The evolution of heart, lung and liver transplantation services in Australia.

The introduction and operation of heart, lung and liver transplantation services in Australia have been assisted by a Nationally Funded Centres policy adopted by health ministers. Assessment for eligibility for support under the program has been undertaken by the Australian Health Technology Advisory Committee. Outcomes/survival rates at transplantation centres have compared favourably with results from other countries. As in other countries, shortage of donor organs has limited the number of procedures undertaken. Some concern remains regarding the viability of newer transplantation programs

The Iranian model of living renal transplantation.

Organ shortage for transplantation remains a worldwide serious problem for kidney patients with end-stage renal failure, and several countries have tried different models to address this issue. Iran has 20 years of experience with one such model that involves the active role of the government and charity foundations. Patients with a desperate demand for a kidney have given rise to a black market of brokers and other forms of organ commercialism only accessible to those with sufficient financial resources. The current Iranian model has enabled most of the Iranian kidney transplant candidates, i

Private capital in a public field: a qualitative study of philanthropic influence in Germany's global health governance.

BACKGROUND: International philanthropic foundations have become prominent actors in global health, influencing funding priorities, institutional partnerships, and governance practices at a scale that increasingly rivals many state actors. While their role in multilateral organisations and low- and middle-income countries (LMICs) has been widely debated, less is known about how these actors operate within high-income donor countries themselves. Over the past two decades, Germany has emerged as a key contributor to global health initiatives and has developed close working relationships with inte

From Hurricane Irma to the Grindavík eruptions: volatility premiums in disaster governance.

Environmental volatility can inflate property values even as it destroys them. To show how, this article pairs a postcolonial micro-state in the Caribbean (Sint Maarten after Hurricane Irma) with a Nordic welfare town (Grindavík in Iceland following volcanic eruptions) because they occupy the opposite ends of the governance capacity spectrum, while sharing certain similarities. Drawing on reports, media archives, 31 ethnographic interviews, and transaction data from 2015-25, it traces four mechanisms that convert danger into a 'volatility premium': distressed asset acquisition; symbolic risk b

Informal payments and the illusion of free care: a perspective from Somalia's fragile health system.

Universal Health Coverage (UHC) promises access to care without financial hardship, yet in many low- and middle-income settings this ideal is undermined by pervasive informal payments. This perspective introduces the concept of the "illusion of free care" to explain how official fee-exemption policies coexist with hidden, off-the-record charges that reshape access and equity. Drawing on Somalia as a critical case of health system fragility, we argue that informal payments are not isolated acts of corruption but predictable responses to chronic underfunding, weak governance, and workforce preca

Green gospel effect of regional financial expansion: evidence from urban commercial banks in China.

The regional financial expansion represented by the development of regional small and medium-sized banks, such as urban commercial banks, is an essential factor affecting the environmental behavior of enterprises. We found that both the marginal expansion and the scale expansion of regional finance help reduce the sulfur dioxide emission intensity and improve the environmental performance of enterprises, indicating a green gospel effect of regional financial expansion. In terms of the impact path, regional financial expansion cannot only reduce the sulfur dioxide generation intensity and impro

Czech Republic: health system review.

This analysis of the Czech health system reviews recent developments in organization and governance, health financing, health-care provision, health reforms and health system performance. The Czech health-care system is based on compulsory statutory health insurance providing virtually universal coverage and a broad range of benefits, and doing so at 7.7 % of GDP in 2012 - well below the EU average - of which a comparatively high 85 % was publicly funded. Some important health indicators are better than the EU averages (such as mortality due to respiratory disease) or even among the best in th

[An innovative system for analysis and further development of long-term care for older people - results of the European PROject INTERLINKS].

In the light of recent demographic changes, the continued development of structures of long-term care (LTC) for older people is needed across Europe. In Germany, as in many other European countries, existing provisions of LTC are neither adequately coordinated nor user-oriented. The integration of relevant institutions and actors constitutes a central challenge for a high quality of care. The European research project INTERLINKS asked how the interfaces within fragmented structures of LTC are currently managed. Thematic foci included quality assessment and assurance, embedding preventive and r

Delivering the EU Safe Hearts Plan: the essential role of laboratory medicine in cardiovascular health.

The European Union Safe Hearts Plan provides an opportunity to strengthen cardiovascular prevention, early detection, treatment, and monitoring across Europe. This EFLM position paper argues that laboratory medicine must be recognized as a central component of its implementation. Reliable and harmonized laboratory data underpin cardiovascular risk assessment, diagnosis, therapeutic guidance, longitudinal monitoring, and evaluation of population outcomes. Beyond the minimum health-check dataset, tiered and personalized pathways should integrate established biomarkers, specialist testing, and va

Public health in a fracturing world: the erosion of the international rules-based order.

One of the greatest threats to public health today is not any single disease, environmental hazard, or conflict, but the erosion of the international rules-based order that enables countries to cooperate in addressing shared risks. Over the past decade, public health has faced increasingly interconnected challenges, including climate change, conflict, pollution, emerging infectious diseases, food and water insecurity, mass displacement, and misinformation. These threats are amplified by fragile governance, growing inequality, and declining trust in institutions. Public health depends fundament

Towards resilient and agile health systems: lessons from abrupt donor withdrawal in Jordan.

BACKGROUND: Donor-funded health programmes strengthen national quality systems in low-income and middle-income countries, but abrupt withdrawal of external support can create institutional gaps and threaten service sustainability. AIM: To analyse the impact of sudden donor funding cessation, using the experience of the Health Care Accreditation Council (HCAC) as an illustrative case, and to explore implications for quality infrastructure and health system resilience. METHODS: This structured case-based policy analysis draws on institutional documentation, administrative records and publicly

Global Access to Comprehensive Care for Paediatric and Congenital Heart Disease.

Paediatric and congenital heart disease (PCHD) is common but remains forgotten on the global health agenda. Congenital heart disease is the most frequent major congenital anomaly, affecting approximately 1 in every 100 live births. In high-income countries, most children now live into adulthood, whereas in low- and middle-income countries, over 90% of patients do not get the care they need. Rheumatic heart disease is the most common acquired cardiovascular disease in children and adolescents. While almost completely eradicated in high-income countries, over 30-40 million people live with rheum

Human African trypanosomiasis: an emerging public health crisis.

There is a dramatic resurgence of human African trypanosomiasis (HAT) in sub-Saharan Africa. T.b. gambiense is spreading epidemically in large areas of Central Africa, especially the Southern Sudan, Congo-Zaire, Angola, Uganda and the Central African Republic. Devastating epidemics of T.b. rhodesiense have occurred in south-eastern Uganda. The causes of the re-emergence of sleeping sickness as a public health problem include widespread civil disturbance and war, declining economies, reduced health financing and the dismantling of disease control programmes. Despite the inevitably fatal outcome

Closing the Surveillance-to-Stewardship Gap in antimicrobial resistance: a narrative review of global lessons from WHO GLASS, UK ESPAUR, and low- and middle-income countries.

BACKGROUND: Antimicrobial resistance (AMR) surveillance has expanded faster than any other component of the global response, yet resistance continues to rise. In a growing number of settings, the principal constraint is the failure to translate available data into action, a disconnect we describe as the Surveillance-to-Stewardship (S2S) Gap. OBJECTIVES: To define the S2S Gap and its component gaps, examine where it binds across health systems of different maturity, analyse why it persists, and propose an operationalisable, testable framework for closing it. SOURCES: A cross-sectional analysi

Do subsidies reach the sick? Implementation challenges and equity implications of pharmaceutical subsidy reform in Iran: a qualitative study.

BACKGROUND: Pharmaceutical subsidy reforms are commonly implemented to improve access to essential medicines, strengthen financial protection, and enhance the sustainability of health systems. In Iran, the Darooyar policy was introduced to shift pharmaceutical subsidies from subsidized foreign exchange allocation toward consumer-oriented insurance coverage. Despite its policy importance, limited evidence exists regarding the implementation challenges of this reform from the perspective of health system stakeholders. METHODS: This qualitative study was conducted using an inductive thematic ana

Revisiting Afghanistan's Drug Policy: A Policy Analysis of Eradication, Harm Reduction, and Economic Dependencies.

BACKGROUND: Afghanistan's responses to illicit drugs have oscillated between punitive eradication and limited harm-reduction initiatives. Two decades of heavy external spending, entrenched conflict, and an economy intertwined with opium have blunted policy effectiveness. METHODS: Guided by Walt and Gilson's policy-triangle, we undertook a document-based review (2001-2024). Twenty-seven national laws, strategies, and analytical reports were retrieved from government and multilateral repositories. Data were coded thematically using Braun-and-Clarke's six-phase approach to map policy content, ac
