The Evaluation of Radiologic Angular Measurements in Patients with Foot Pain.

BACKGROUND: The aim of this study was to investigate the relationship between foot deformities by comparing foot radiographs of patients with complaints of foot pain with those of healthy individuals. METHODS: The study included 30 patients with pes cavus, 30 patients with pes planus, 30 patients with calcaneal spur, and 30 controls aged 30 to 60 years. All participants underwent measurement of right and left foot length; metatarsophalangeal width; and calcaneal pitch (CA), talohorizontal (TA), talometatarsal (TM), and lateral talocalcaneal (LTC) angles from lateral radiographs. RESULTS: The…

A real-world multicenter cross-sectional observational study to assess the clinical profile of peripheral neuropathy in patients with diabetes.

BACKGROUND: There are limited studies on the prevalence of diabetic peripheral neuropathy (DPN) and related foot deformities in patients with T2DM from India. AIM: To investigate the prevalence, characteristics, and risk factors for foot deformities in Asian-Indian individuals with T2DM and DPN. METHODS: We analyzed 4290 patients (32.3% female, 67.7% male, mean age 51.1 ± 9.3 years) using a cross-sectional, retrospective observational method, focusing on signs and symptoms of foot complications and neuropathy. RESULTS: Dry Skin (44%), infection (19.7%), and ingrown toenails (16.6%) were the…

Is Subtle Cavovarus a Problem for Athletes?

Cavovarus or high-arched foot is a common foot deformity that occurs due to the disruption of the foot-driven equilibrium between the first metatarsal, fifth metatarsal, and the heel. This imbalance leads to an increase in the foot's normal plantar concavity. Cavovarus deformity ranges from a mild and flexible malalignment to a fixed, complex, and severe deformation. Subtle cavovarus foot, the mild form of the cavus foot, was first described by Manoli and colleagues.

New insights into the pathophysiology of pes cavus in Charcot-Marie-Tooth disease type 1A duplication.

Forefoot pes cavus is a cardinal sign of Charcot-Marie-Tooth disease (CMT). This review is focused on the pathophysiology of pes cavus in CMT1A duplication, which is the most common subtype of the disease. Assessment of foot deformities in CMT1A, their prevalence and proposed mechanisms, and recent contributions of magnetic resonance imaging studies of lower-leg and foot musculature are revised. Special attention is given to papers on foot deformities at initial stages of the disease. We conclude that pes cavus is an early and age-dependent manifestation of CMT1A duplication. Selective denerva…

What's New in Severe Deformity Correction: The German Perspective.

Foot deformities are found in several neurologic conditions, most typically, but not exclusively, Charcot-Marie-Tooth disease. Posttraumatic deformities and undercorrection or overcorrection of congenital talipes equinovarus are also encountered. A severely deformed foot that cannot fit into normal shoes presents a significant day-to-day challenge to the young and active patient. This article presents some basic principles for evaluating the deformity and a toolkit of procedures to deal with these complex cases.

Complex reconstruction of the diabetic foot and ankle.

Reconstruction of the diabetic foot presents significant challenges for the surgeon. The goals of treatment are correction of deformity as well as elimination of infection, with production of a stable, plantigrade foot. Certainly not all patients with Charcot deformities require reconstruction. Many can be effectively managed with proper shoe modifications with orthoses. Bracing is also effective in providing support and in preventing further deformity. Surgical treatment is indicated for patients with recurrent ulceration or an unstable foot. Reconstruction of the Charcot foot should eliminat

Outpatient assessment and management of the diabetic foot.

Patients with diabetes and peripheral neuropathy are at risk for foot deformities and mechanical imbalance of the lower extremity. Peripheral neuropathy leads to an insensate foot that puts the patient at risk for injury. When combined with deformity due to neuropathic arthropathy, or Charcot foot, the risks of impending ulceration, infection, and amputation are significant to the diabetic patient. Education of proper foot care and shoe wear cannot be overemphasized. For those with significant malalignment or deformity of the foot and ankle, referral should be made immediately to an orthopedic…

The Cavovarus Ankle: Approaches to Ankle Instability and Inframalleolar Deformity.

Pathologic affects from a cavus foot deformity range from flexible subtle to rigid severe deformities and are related to many pathologic conditions of the foot and ankle. Understanding the underlying deformity and the deforming force is essential in treating the cavus ankle and foot. Every deformity is different and unique to a given patient; therefore, surgical plans should be modified to each patient.

Pediatric Foot: Development, Variants, and Related Pathology.

Pediatric foot development throughout childhood and adolescence can present a diagnostic dilemma for radiologists because imaging appearances may be confused with pathology. Understanding pediatric foot development and anatomical variants, such as accessory ossification centers, is essential to interpret musculoskeletal imaging in children correctly, particularly because many of these variants are incidental but others can be symptomatic. We first briefly review foot embryology. After describing common accessory ossification centers of the foot, we explain the different patterns of foot matura

[Grice's method of subtalar arthrodesis--long term results of 63 operations].

Deformities of the back foot should be operated after failed conservative treatment. In this article long time results of 36 patients are demonstrated, which are operated on with extraarticular blocking (Grice-OP) of the lower ankle. Most patients are suffering of cerebral palsy. Walking could be improved after surgery but we could not reduce orthopaedic devices. In relation to Spina bifida patients CP-patients showed worse correlation of the back foot valgus. We got better results using k-wires for stabilisation of the back foot. Grice-OP is able to prevent severe deformities but it is often

Analytical radiography of club feet.

A radiographic method is described for making the diagnosis of talo-navicular subluxation before ossification of the navicular bone occurs. Seven basic combinations of deformities are thought to occur in the club foot. Radiography before operation enables the surgeon to determine which of these combinations exists in a particular foot, and radiography during operation enables him to determine wheter or not all the existing deformities have been corrected, and therefore whether or not the operation must be extended.

Shoe modification and the use of orthoses in the treatment of foot and ankle pathology.

Shoe modification and foot orthoses can play an important role in the nonsurgical management of foot and ankle pathology. Therapeutic footwear may be used to treat patients with diabetes, arthritis, neurologic conditions, traumatic injuries, congenital deformities, and sports-related injuries. These modalities may improve patient gait and increase the level of ambulation. They also may be used to treat acute problems such as plantar fasciitis or metatarsalgia and as preventive tools in patients with diabetic neuropathy. Shoe selection is primarily based on the condition of the patient, the foo

Extensor Digitorum Brevis Transfer Technique to Correct Multiplanar Deformity of the Lesser Digits.

UNLABELLED: Lesser digital deformities may present a surgical challenge to even the most skilled foot and ankle surgeon. Multiplanar toe deformities, including the crossover toe, are especially difficult to correct with reproducible results. Undercorrection, pain, stiffness, and recurrent deformity are well reported throughout foot and ankle literature. The goal of this article is to describe a method of correcting digital deformity utilizing the extensor digitorum brevis tendon transfer and a biotenodesis screw. The controlled tension established with the extensor tendon provides the necessar

Congenital talipes equinovarus and congenital vertical talus secondary to sacral agenesis.

Sacral agenesis is a rare congenital defect which is associated with foot deformities such as congenital talipes equinovarus (CTEV) and less commonly congenital vertical talus (CVT). We report a 3-year-old Caucasian girl who was born with right CTEV and left CVT secondary to sacral agenesis. Her right foot was managed with a Ponseti casting method at 2 weeks, followed by an Achilles tenotomy at 4 months. The left foot was initially managed with a nocturnal dorsi-flexion splint. Both feet remained resistant and received open foot surgery at 10 months producing plantigrade feet with neutral hind…

Neuropathic Deformities of the Lesser Toes.

Neuropathic deformities of the lesser toes refer to a variety of neuromuscular disorders in which a stiff lesser toe deformity is encountered. Claw toes, hammer toes, and mallet toes can be found. Each deformity is better described as which joint of a particular toe is in a flexed or extended position. This information guides the surgeon to which joint should be addressed and what additional soft tissue procedure may be needed. Interphalangeal joint fusions are the procedure of choice, together with extensor or flexor tenotomies. Good results can be expected, with low revision rates.
