The etiology of painful papule of the external ear, chondrodermatitis nodularis chronica helicis, has been an enigma for over half a century. I reviewed 84 skin biopsy specimens from 82 patients with this histopathologic diagnosis and found evidence that the infundibular portion of the hair follicle is probably the primary cutaneous structure involved in this condition. This lesion appears to evolve from an acute, suppurative, granulomatous dermatitis into a later-fibrosing dermatitis having the clinical and histologic features of perforating folliculitis and prurigo nodularis.

Minocycline is a tetracycline agent frequently used for acne therapy. It has a few rare but severe side effects that are not widely known but should be recognized early as drug related. These include acute hepatitis and liver failure; a Löffler-like syndrome with pulmonary infiltrates, wheezing, fever, and eosinophilia; skin eruptions, eosinophilic cellulitis, and pustular folliculitis with eosinophilia; and a lupuslike syndrome. Side effects that are better known and recognized include photosensitization, skin exanthema with pruritus, and pseudotumor cerebri.

One hundred and one patients who met criteria for alcoholism were evaluated for dermatologic signs. There were 98 men and 3 women, with an average age of 40.85 years; 40 were white and 61 black. Significant findings were no skin disease in 19, acne in 27, folliculitis in 8, seborrheic dermatitis in 12, tinea pedis in 32, and xerosis in 7. Only two patients had rosacea, which is contrary to previously held beliefs about the drinker's nose.

Deucravacitinib is a novel oral selective tyrosine kinase 2 inhibitor used to treat plaque psoriasis. The most common cutaneous adverse effects are acne and folliculitis. We present a series of seven patients with prototypical papulopustular rosacea (PPR) following commencement of deucravacitinib. Although most of our patients demonstrated improvement in psoriasis on deucravacitinib, all seven patients discontinued therapy because of the severity and impact of PPR. Rosacea can be a severe adverse effect of deucravacitinib, impacting tolerability and influencing future treatment options.
