus makes in utero can be detected and used as a valuable aid in determining fetal well-being. Their presence or absence, amplitude, and duration can all provide important information. Their absence, however, does not carry a poor prognosis unless other indices of fetal status, such as the nonstress test and oxytocin-challenge contraction-stress test, are also abnormal at the same time.

Fetal Non-invasive Electrocardiography Contributes to Better Diagnostics of Fetal Distress: A Cross-sectional Study Among Patients with Pre-eclampsia.

INTRODUCTION: Fetal distress is a result of acute or chronic disturbances in the system of "mother-placenta-fetus" in pre-eclampsia (PE). The aim of the investigation was to compare the accuracy of antenatal fetal distress diagnostics in cases of traditional cardiotocography (CTG) waveform evaluation and analysis of morphological non-invasive electrocardiogram (ECG) parameters in anterpartum patients with PE. MATERIALS AND METHODS: Fetal non-invasive ECG antenatal recordings of 122 pregnant patients at 34 to 40 weeks of gestation were examined. In Group I, there were 32 women with physiologic

Practice variations for fetal and neonatal congenital heart disease within the Children's Hospitals Neonatal Consortium.

BACKGROUND: Many aspects of care for fetuses and neonates with congenital heart disease (CHD) fall outside standard practice guidelines, leading to the potential for significant variation in clinical care for this vulnerable population. METHODS: We conducted a cross-sectional survey of site sponsors of the Children's Hospitals Neonatal Consortium, a multicenter collaborative of 41 Level IV neonatal intensive care units to assess key areas of clinical practice variability for patients with fetal and neonatal CHD. RESULTS: We received responses from 31 centers. Fetal consult services are share

Fetal bradycardia in the first trimester: an unusual presentation of atrial extrasystoles.

We report a fetus with fetal bradycardia at 13 weeks of gestation secondary to atrial extrasystoles. The fetus subsequently developed paroxysmal supraventricular tachycardia and hydrops fetalis. The cardiac arrhythmia recovered spontaneously without any medical intervention. This case illustrates that atrial ectopic beats can present in the first trimester with fetal bradycardia. Rapidly evolving hydrops fetalis secondary to supraventricular tachycardia can develop, warranting close monitoring with weekly heart rate assessment. Fetal bradycardia secondary to atrial extrasystole should be diffe

Computerized analysis of cardiotocograms in clinical practice and the SisPorto® system thirty-two years after: technological, physiopathological and clinical studies.

OBJECTIVES: The objective of this study is to present the why, what and how about computerized analysis of cardiotocograms (cCTG) and the Si … ante and intrapartum periods. The SisPorto system for cCTG closely follows the FIGO guidelines for fetal monitoring. It may be used both in the ante and intrapartum periods, alone or connected to a central monitoring station, with simultaneous monitoring of fetal and maternal signals, not only in singletons but also in twins. It has been assessed in technical, ph …

Rationale, study protocol and the cluster randomization process in a controlled trial including 40,000 women investigating the effects of mindfetalness.

BACKGROUND: Shortening pre-hospital delay may decrease stillbirth rates and rates of babies born with a compromised health. Stillbirth may be preceded by a decrease in fetal movements. Mindfetalness has been developed as a response to the shortcomings of kick-counting for the monitoring of fetal movements by the pregnant woman. We do not know if practicing Mindfetalness may diminish pre-hospital delay. Nor do we know if practicing Mindfetalness may increase or decrease the percentage of women seeking health care for unfounded, from a medical perspective, worry for her fetus' well-being. METHO

[Fetus, small for gestational age].

A small for gestational age foetus is defined by the foetal weight below the 10th centile for the corresponding gestational age. However, the vast majority of these cases has no apparent underlying abnormality, while in other cases a serious causative pathological condition can be identified. The detection, follow-up and treatment of an intrauterine growth retarded, compromised foetus has great obstetric and neonatologic relevance. In this review, the causes, clinical aspects and screening methods of intrauterine growth retardation are summarized based on the most recent international guidelin

[Spectral analysis of fetal heart rate variability].

OBJECTIVE: To assess power spectral densities and power spectrum of FHR variability in two categories of fetal activity, quiescence and breathing movements, in pregnancies complicated by diabetes mellitus or intrauterine growth retardation. MATERIAL AND METHODS: The study group included 70 women, 35 in uncomplicated pregnancies and 35 with above-mentioned complications. The spectral analysis of FHR variability in fetal quiescence or breathing activity was performed. The power spectrum was analyzed at frequencies 0-1.0 Hz. Successively, the power spectral density was calculated. It has been pr

Fetal heart rate patterns associated with periventricular leukomalacia.

OBJECTIVE: Our purpose was to assess the applicability of fetal heart rate (FHR) monitoring to detect fetuses at risk of developing periventricular leukomalacia (PVL). METHODS: FHR tracings obtained for babies delivered under 33 weeks' gestation and with a birth weight under 2000 g were assessed for baseline heart rate, variability, deceleration and "flip flap' (an oscillatory tracing pattern). RESULTS: PVL developed in 19 of the 103 infants studied. All of these infants were among the fetuses who exhibited average and increased variability. In addition, PVL was detected in 10 (47.6%) of the

Atrioventricular block during fetal heart rate decelerations.

Electrocardiograms (ECG) was examined in 15 fetuses during fetal heart rate decelerations in labour. Sinus bradycardia was demonstrated in six cases and in two cases inversion of the P wave was seen. In seven cases there was complete dissociation of the P wave from the QRS complex, indicating complete atrioventricular heart block. Many decelerations are vagally mediated, but such cases associated with complete atrioventricular heart block may be due to the effect of hypoxia on the bundle of His.

Fetal echocardiography. VII. Doppler color flow mapping: a new technique for the diagnosis of congenital heart disease.

One of the difficulties for the fetal sonographer is the complete elucidation of structural defects of the cardiovascular system that are associated with intracardiac or great vessel flow disturbances. With the recent introduction of Doppler color flow mapping, in which blood flow is displayed in color superimposed on a real-time image, it has been impossible to identify flow disturbances in the pediatric and adult patient. This study was undertaken to determine whether Doppler color flow mapping could be used in the fetus to identify normal and abnormal cardiovascular anatomy. Thirty-five nor

The prevalence of congenital abnormalities in patients with positive contraction stress tests.

Over a period of 10 years, 60,197 stress and nonstress tests were done in 25,313 patients. There were 229 (0.9%) positive tests. In 47.2% of patients who had positive tests, preeclampsia and hypertension were the indications for the test, followed by suspected intrauterine growth retardation (26.6%) (table I). The cesarean section incidence was 71%. The mean age of the patients was 32 years. Nine per cent of patients were 45 years or older, 7% between 40 and 44 years, 23% between 35 and 39 years, 30% between 30 and 34 years, 20% between 25 and 29 years, 7% between 20 and 24 years and 4% younge
