Focal splenic lesions in patients with AIDS: sonographic findings.

BACKGROUND: The purpose of this study was to describe the sonographic features of the focal splenic lesions in patients with AIDS and to know the frequency and etiology of these features. METHODS: Sonographic exams of 278 AIDS patients were reviewed retrospectively. We recorded the clinical indications for sonograms and sonographic findings of those patients with focal splenic lesions. In addition, patients' histories were reviewed to determine the etiology of such lesions. Ultrasound exams were performed with a 3.5-MHz transducer. RESULTS: Sonography demonstrated focal splenic lesions in 22

Prevalence and risk factors of seizure clusters in adult patients with epilepsy.

PURPOSE: In the current study, we explored the prevalence of physician-confirmed seizure clusters. We also investigated potential clinical factors associated with the occurrence of seizure clusters overall and by epilepsy type. METHODS: We reviewed medical records of 4116 adult (≥16years old) outpatients with epilepsy at our centers for documentation of seizure clusters. Variables including patient demographics, epilepsy details, medical and psychiatric history, AED history, and epilepsy risk factors were then tested against history of seizure clusters. Patients were then divided into focal e

Early formative stage of human focal cortical gyration anomalies: fetal MRI.

OBJECTIVE: Limited information is available about the development of focal cortical gyration anomalies in the human brain. Using prenatal MRI, we characterized focal cortical gyration anomalies at an early formative stage and sought clues about the mechanisms of their development. MATERIALS AND METHODS: From a large prenatal MRI database, 30 cases (gestational age, ≤ 24 weeks) with reported focal distortion of the cortical rim profile were selected. Eight cases were matched with histologic examinations; another seven had prenatal MRI, MRI autopsy, or postnatal MRI follow-up; and 15 had no fol

Complicated Brucellosis With Multisystem Involvement: Two Case Reports.

Brucellosis is a zoonotic infection caused by facultative intracellular bacteria of the genus Brucella. The ability of the organism to invade both phagocytic and non-phagocytic cells and survive in the intracellular environment makes brucellosis a systemic infection that can affect various organs of the body. Complications of brucellosis occur when the infection involves one or more focal body sites. Early identification of complications of brucellosis and initiation of appropriate treatment is the key to a better outcome. Here we present two cases of complicated brucellosis, both having multi

Human immunodeficiency virus-associated nephropathy mainly due to cellular variant of focal segmental glomerulosclerosis.

A 45-year-old man with a low titer of hepatitis B virus (HBV) was diagnosed with nephrotic syndrome. A subsequent test for human immunodeficiency virus (HIV) was positive. Kidney biopsy revealed some signs of collapsing variant of focal segmental glomerulosclerosis (FSGS), but the predominant finding was a cellular variant of FSGS. Two years after receiving tenofovir, urine protein became negative, and the patient was finally diagnosed with HIV-associated nephropathy. Collapsing variant of FSGS is considered typical of HIV-related nephropathy, but the cellular variant of FSGS in this patient r

Diabetic ketoacidosis secondary to dento-alveolar infection.

Diabetic ketoacidosis (DKA) is a medical emergency with a potentially fatal outcome if not recognized and treated appropriately. Infective processes are a common precipitant of DKA. We report two cases of dentoalveolar infections in patients with type I diabetes mellitus who presented with DKA. The management of such cases requires both specialist surgical and medical intervention.

Focal eosinophilic proctitis with associated rectal prolapse in a pony.

Focal intramural nodules were palpated in the rectal wall of a 12-year-old pony mare presented for rectal prolapse. Eosinophilic proctitis was diagnosed by examination of fine needle aspirates and biopsy of the largest rectal nodule. After treatment with a course of corticosteroids, the rectal nodule and accompanying peripheral eosinophilia resolved. There was no recurrence of the condition during the follow-up period of 20 months. Focal eosinophilic proctitis appeared to be an unusual cause of tenesmus and rectal prolapse in this case.

[Pleural empyema as a complication of descending necrotizing mediastinitis].

Fulminant infections of oropharyngeal origin may cause the most lethal form of mediastinitis due to extension of the infection along the neck fascial planes or by lymphatic way: descending necrotizing mediastinitis. Two cases of this rare life-threatening entity of odontogenic and tonsillar origin are reported. A pleural empyema and pericarditis as complications were observed. Bacteriologic etiology, possibilities of the early diagnosis and the aggressive treatment by means of a thoracotomy are discussed.

[The prognostic significance of natural killers and dehydrogenases of the peripheral blood lymphocytes in patients with a suppurative inflammation].

In patients with purulent inflammation foci of various location, reduction in the number of natural killers (NK) and in the activity of alpha-glycerophosphatedehydrogenase (alpha-GPDH) and adenosinetriphosphatase (ATP-ase) of lymphocytes was revealed. Maximum reduction in indices of NK, alpha-GPDH and ATP-ase was noted in patients with diffuse peritonitis, phlegmonous-gangrenous cholecystitis, destructive appendicitis. After treatment, the mentioned indices increased, but by the time of discharge of the patients from the hospital they were lower than the normal ones.

[The bacterial associations in maxillofacial actinomycosis].

A variety of clinical forms of actinomycosis, difficulties of its early diagnosis and particularly of the differential diagnosis between this condition and tuberculosis, nocardiosis, aspergillosis necessitate the development of present-day methods for the early laboratory diagnosis of actinomycosis. Basing on the data on present-day methods for isolation of anaerobic and microaerobic diphtheroids, the authors characterize the bacterial associations occurring in maxillofacial actinomycosis and other odontogenic processes, discuss problems in isolation and identification of Actinomyces cultures.

Mediastinitis occurring as a complication of odontogenic infections.

Mediastinitis occurring from a descending odontogenic infection occurs rarely. The diagnosis is sometimes difficult to make and is often delayed until the patient is in extremis. The physical examination is often nondiagnostic, but may include brawny edema of the neck and chest. CT scanning provides the most accurate diagnostic information. The treatment is always surgical, in combination with appropriate antibiotics. Five patients with mediastinitis secondary to odontogenic infections are presented, and the technique of transcervical drainage of the anterior and posterior mediastinum is revie

Early detection of bacteremia in an outpatient clinic.

A total of 570 patients, aged less than 24 months, who were seen in the emergency room at Texas Children's Hospital for evaluation of acute febrile illness had a WBC count, differential count, and a blood culture obtained. Bacteremia occurred in 7.7% (n = 44) (22 Haemophilus influenzae; 17 Streptococcus pneumoniae; three Escherichia coli; one group B Streptococcus; one Staphylococcus aureus). The sensitivity, specificity, and positive predictive value of morphologic changes (vacuolization and toxic granulation) of polymorphonuclear neutrophils (segmented neutrophils plus band cells), WBC count
