Elbow, forearm, wrist, and hand injuries among sport rock climbers.

OBJECTIVES: Sport rock climbing with its repetitive high-torque movements in gaining the ascent of a rock face or wall, often in steep overhanging positions, is associated with a unique distribution and form of upper limb injuries. In this article, we review the biomechanical aspects of sport rock climbing and the types of injuries commonly encountered in the forearm, wrist, and hand regions of elite sport rock climbers. Because elbow, forearm, wrist, and hand injuries predominate, representing 62% of the total injuries encountered, these anatomical areas have been selected for review. DATA S…

Reduction of forearm fractures in children using axillary block anesthesia.

One hundred eleven patients (age range 15 months-16 years, mean 9.6 years) with displaced forearm fractures who had axillary blocks performed by orthopaedists in the emergency room were retrospectively reviewed. Thirty-seven fractures involved the distal radius, 56 involved both bones of the forearm, and 12 involved the physis of the distal forearm. Three Monteggia fractures, one radial neck fracture, one olecranon with an associated radial neck fracture, and one distal radius fracture with an associated elbow dislocation also were included. Of the 111 axillary blocks, 105 were rated as effect…

Posterior interosseous reverse forearm flap: experience with 80 consecutive cases.

The results of an anatomic investigation performed in 40 fresh cadaver specimens and 80 consecutive clinical cases of the posterior interosseous reverse forearm flap are reported. It was observed that there is a choke anastomosis between the recurrent dorsal branch of the anterior interosseous artery and the posterior interosseous artery at the level of the middle third of the posterior forearm. Ink injections through a catheter placed in the distal part of the anterior interosseous artery stained the distal and middle thirds of the posterior forearm, but the proximal third remained unstained;…

Dual free flap transfer using forearm flap for mandibular reconstruction.

To reconstruct a composite mandibular defect, we have simultaneously transferred a vascularized bone graft or osteocutaneous flap together with a forearm flap. The radial forearm flap, being thin, pliable, and having a long vascular stalk, served as mucosal lining and/or an interpositional flap acting as a vascular bridge. Between 1982 and 1989, we used this procedure in 17 patients with a mandibular defect or deformity which developed following treatment of oral cancer. Our clinical experience has demonstrated that this dual free tissue transfer has many advantages. It is useful for obtaining…

The radial forearm--flexor carpi radialis myocutaneous flap: case report.

A patient sustained a compound fracture of the elbow resulting in a deep soft tissue defect with exposed bone, joint and nerve. The flexor carpi radialis (FCR) muscle was incorporated within the radial forearm island flap to cover the defect. Wrist flexion was not compromised by this procedure. The blood supply and applications of the radial forearm--flexor carpi radialis myocutaneous flap are described.

Pedicled radial forearm flaps for recalcitrant defects about the elbow.

Chronic, open wounds about the elbow frequently require flap coverage. Skin grafts, local transposition flaps, muscle transposition flaps, and two-staged pedicled flaps from the chest and abdomen as well as free tissue transfers have all been reported. Recalcitrant elbow defects in patients in whom these previous flaps have failed are especially challenging to the upper extremity reconstructive surgeon. We have successfully employed pedicled radial forearm flaps in three patients in whom one or more previous flaps have failed.

Cross-union complicating fracture of the forearm. Part II: Children.

Cross-union is a rare complication of fractures of the forearm in children. Of the ten cross-unions reported in this series, four were Type 2 (middle third or non-articular part of the distal third of the radius and ulna) and six were Type 3 (proximal third of the forearm). There were no Type-1 (intra-articular part of the distal third of the radius and ulna) cross-unions. As in adults, the Type-2 cross-unions followed a high-energy injury. Three of the four Type-2 cross-unions were excised; one of them recurred. Type-3 cross-unions occurred equally after closed or open reduction of a fracture…

Routine Forearm Cast Splitting in Pediatric Patients Following Closed Reduction.

Introduction Routine cast splitting following closed reduction of pediatric forearm fractures is commonly performed to accommodate swelling and reduce cast-related complications. However, evidence supporting routine splitting remains conflicting. This study aimed to compare cast-related complications between split and non-split casts and assess the associations of cast splitting, fracture displacement, and gap index with clinical outcomes. Materials and methods A retrospective cohort study was performed of children aged two to 16 years with forearm fractures initially managed with cast immobil…

Structural balance restoration in complex forearm problems using 3D technology.

INTRODUCTION: The human forearm is a finely tuned system in which the radius and ulna act as an interdependent pair to enable stability, mobility and efficient load transfer. Disruption through trauma, malunion, congenital deformity or failed surgical intervention produces imbalance, presenting with restricted motion, instability, pain, and functional loss. Advances in three-dimensional (3D) imaging, planning, and printing now allow for more accurate analysis and more precise correction of forearm deformities.Reconstruction:Restoration of bone morphology alone does not ensure functional recove…

Delayed pancreatic metastasis from forearm mesenchymal chondrosarcoma: a diagnostic challenge.

Mesenchymal chondrosarcoma is a rare, high-grade malignant tumour comprising 2-4% of all chondrosarcomas, with pancreatic metastasis being exceptionally uncommon. A woman in her late 20s, with a history of forearm mesenchymal chondrosarcoma treated 5 years ago presented with jaundice, weight loss and generalised pruritus. Ultrasound followed by CT revealed a calcified pancreatic mass with surrounding vascular encasement, initially raising the possibility of pancreatic adenocarcinoma. A biopsy revealed the lesion to be a metastatic mesenchymal chondrosarcoma based on histomorphology and immunop…

Fractional Lengthening of Forearm Flexor Tendons: A Cadaveric Biomechanical Analysis.

PURPOSE: Multiple procedures have been described for wrist and finger flexion contractures and spasticity. Fractional lengthening of forearm flexor tendons involves making parallel transverse tenotomies at the musculotendinous junction to elongate the muscle. Currently, there is limited literature to define the biomechanical consequences of this lengthening technique. METHODS: Forty-eight flexor tendons were harvested from eight paired upper limbs including flexor carpi radialis, flexor carpi ulnaris, flexor pollicis longus, and flexor digitorum superficialis tendons. Each tendon that was len…

Derotational Corrective Osteotomy for Pediatric Forearm Malunion: A Case Report.

In this case report, we describe an eight-year-old boy with both-bone forearm diaphyseal fracture that was treated conservatively after closed reduction with manipulation. Nine months after the injury, he returned to consultation, presenting a rotational deformity of the forearm and 20° of pronosupination limitation. He was submitted to corrective osteotomies, using three-dimensional (3D) planning and templating, using a double approach, and fixation with a four screw holes plate in each osteotomy. At one year of follow-up, there are signs of bone consolidation and complete recovery of range o…

Optimal flow volume measurements in forearm versus arm arteriovenous fistulas.

OBJECTIVES: Successful haemodialysis is dependent on optimal arteriovenous (AV) access flow. Although 600 ml/min is frequently quoted as the critical level for functional flow volume (Qa) according to the National Kidney Foundation guideline, this may not be applicable for the different configurations of AV fistulas (AVF) or AV grafts (AVG). This study evaluates ultrasound derived Qa measurement in the inflow brachial artery to autologous AVF in the forearm radiocephalic and arm brachiocephalic/basilic configurations in relation to significant flow related AV dysfunction. METHODS: Five hundre…

Refracture Following Operative Treatment of Pediatric Both Bone Forearm Fractures.

INTRODUCTION: Both bone forearm fractures are common injuries in children. Most can be treated with reduction and casting. Those that fail nonoperative management can be treated with a plate or intramedullary fixation; however, refracture remains a problem. The goal of this study is to evaluate the refracture rate in both bone forearm fractures based on the mode of fixation. METHODS: Institutional board review approval was obtained. A retrospective chart review from 2010 to 2021 at a single tertiary care institution was conducted for all operative patients <18 years who sustained a both bone …

Prescribing braces after forearm fractures does not decrease refracture rate.

Refracture is one of the most common complications of pediatric forearm fractures. One way to decrease this risk is to extend immobilization with a brace after the cast has been removed to allow for a range of motion exercises. The purpose of this study was to examine whether prescribing a brace after casting was discontinued decreased the risk of refracture. A retrospective, cohort study was performed at one level I trauma center. Girls under 10 years and boys under 12 years who sustained a forearm fracture from January 2013 to December 2018 were included. Patients with open fractures, fractu…

Complex Reconstruction of a Mangled Forearm Following Washing Machine Trauma.

Automatic washing machine injuries are more commonly associated with minor injuries in the pediatric population but may cause life and limb-threatening adult injuries in rare instances. This case describes a severe upper extremity injury after a schizophrenic patient placed her arm into a running machine. Herein, we describe the management, complex reconstruction, and repair of radial, ulnar, and metacarpal fractures in addition to transected tendons and vasculature. The patient had an excellent functional outcome with minor restrictions in motion and complete recovery of sensation.
