Endobronchial streptokinase has been used previously to dissolve blood clots caused by massive spontaneous hemoptysis, in settings including sarcoidosis, cavitary histoplasmosis, and multiple myeloma. To our knowledge, however, the use of thrombolytic agents to dissolve clots following transbronchial biopsy has not been reported previously. We describe a patient in whom endobronchial urokinase was used for successful dissolution of clots secondary to massive bleeding after transbronchial biopsy.

Securing the airway following surgery can be a problem in certain patients. Preparation, therefore, should include a plan for safe reintubation, if necessary. We report two patients with a difficult airway (recurrent thyroid carcinoma, inoperable carcinoma of the hypopharynx) who were at a high risk for intubation under general anaesthesia. In addition, one of the patients suffered from an acute abdomen and ileus. We used a guidewire placed into the trachea via a fiberscope to facilitate fiberoptic reintubation.

Plastic hollow fibers for the transmission of CO 2 laser energy in curved paths were produced by plating the inner surface of plastic tubes with a metal film and dielectric film upon it. These fibers can transmit high power up to 40 W at the outlet, with high transmission yield even through several bendings. A three-dimensional simulator was built to simulate paths in the dog's body and the outlet power was measured. The laryngeal mask airway (LMA) provides a patent airway when placed 'blindly' into the hypopharynx. We present the bacteriological findings in 329 aspirates from fiberoptic bronchoscopy. Records of 1,010 bronchofibroscopies were reviewed. Patients were examined with diagnostic and therapeutic indications, of which 6.8% were asthmatics. In most of them typical findings were seen signs of inflammation with thick, mucous secretions. In patients with status asthmaticus (0.6% of the analyzed population) bronchofibroscopy was carried out under life threatening conditions, the endoscopes being introduced through an intubation tube. Complications and changes in tracheal mucosa after minitracheotomy were evaluated in 28 patients. Tracheal mucosa was inspected fibreoptically after the insertion of a minitracheotomy cannula, and then at 3-day intervals until the cannula was removed. Thereafter, assessments were made every third day until the mucosa was considered normal. Three significant complications occurred: mediastinal puncture, paratracheal entrance of the cannula and subcutaneous emphysema. The results of screening for urinary tract tumors provided by the Republican Endoscopic Center are discussed. Groups at high risk for cancer were formed on the evidence of microhematuria. Cases of hematuria were referred to the endoscopic center for complex endoscopic, sonographic and morphological examination. Out of 575 patients at high risk, renal pathology was detected in 39.7% (tumors in 3.1% included) and urethral and bladder pathology--in 19.1% (benign and malignant tumors of the bladder in 4.4% included).

Massive upper gastrointestinal haemorrhage may be caused by ulcus simplex Dieulafoy. Of 144 patients consecutively operated on for benign gastric ulcer in 1975-1980, 29 were admitted as emergencies because of massive bleeding from the stomach. Six had the Dieulafoy lesion. Identification of the source of bleeding was made by fibreoptic endoscopy in five of these cases, while wide gastrotomy was required in one case. Patients who had benign polyps and cancers were studied retrospectively to define whether use of the 60-cm flexible sigmoidoscope would markedly improve the diagnostic yield over the 25-cm rigid sigmoidoscope. Of the polyps found, 48% could have been seen by the rigid sigmoidoscope and 85% by the flexible sigmoidoscope. Cancers were within reach of digital examination in 21%, of the rigid sigmoidoscope in 38% and of the flexible sigmoidoscope in 60%. The authors report the results of a retrospective study of 427 fiberoptic bronchoscopies in patients with haemoptysis. The patients were classified into 4 groups according to the results: Group I: a specific diagnosis was made (cancer, tuberculosis, infection); 151 cases (35 p. 100); Group II: inflammatory mucosa, 172 cases (40 p. 100); Group III: normal bronchoscopy, 84 cases (20 p. 100); Group IV, only blood, 20 cases (5 p. 100).
