A complicated orthopaedic injury known as a comminuted distal femur fracture occurs when the lower portion of the femur bone breaks into several pieces close to the knee joint. Distal femur fractures are more commonly caused by injuries to the supracondylar and intercondylar regions. Managing comminuted distal femur fractures is a significant clinical challenge. The age of the patient, the condition of the bone, any concomitant injuries, and the level of comminution all influence the treatment plan. Handling distal femur fractures that have comminuted presents a major clinical challenge. One o

BACKGROUND: Previous studies assessing surgical fixation of osteoporotic proximal humeral fractures have primarily focused on medial calcar support. In this study, we utilized a specific model for 2-part surgical neck fracture of the osteoporotic proximal humerus to investigate how severe comminution of the greater tuberosity (GT) lateral wall affects biomechanical stability after fixation with a plate.
METHODS: Ten matched pairs of cadaveric humeri (right and left) were assigned to either a surgical neck fracture alone (the SN group) or a surgical neck fracture with GT lateral wall comminuti

PURPOSE OF THE STUDY: Fractures of the proximal radius constitute a significant proportion of elbow joint injuries, representing approximately one-third of all such fractures and 75% of proximal forearm fractures. In adults, they account roughly for 4% of all fractures. Treatment of these fractures varies depending on their severity. No uniform guidelines have been published to date for the treatment of severely comminuted fractures.
MATERIAL AND METHODS: The study included 16 patients who underwent on-table reconstruction, with a mean age of 49 years. Half of the cases involved Mason type II

BACKGROUND: Many types of projectiles, including modern hollow point bullets, fragment into smaller pieces upon impact, particularly when striking bone. This study was performed to examine the effect on time to union with retained bullet material near a fracture site in cases of gunshot injury.
METHODS: All gunshot injuries operatively treated with internal fixation at a Level 1 Trauma Center between March 2008 and August 2011 were retrospectively reviewed. Retained bullet load near the fracture site was calculated based on percentage of material retained compared to the cortical diameter of

The results of primary Bateman-Leinbach bipolar prosthetic replacement for comminuted intertrochanteric fractures of the hip in the elderly are examined. In an effort to avoid the postoperative complications seen in open reduction and internal fixation of severely comminuted fractures with osteoporotic bone, and to avoid postoperative restrictions when fixation is suboptimal, a group of 58 patients were treated with a bipolar Bateman-Leinbach prosthesis. They were followed for an average of 28 months. Surgery was performed using an anterolateral approach, which is recommended. A detailed descr

Treatment of fractures of the proximal phalanx and metacarpals is based on the presentation of the fracture, degree of displacement, and difficulty in maintaining fracture reduction. A wide array of treatment options exists for the variation in fracture patterns observed. Inherently stable fractures do not require surgical treatment; all other fractures should be considered for additional stabilization. In general, of the many combinations of internal fixation possible, Kirschner wires and screw-and-plate fixation predominate. Early closed reduction typically is successful for unicondylar frac

The management of victims of gunshot fractures is a challenge for surgeons, mainly due to the complexity of the injury itself and degree of bone comminution. The titanium functionally dynamic bridging plate has often been used in reconstructive surgery after mandibular fractures. The classic indication for bridge plate use is comminuted fractures with or without associated substance losses. The aim of this article is report the case of gunshot victim and to discuss the bridge plate technique utilized for treatment. A 21 years-old, melanoderma, male patient was victim of a firearm-related assau

INTRODUCTION: The current standard surgical approach for treating displaced transverse or simple patellar fractures is the tension band wiring technique with Kirschner wires. The use of cannulated screw with modified augmented cerclage wire (c-wire) has a potential to be an alternative use for this fixation especially in complex cases.
PRESENTATION OF CASE: This case series reported four patients with patellar fractures treated with cannulated screws and modified augmented c-wire with a follow-up for at least twelve months of follow-up. There were two cases of two-part transverse fractures, o

Tendon adhesions in zone IV after proximal phalangeal fractures are common and may lead to loss of range of motion at the proximal interphalangeal joint. The type of fracture, surgical technique and rehabilitation strategy also influence the final functional outcome. Plate fixation is a reliable solution in cases of comminuted phalangeal fracture. This article describes how adhesions between the plate and extensor apparatus in cases of comminuted fractures of the proximal phalanx can be reduced by using an adipofascial flap.

Coronal plane articular shear fractures of the distal humerus are a rare injury, comprising 6% of distal humerus fractures. The fracture can be easily missed on initial radiographs; therefore, a high index of suspicion is warranted. Radiographically obvious fractures are often more complex than initially appreciated, with plain radiographs often underestimating the extent of posterior comminution and trochlear involvement. Associated bony and ligamentous injuries are common. Anatomic reduction and internal fixation of displaced fractures is the standard of care because poor results have been d

Facial fractures have formerly been classified solely by anatomic location. CT scans now identify the exact fracture pattern in a specific area. Fracture patterns are classified as low, middle, or high energy, defined solely by the pattern of segmentation and displacement in the CT scan. Exposure and fixation relate directly to the fracture pattern for each anatomic area of the face, including frontal bone, frontal sinus, zygoma, nose, nasoethmoidal-orbital region, midface, and mandible. Fractures with little comminution and displacement were accompanied by subtle symptoms and required simple

OBJECTIVES: To characterize the location and frequency of the fracture lines and comminution zones of both-column fractures using fracture mapping.
METHODS: Both-column fractures were retrospectively reviewed in 4 Level 1 trauma centers. Mimics software was used to reconstruct the both-column fractures and simulate the fracture reduction. Then, the fracture lines and the intra-articular and extra-articular comminution zones were drawn on a 3-dimensional innominate bone template. The distribution of fracture lines and fracture fragments were also mapped on the two-dimensional template of the a
